Diffusion-weighted MRI of breast lesions: a prospective clinical investigation of the quantitative imaging biomarker characteristics of reproducibility, repeatability, and diagnostic accuracy.

Diffusion-weighted MRI (DWI) provides insights into tissue microstructure by visualization and quantification of water diffusivity. Quantitative evaluation of the apparent diffusion coefficient (ADC) obtained from DWI has been proven helpful for differentiating between malignant and benign breast lesions, for cancer subtyping in breast cancer patients, and for prediction of response to neoadjuvant chemotherapy. However, to further establish DWI of breast lesions it is important to evaluate the quantitative imaging biomarker (QIB) characteristics of reproducibility, repeatability, and diagnosti

Shear Wave Elastography May Be Superior to Greyscale Median for the Identification of Carotid Plaque Vulnerability: A Comparison with Histology.

PURPOSE: There is a need to develop methods that reliably quantify characteristics associated with vulnerable carotid plaque. Greyscale median (GSM) and shear wave elastography (SWE) are two techniques that may improve individual plaque risk stratification. SWE, which quantifies Young's Modulus (YM) to estimate tissue stiffness, has been researched in the liver, breast, thyroid and prostate, but its use in carotid plaques is novel. MATERIALS AND METHODS: The aim of this study was to quantify YM and GSM of plaques and compare to histology. 25 patients (64% male) with a mean age of 76 underwent

Feasibility of diffusion-weighted magnetic resonance imaging in patients with juvenile idiopathic arthritis on 1.0-T open-bore MRI.

OBJECTIVE: To evaluate the feasibility of non-invasive diffusion-weighted imaging (DWI) of the knee of children with juvenile idiopathic arthritis (JIA) and, further, to analyze the apparent diffusion coefficient (ADC) levels to distinguish synovium from effusion. MATERIALS AND METHODS: Standard magnetic resonance imaging of the knee including post-contrast imaging was obtained in eight patients (mean age, 12 years 8 months, five females) using an open-bore magnetic resonance imaging system (1.0 T). In addition, axially acquired echo-planar DWI datasets (b-values 0, 50, and 600) were prospect

Elastic modulus of the prostate: a new non-invasive feature to diagnose bladder outlet obstruction in patients with benign prostatic hyperplasia.

The purpose of our study was to develop a reliable method for the non-invasive evaluation of bladder outlet obstruction (BOO) caused by benign prostate hyperplasia (BPH). In our study, the International Prostate Symptom Score was assessed in, and trans-rectal ultrasound (TRUS) and shear wave sonoelastography (SWE) were performed on, 55 patients with BPH who had undergone urodynamic evaluation (the gold standard diagnostic procedure for BOO). The results indicated that the elastic modulus of the transitional zone was the indicator most strongly correlated with BOO stage (r = 0.666, p < 0.001),

Perfusion deficits detected by arterial spin-labeling in patients with TIA with negative diffusion and vascular imaging.

BACKGROUND AND PURPOSE: A substantial portion of clinically diagnosed TIA cases is imaging-negative. The purpose of the current study is to determine if arterial spin-labeling is helpful in detecting perfusion abnormalities in patients presenting clinically with TIA. MATERIALS AND METHODS: Pseudocontinuous arterial spin-labeling with 3D background-suppressed gradient and spin-echo was acquired on 49 patients suspected of TIA within 24 hours of symptom onset. All patients were free of stroke history and had no lesion-specific findings on general MR, DWI, and MRA sequences. The calculated arter

Automated quantification of caudate atrophy by local registration of serial MRI: evaluation and application in Huntington's disease.

OBJECTIVE: Caudate atrophy rate measured from serial MRI is proposed as a biomarker of HD progression that may be of use in assessing putative disease-modifying agents. Manual measurement techniques are the most widely applied but are time-consuming. We describe and evaluate an automated technique based on a local registration and boundary shift integral (BSI) approach at the caudate-CSF and caudate-white matter boundaries; caudate boundary shift integral (CBSI). METHODS: Two-year caudate volume change was measured in controls, premanifest HD and early HD using the CBSI and compared with a de

Malignant lesion segmentation in contrast-enhanced breast MR images based on the marker-controlled watershed.

Breast tumor volume measured on MRI has been used to assess response to neoadjuvant chemotherapy. However, accurate and reproducible delineation of breast lesions can be challenging, since the lesions may have complicated topological structures and heterogeneous intensity distributions. In this article, the authors present an advanced computerized method to semiautomatically segment tumor volumes on T1-weighted, contrast-enhanced breast MRI. The method starts with manual selection of a region of interest (ROI) that contains the lesion to be segmented in a single image, followed by automated se

Coxa magna quantification using MRI in Legg-Calve-Perthes disease.

BACKGROUND: The idiopathic avascular necrosis of the femoral epiphysis characterizes the Legg-Calve-Perthes disease in pediatric osteoarticular pathologies. The coxa magna, more frequently observed, corresponds to an enlargement and deformation of the femoral head. The volume extension induces a subluxation of the hip, which is a bad prognosis for the healthy function of the joint. METHODS: The aim of the study was to quantify the coxa magna in Legg-Calve-Perthes disease using magnetic resonance imaging. Twenty-five patients with unilateral Legg-Calve-Perthes disease were included in the clin

Comparison of similarity measures for the task of template matching of masses on serial mammograms.

We conducted a study to evaluate the effectiveness of twelve different similarity measures in matching the corresponding masses on temporal pairs of current and prior mammograms. To perform this comparison we implemented each of the twelve similarity measures in the final stage of our multistage registration technique for automated registration of breast lesions in serial mammograms. The multistage technique consists of three stages. In the first stage an initial fan-shape search region was estimated on the prior mammogram based on the geometrical position of the mass on the current mammogram.

Compressed Sensing Technology Accelerated 3D-FLAIR MRI Sequence for Endolymphatic Hydrops at 3T.

BACKGROUND AND PURPOSE: 3D-FLAIR sequence has an important contribution to the display of endolymphatic hydrops (EH) in Meniere disease (MD), but its clinical application is limited because of the long acquisition time. We investigated whether 3D-FLAIR combined with compressed sensing (CS) technology (3D-FLAIR-CS) can shorten the scan time while maintaining the image quality and diagnostic efficiency for EH. MATERIALS AND METHODS: This prospective study included 50 patients with unilateral definite MD who underwent 3T MR imaging 4 hours after gadolinium injection using traditional 3D-FLAIR (1

Imaging of the optic chiasm and retrochiasmal visual pathways.

The exploration of the chiasmal and retrochiasmal visual pathways is based on magnetic resonance imaging. A bitemporal hemianopsis suggests a lesion of the optic chiasm while homonymous lateral hemianopsis should lead to a search for a lesion of the retrochiasmal visual pathways. The causes of chiasmal impairment are mainly tumoral. The exploration protocol is based on MRI with T1-weighted sagittal sections, then T2- and T1-weighted coronal sections with and without injection. In case of a retrochiasmal syndrome, the MRI exploration protocol is a function of the type of occurrence of the defic
