[The re-evaluation of the vasodepressive component in the carotid sinus syndrome].

BACKGROUND: The evaluation of the vasodepressor (VD) reflex of the carotid sinus syndrome is usually inaccurate, due to the difficulty in blood pressure measurement. AIM: To study the VD reflex with a beat-to-beat not invasive technique. METHODS: We investigated 68 patients (49 males, 19 females; mean age 70 +/- 11 years) affected by carotid sinus syndrome: cardioinhibitory (CI) form was present in 47 patients, mixed (M) form in 10 patients and VD form in 11 patients. The control group consisted of 9 patients (6 male, mean age 71 +/- 8 years) affected by third degree atrioventricular block w…

Identifying the Hiccup-Induced Esophagogastric Waveform: A Case Series Report Using High-Resolution Manometry.

Hiccups result from involuntary contractions of the diaphragm, driven by a complex neuromuscular reflex. Three patients with persistent hiccups underwent esophageal high-resolution manometry during hiccup episodes, revealing a consistent finding: sustained contraction of the esophagogastric junction with intermittent pressure peaks. This pattern, termed the "Hiccup-Induced Esophagogastric Waveform," shows significant esophageal pressure changes linked to hiccup reflex. It may reflect a compensatory mechanism to expel excess esophageal residue or gas. These findings suggest hiccups could exacer…

Vasovagal Syncope during Office Hysteroscopy-A Frequently Overlooked Unpleasant Complication.

Due to technological advancements in miniaturization of instruments and improved optics, the number of office hysteroscopic procedures has increased over time. Office hysteroscopy is preferred due to avoidance of general anesthesia and decreased overall cost. Vasovagal syncope has been implied as the most common complication. Vasovagal syncope is associated with inappropriate reflex vasodilation and bradycardia in the setting of an acute malfunction between the autonomic nervous system and the cardiovascular system; however, there is no mortality associated with vasovagal syncope. A management…

Clinical application of vestibular evoked myogenic potential (VEMP).

The author reviewed clinical aspects of vestibular evoked myogenic potentials (VEMPs). Now two types of VEMPs are available. The first one is cervical VEMP, which is recorded in the sternocleidomastoid muscle and predominantly reflects sacculo-collic reflex. The other is ocular VEMP, which is usually recorded below the lower eye lid and predominantly reflects utriculo-ocular reflex. VEMPs play important roles not only for assessment of common vestibular diseases but also for establishment of new clinical entities. Clinical application in Meniere's disease, vestibular neuritis, benign paroxysma…

Hypoglossal-facial nerve anastomosis: a clinical and electrophysiological follow-up.

Eight patients with acoustic neuroma and five patients with hemifacial spasm, who had undergone hypoglossal-facial nerve anastomosis 1-14 years previously, were reviewed clinically and electrophysiologically with an electrically elicited blink reflex. Functional recovery from the anastomosis, as rigorously judged on a scale of good, fair and poor, was fair to poor. Electrically it was found that the blink reflex was present in eight patients, suggesting facial nerve re-innervation of the facial musculature. This was confirmed by the mild recurrence of hemifacial spasm in four patients. Implica…

Amitriptyline-induced ophthalmoplegia.

Total external ophthalmoplegia, unresponsive to caloric stimulation, was observed in a gravid woman who had ingested approximately 1.0 to 1.5 gm of amitriptyline. The intravenous administration of 4.0 mg physostigmine salicylate (PS) produced a revival of consciousness and reflex activity but had no appreciable effect on ocular motility. A prior case report of amitriptyline-induced ophthalmoplegia in a patient who took lesser amounts of the medication described immediate restoration of eye movement with only 2 mg of intravenous PS. The action of the amitriptyline on the vestibuloocular reflex …

Long-term electrophysiological assessment after hypoglossal-facial anastomosis.

PURPOSE: To investigate and provide objective documentation of the possible differences in the axonal reinnervation process of facial muscles after hypoglossal-facial nerve anastomosis. Then, to search for the presence of the trigemino-hypoglossal reflex and determine whether it indicates better peripheral recovery. METHODS: Electrophysiological examination performed on 20 patients who had undergone VII-XII anastomosis, with follow-up periods of more than 2 years. RESULTS: The mean follow-up time after surgery was 4.1 ± 1.3 years (range 2-8 years). The degrees of axonal reinnervation for the…

Trigeminal laser-evoked potentials: a neurophysiological tool to detect post-surgical outcome in trigeminovascular contact neuralgia.

BACKGROUND: The aim of this study was to explore the nociceptive system of patients affected by trigeminal neuralgia (TN) secondary to documented vascular contact who underwent microvascular decompression. For that purpose, we used the classical trigeminal reflexes and the trigeminal laser-evoked potentials (tLEPs) before and after surgery, in order to verify any possible change after decompression and determine if there was any correlation between the neurophysiological parameters and the clinical outcome. METHODS: Eleven patients affected by TN caused by trigeminovascular contact and 10 age…

The control of male sexual responses.

Male sexual responses are reflexes mediated by the spinal cord and modulated by neural circuitries involving both the peripheral and central nervous system. While the brain interact with the reflexes to allow perception of sexual sensations and to exert excitatory or inhibitory influences, penile reflexes can occur despite complete transections of the spinal cord, as demonstrated by the reviewed animal studies on spinalization and human studies on spinal cord injury. Neurophysiological and neuropharmacological substrates of the male sexual responses will be discussed in this review, starting w…

High frequency immittance for neonates: a normative study.

CONCLUSION: High frequency immittance measurements demonstrate promise in clarifying middle ear status for neonates but age- and gender-specific norms should be consulted. OBJECTIVE: To describe high frequency immittance measurements using a 1000 Hz probe tone for a sample of 278 neonatal ears (0-4 weeks of age) in order to compile normative tympanometric and acoustic reflex criteria. SUBJECTS AND METHODS: Assessment of neonatal ears included 1000 Hz probe tone immittance measurements (tympanograms and ipsilateral acoustic reflexes), and distortion product oto-acoustic emission (DPOAE) scree…

Emergency craniotomy in patients worsening due to expanded cerebral hematoma: to what purpose?

BACKGROUND: Supratentorial intracerebral hematomas often are evacuated in rapidly deteriorating patients. Surgery may prevent death but not necessarily disability. The authors studied the outcome of emergent clot evacuation in patients with worsening massive intracerebral hemorrhage. METHODS: The authors reviewed data on 26 consecutive, acutely worsening patients with nontraumatic intracerebral hemorrhage who had surgery for clot evacuation. All patients had clinical (stupor or coma, loss of pontomesencephalic brainstem reflexes, extensor posturing) or radiologic (midline shift of septum pell…

Computed tomography-guided drainage of a canine intracerebral abscess.

An approximately 7-week-old female American Bulldog was presented for head injury and seizures. The dog was obtunded with absent left-sided postural reactions, absent left palpebral reflex, delayed vestibulo-ocular reflex bilaterally, and calvarial hyperesthesia. CT revealed a fragmented, depressed fracture of the right frontal bone, with an associated abscess and evidence of raised intracranial pressure. The dog initially declined on medical management. Following a CT-guided drainage procedure, the dog improved and remained clinically well twenty months later. This is the first report of mini…

Prophylactic Gynecologic Specimens from Hereditary Cancer Carriers.

Hereditary breast ovarian cancer and Lynch/hereditary nonpolyposis colorectal cancer syndrome account for most hereditary gynecologic cancers. In the absence of effective cancer screening and other preventative strategies, risk-reducing surgery in women who are known to be at genetic risk of BRCA-associated or of Lynch syndrome carcinomas is effective in significantly decreasing the lifetime risk of developing malignancy. Reflex genomic testing of high-grade ovarian cancers and reflex immunohistochemistry in endometrial cancers will lead to greater recognition of germline-associated cancers. A…

[Therapy progress of atonic bladder after spinal cord injury].

OBJECTIVE: To study the advance in the treatment of atonic bladder after spinal cord injury. METHODS: The recent literature concerned was extensively reviewed and some methods of therapy for atonic bladder were introduced. RESULTS: No effective method of therapy was found for atonic bladder after spinal cord injury. When compared with clean intermittent catheterization, pharmacologic therapy, compressive micturition and detrusor function reconstruction, the establishment of an artificial bladder reflex arc may have a potential of controllable micturition. CONCLUSION: To establish an artific…

Influence of opioid receptor antagonists on motor-impairing effects of benzodiazepines.

The influence of naloxone and naltrexone on the motor-impairing effects of diazepam, chlordiazepoxide, clonazepam and estazolam were studied in the aerial righting reflex test (mice, rats), and the first two drugs were examined in the rota-rod test (mice). Benzodiazepine-induced motor incoordination was significantly decreased by naloxone and naltrexone (4-16 mg/kg) in mice and rats in aerial righting reflex test. The motor-impairing effects of diazepam and chlordiazepoxide observed in rota-rod test were significantly diminished only by naltrexone (8-16 mg/kg). These data seem to confirm some …
