Pedicled Oblique Para-Umbilical Perforator (OPUP) Flap for Upper Limb Reconstruction.

BACKGROUND: Groin flap has been considered the workhorse flap for hand reconstruction. However it has certain drawbacks when covering defects over elbow or proximal forearm. Pedicled oblique paraumbilical perforator (OPUP) based flaps provide a good alternative in such cases. METHODS: We performed OPUP flap in 11 cases of complex upper limb reconstructions over the past one year. We report our experience with this flap. RESULTS: All flaps survived. One patient had marginal necrosis of 1 cm in the proximal end after flap division. Three patients had donor site scar hypertrophy. CONCLUSIONS: …

Progressive hip joint subluxation in Saul-Wilson syndrome.

Saul-Wilson syndrome (SWS) is a rare congenital skeletal syndrome characterized by postnatal onset of short stature, relative microcephaly, frontal bossing, prominent eyes with shallow orbits, midface hypoplasia, cataract, and generalized skeletal changes, including spondylar dysplasia, overtubulation of the long bones with metaphyseal flaring and megaepiphyses, coxa valga, elbow deformity, and brachydactyly. We describe a boy with the overall clinical and radiological features fitting the characteristics of SWS, although cataract, elbow deformity, and overt brachydactyly were not seen. He pre…

Proximal median nerve entrapment caused by a distal biceps tendon cyst: an ultrasonographic diagnosis.

A 60-yr-old woman reported severe pain in the proximal part of her left forearm. The pain was also radiating toward the median nerve-innervated areas of the arm, especially during elbow flexion. Ultrasonographic imaging showed a well defined ganglion cyst around the distal biceps tendon close to the median nerve. Under ultrasound guidance, the cyst was aspirated and corticosteroid injection was performed. The patient's complaint improved 1 wk after the injection, and she started to flex her elbow without any pain. This case highlights the role of ultrasonography as a useful adjunctive tool not…

Chronic triceps insufficiency managed with extensor carpi radialis longus and palmaris longus tendon grafts.

Chronic triceps insufficiency, causing prolonged disability, occurs due to a missed diagnosis of an acute rupture. We report a 25 year old male with history of a significant fall sustaining multiple injuries. Since then, he had inability in extending his right elbow for which he sought intervention after a year. Diagnosis of triceps rupture was made clinicoradiologically and surgery was planned. Intraoperative findings revealed a deficient triceps with a fleck of avulsed bone from olecranon. Ipsilateral double tendon graft including extensor carpi radialis longus and palmaris longus were ancho…

Antley-Bixler syndrome: case report and review of the literature.

The purpose of this report is to describe a patient with a pattern of malformations very similar to those described by Antley and Bixler. They include craniosynostosis, midface hypoplasia with proptosis, ear anomalies, choanal stenosis, long tapered fingers with a bulbous tip, elbow joint contracture due to radio-ulnar synostosis and talipes equinovarus. Necropsy revealed cardiac and renal malformations. The infant died a few days after birth, of respiratory failure. Unlike previously described cases the elbow joint contracture was due to radio-ulnar synostosis rather than radio-humeral synost…

[Value of proximate musculocutaneous flaps for covering open wounds caused by decubitus ulcers].

Musculo cutaneous flaps help to cover the important tissues defects of the pressure sore. During 1988, two of our patients in general bad conditions, needed a quick simple treatment for bed sores involving joints. The patients, one who was paraplegic, the other tetraplegic, were suffering from joint lesions in the elbow and both knees. We carried out a musculo cutaneous flap of the brachio radialis and two of the gastrocnemius medialis, once septic conditions had improved. The simple procedure by transfer assured the cover of the skin defect. The results of the treatment being successful allow…

Soft tissue calcification treated with local and oral magnesium therapy.

Eighty patients with soft tissue calcification were treated: 24 suffered from myositis ossificans traumatica, 23 from calcific bursitis (Duplay's disease), six from osteoarthropathy of elbow joint after severe craniocerebral trauma, nine from calcification around the elbow joint after local trauma, 13 from calcification around the hip joint, and five from calcification in ligaments and tendons. Using a new method of treatment about 75% of patients were cured. Calcifications disappeared or diminished substantially. Very good functional improvement followed in affected joints. The treatment invo…

Polymicrobial polyarticular septic arthritis.

A bacteriology technologist was found to have acute polyarticular arthritis after a brief prodromal illness. Gram's stain of fluid from the right knee showed pleomorphic gram-negative organisms, while that of fluid from the right elbow and left wrist showed gram-negative cocci and diplococci. Culture of fluid from the right knee yielded Salmonella enteritidis. Cultures of fluid from all other joints, collected after starting therapy, were negative. An enzyme-linked immunosorbent assay of fluid from the right elbow confirmed Neisseria meningitidis, group C. This organism was also isolated from …

Ulnar nerve mononeuropathy in a patient with Hansen's disease: Clinical and radiological features.

Leprosy is a chronic infection caused by bacteria called Mycobacterium leprae. This is a prevalent disease in low-income countries, and it has not been completely eradicated. We present the case of a 29-year-old man with a previous diagnosis of Hansen's disease, who consulted for pain in the left elbow and wrist, associated with fever with preserved mobility and pain on palpation. Diagnosis of mononeuropathy of the ulnar nerve was made by MRI of the elbow and forearm. This entity corresponds to an infrequent complication of this infection with few cases reported in the literature. Our added va…

Outcome of hand surgery in children with spasticity - a 9-year follow-up study.

The aim of this study was to evaluate whether short-term positive effects on bimanual function after surgery of the paretic arm in cerebral palsy are maintained long term. Assisting Hand Assessment (AHA) and active range of motion was tested before surgery and at 7 month and 9-year follow-up (n=18). AHA improved significantly from 50 to 52 U at 7 months, but was not different from before surgery at the 9-year follow-up, 49 U. Surgery of wrist and elbow flexors significantly improved active extension. Improvement in wrist and elbow extension was maintained at the 9-year follow-up, but usefulnes…

Wide Decompression May Render the Ulnar Nerve Unstable: A Cadaveric Study.

The purpose of this study was to evaluate for ulnar nerve instability following incrementally widened in situ decompression. A standard release of the ulnar nerve was performed in 16 cadaveric elbows, extending from 7cm distal to the medial epicondyle, and then released proximally for a total of 10 cm in 2-cm increments. Eight of the 16 elbows (50%) displayed subluxation of the ulnar nerve following complete in situ decompression. The rate of subluxation was found to increase with increasing length of proximal decompression. The greatest increase in rate of subluxation was seen beyond 4 cm pro…

Excellent Functionality Despite Clinico-Radiological Deformity in Osteomyelitis Variolosa - A Case Report.

Despite the eradication of smallpox from the world in 1980 the osteo-articular sequelae of smallpox are still occasionally noticed in previously endemic areas. The sequelae of osteomyelitis variolosa may raise a diagnostic challenge to the untrained eyes of the surgeon. We present a case of "osteomyelitis variolosa" in a 70 years old patient. The patient had bilateral dislocation of the elbow joint with multidirectional instability. There was distortion of the articular surfaces and ankylosis of the bilateral proximal radio-ulnar joint. Hypoplasia of the right ulna with short fourth and fifth …

Arthroscopic operations in joint tuberculosis.

We assessed the usefulness of arthroscopy in bone and joint tuberculosis (11 knees, 1 elbow and 1 ankle). In all 11 patients with knee joint involvement the arthroscopic operation were successful with a significant improvement in the range of motion (P=0.007) and knee score (P=0.003). There were no cases of reactivation or recurrence during a follow-up period of 15-42 months. In contrast, in the two patients with ankle or elbow joint involvement and bone destruction, arthroscopy had to be converted to open operation. Minor trauma, a short hospitalization time and a short post-operative rehabil…

Examination of the lateral antebrachial cutaneous nerve: an anatomic study in human cadavers.

Variations in the anatomic course of the cutaneous nerves about the lateral aspect of the elbow are important when surgical exposures and the establishment of arthroscopic portals are considered. The specific anatomic course taken by the lateral antebrachial cutaneous nerve and its relationship to the lateral epicondyle were determined by studying 33 upper extremities in 22 preserved adult cadavers. Considerable anatomic variation was found regarding the location of the lateral antebrachial cutaneous nerve as it crossed the elbow. The nerve pierced the brachial fascia an average of 3.2 cm prox…

Untreated intra-articular entrapment of the medial humeral epicondyle.

Six children with entrapment of the medial epicondyle in the elbow after closed reduction of a posterior dislocation were seen an average of 14 weeks after injury. The elbows were painful and the average range of flexion was 22 degrees. Two children had ulnar nerve involvement which recovered after operation. The epicondyle was removed from the joint and either reattached to the humerus or excised, and the muscles reattached. Two children had anterior transposition of the ulnar nerve, one for pre-operative hyperaesthesia, and the other to relieve tension on the nerve. At follow-up, at an avera…

Late results in fractures of the radial head treated by excision.

Late results of radial head excision were studied in 60 patients who had isolated fractures of the radial head. Symptomatic elbows were found in 43% of the patients, osteoarthritis in 52%, and valgus deformity in 30%. Elbow motion was restricted in 63%, and new bone formation at the surgical site was noted in 57% of the patients. Forearm rotation was limited in 58%. A proximal radioulnar synostosis was found in 7%, and proximal migration of the radius was present in 47%. A definite and symptomatic subluxation of the distal radioulnar joint was found in 25%. Overall results were unsatisfactory …
