Development of the Anesthesia Risk Assessment Score (ARAS) for postoperative mortality and adverse discharge to a nursing facility.

BACKGROUND: We developed a simple questionnaire that the surgeon's office uses when meeting with their patients to book a case. In this study, we used these questions to evaluate their predictive value for mortality and adverse discharge to a nursing facility in comparison with the American Society of Anesthesiologists Physical Status [ASA-PS] and other risk assessment scores. METHODS: We analyzed data from adult patients undergoing non-ambulatory surgery between January 2016 and February 2023 at Montefiore Medical Center, a tertiary academic center in the Bronx, NY. The predetermined questio

Determinants of early initiation of breastfeeding following birth in West Africa: A multilevel analysis using data from multi-country national health surveys.

BACKGROUND: Early initiation of breastfeeding (EIBF), within the first hour of birth, is crucial for promoting exclusive breastfeeding and establishing optimal nursing practices. However, global EIBF rates remain low, with even lower rates observed in Africa. Despite existing research gaps, this study aims to determine the prevalence of EIBF and identify maternal and child-related factors associated with its practice in West Africa. METHODS: This study utilized West African Demographic and Health Survey (DHS) data from 13 countries, including 146,964 children's records. To assess model fit, l

Environmental Triggers of Specific Subtypes of Agitation in People With Dementia: Observational Study.

BACKGROUND: Among the most critical behavioral and psychological symptoms of dementia, agitation can lead to decreased quality of life of people with dementia and their caregivers. Monitoring triggers of agitation and its subtypes could enable early detection or prediction of agitated moments, which could be used to guide preventive or mitigating interventions. However, at this point in time, limited research exists on quantifying environmental triggers of agitation or its subtypes. OBJECTIVE: In this paper, we aim to quantify the relationships between specific environmental factors and agita

Transformer- and Generative Adversarial Network-Based Inpatient Traditional Chinese Medicine Prescription Recommendation: Development Study.

BACKGROUND: Traditional Chinese medicine (TCM) practitioners usually follow a 4-step evaluation process during patient diagnosis: observation, auscultation, olfaction, inquiry, pulse feeling, and palpation. The information gathered in this process, along with laboratory test results and other measurements such as vital signs, is recorded in the patient's electronic health record (EHR). In fact, all the information needed to make a treatment plan is contained in the EHR; however, only a seasoned TCM physician could use this information well to make a good treatment plan as the reasoning process

A risk prediction model for oral frailty in elderly patients with COPD was constructed based on the health ecology model.

BACKGROUND: Elderly patients with chronic obstructive pulmonary disease (COPD) often experience oral health problems, such as dry oral mucosa, tooth loss, and gum disease. Dry oral mucosa makes it difficult for food to slide and chew in the mouth. Tooth loss or gum disease can affect chewing and occlusal functions, causing difficulty in swallowing during meals for such patients and increasing the risk of difficulty in swallowing. Oral frailty is an independent risk factor for dysphagia in elderly patients with COPD. This study aims to explore the influencing factors of oral frailty in this gro

A scalable educational intervention to reduce airway-related harm in low-resource PICUs: A design-test-scale experience.

PROBLEM: Across low- and middle-income countries (LMICs), an estimated 1.7 billion children lack access to safe surgical and perioperative care. In Latin America, pediatric postoperative mortality remains several-fold higher than in high-income countries, driven largely by preventable airway and critical-care complications. Following the death of an infant after an unplanned extubation in El Salvador, system-level gaps in airway safety, sedation, and communication were identified, motivating development of a scalable intervention to strengthen postoperative critical care for intubated children
