Reamed versus minimally reamed nailing: a prospectively randomised study of 100 patients with closed fractures of the tibia.

It is generally accepted that in tibial fractures the results of reamed intramedullary nailing are better than those of unreamed. However, i … mpared to patients with minimally reamed nailing (19 wks), and there were more patients with reamed nails in whom the fracture had healed by 16 weeks (57%) versus the minimally reamed group (43%), however, this was not statistically significant. Pain scales were similar for both groups from week 4 to week 52. A considerable number of outcome parameters …

Tibial lengthening over an intramedullary nail in patients with short stature or leg-length discrepancy: a comparative study.

PURPOSE: The aim of this study was to review our experiences with tibial lengthening over an intramedullary nail in comparison to the conventional Ilizarov method. METHODS: We performed a retrospective comparison of tibial lengthening using the conventional Ilizarov method (group A: 23 limbs in 13 patients) versus over a nail (group B: 51 limbs in 26 patients). The percentage increase in tibial length, lengthening index, external fixation index, consolidation index and complications were assessed. RESULTS: The mean gain in tibial length was 7.4 cm, which represents a mean increase of 26.0%.

Treatment of femur fractures in school-aged children using elastic stable intramedullary nailing: a systematic review.

Femur fractures are common long-bone injuries in school-aged children (6-12 years). Among the various acceptable treatment options, elastic stable intramedullary nailing (ESIN) has gained popularity over recent years although the level of evidence for ESIN is low. This study was a systematic review of the literature to examine the outcomes and complications of ESIN in school-aged children and to critically evaluate the quality of the available literature. Although most complications were minor, some series report complication rates of more than 50%. Union rates are high. Malunion or mechanical

Outcome analysis of retrograde nailing and less invasive stabilization system in distal femoral fractures: A retrospective analysis.

BACKGROUND: TWO MAJOR THERAPEUTIC PRINCIPLES CAN BE EMPLOYED FOR THE TREATMENT OF DISTAL FEMORAL FRACTURES: retrograde intramedullary (IM) nailing (RN) or less invasive stabilization on system (LISS). Both operative stabilizing systems follow the principle of biological osteosynthesis. IM nailing protects the soft-tissue envelope due to its minimally invasive approach and closed reduction techniques better than distal femoral locked plating. The purpose of this study was to evaluate and compare outcome of distal femur fracture stabilization using RN or LISS techniques. MATERIALS AND METHODS:

Preoperative Assessment Using Magnetic Resonance Imaging for Subungual Exostosis beneath the Proximal Region of the Nail Plate.

Subungual exostosis is a benign calcifying projection that occurs on the distal phalanx beneath or beside the nail. The problems in treatment are frequent postoperative recurrence and nail deformity or loss. Here, we describe the case of a male patient with a subungual exostosis beneath the proximal region of the nail plate of the right big toe. A precise preoperative evaluation of the anatomical structure using magnetic resonance imaging was useful for removing the subungual exostosis.

External jig-aided intramedullary interlocking nailing of diaphyseal fractures: experience from a tropical developing centre.

Intramedullary interlocking nailing for diaphyseal fractures is a standard treatment option in affluent societies. These procedures are often performed under image intensifier guidance. The cost of these gadgets precludes their common use in resource poor regions. External jig-aided intramedullary interlocking nailing is relatively cheap and offers the chance for performing these procedures in resource poor regions. The aim of this study was to document the advantages, challenges and outcome of this form of treatment in a resource poor setting. The Surgical Implant Generation Network (SIGN) im

Efficacy of intraoperative surgical irrigation with polihexanide and nitrofurazone in reducing bacterial load after nail removal surgery.

BACKGROUND: A common challenge of nail avulsion surgery is the associated bacterial contamination and infection that can manifest. The toe has a difficult anatomy to antiseptically prepare and properly maintain throughout the surgical procedure, lending to this widespread problem. OBJECTIVE: We conducted a controlled, prospective randomized study to examine the antiseptic efficacy of 3 intraoperative irrigation methods during nail avulsion surgery. METHODS: We compared intraoperative antiseptic irrigation using 0.9% saline solution (24 patients), 0.2% nitrofurazone (22 patients), and 0.1% po

Medial migration of lag screw with intrapelvic dislocation in gamma nailing--a unique problem? A report of 2 cases.

Within the last decade, intramedullary nailing systems have gained popularity in the treatment of unstable fractures of the trochanteric region with good clinical results. However, these implants are not free of complications. Commonly reported treatment failures include implant cutout through the femoral neck, stress-derived femur fractures at the distal end of the nail, and secondary rotational displacement and varus deformity of the femoral neck and head. Only few reports exist on primary medial migration of the lag screw after treatment with intramedullary implants. We report on a rarely d

[Examination about the influence for daily life that nail and skin disorders induced by docetaxel in patients with breast cancer].

Docetaxel is one of the most important chemotherapeutic agents for the breast cancer patients. However, it has also experienced as toxicities causing nail diformation and skin trouble, and is considered as adverse elements other than edema and neurotoxicity. It is expected to have tremendous influence on women's daily life-decrease in quality of life. Nail and skin toxicity might occur among patients treated with docetaxel more frequently than evidence in the clinical medical records. Therefore, we have conducted a questionnaire investigation, targeting the patients who had been treated with d

Pulsed dye laser vs. photodynamic therapy in the treatment of refractory nail psoriasis: a comparative pilot study.

BACKGROUND: Nail psoriasis is often refractory to traditional treatments, and patients with nail psoriasis usually demand a therapeutic option. Both photodynamic therapy (PDT) and pulse dye laser (PDL) have proved effective for plaque-type psoriasis, but they have not been evaluated in nail psoriasis. On the other hand, delta-aminolaevulinic acic has been shown to penetrate into the nail matrix and nail bed occluded with bioadhesive patches. OBJECTIVES: To compare the efficacy of PDT and PDL in the treatment of nail psoriasis. METHODS: We studied 61 nails treated with PDT and 60 nails treate…

Superficial femoral artery injury resulting from intertrochanteric hip fracture fixation by a locked intramedullary nail.

UNLABELLED: Iatrogenic vascular injuries are uncommon during the course of proximal femur surgical procedures. We report the case of an 85-year-old female presenting with an intertrochanteric fracture, treated by anterograde (cephalocondylic) intramedullary nailing (Stryker gamma 3 nail) and complicated by a superficial femoral artery laceration at the level of the distal locking screw. Lower limb traction in adduction and internal rotation on the operating table might put at risk the superficial femoral artery during distal screw drilling and insertion. We therefore recommend returning to the

Fontaine-Farriaux syndrome: a recognizable craniosynostosis syndrome with nail, skeletal, abdominal, and central nervous system anomalies.

Craniosynostosis is an etiologically heterogeneous malformation, which may present as an isolated finding or in association with other anomalies. The concurrence of craniosynostosis together with specific central nervous system, abdominal, genital, and limb malformations defines the Fontaine-Farriaux syndrome, described so far in only two patients. We report on a stillborn who mainly presented severe intrauterine growth retardation, bilateral coronal synostosis, generalized nail hypo/aplasia more evident on the posterior side, tapered digits, mild cutaneous syndactyly, abdominal muscle hypopla

A novel technique for distal locking of intramedullary nail based on two non-constrained fluoroscopic images and navigation.

Distal locking is one of the most difficult steps in intramedullary nailing. Numerous methods can help the surgeon, but all are time-consuming and involve much irradiation. We have developed and tested a new method based on only two fluoroscopic shots that do not need to be taken in the axes of the holes. This avoids requiring the presence of an experienced fluoroscopy operator to accurately adjust the imaging device in front of the locking holes, and decreases the exposure to radiation of the patient and medical team. A 3-D model of the distal nail and of its locking holes was constructed fro

Soft tissue anatomy around the hip and its implications for choice of entry point in antegrade femoral nailing.

Antegrade intramedullary nailing is an accepted method of treatment for femoral shaft fractures. Entrance of the nail through the trochanteric fossa is currently recommended by some surgeons. This approach results in some cases, however, in loss of abduction strength and persistent pain. Nail insertion at the tip of the greater trochanter may be more favorable. In this study the anatomical relationships of the trochanteric fossa and of the tip of the greater trochanter were explored. Dissection was carried out in 10 fresh human cadaver femurs. The risks and safety of the two entry points with
