Realistic Vascular Replicator for TAVR Procedures.

Transcatheter aortic valve replacement (TAVR) is an over-the-wire procedure for treatment of severe aortic stenosis (AS). TAVR valves are conventionally tested using simplified left heart simulators (LHS). While those provide baseline performance reliably, their aortic root geometries are far from the anatomical in situ configuration, often overestimating the valves' performance. We report on a novel benchtop patient-specific arterial replicator designed for testing TAVR and training interventional cardiologists in the procedure. The Replicator is an accurate model of the human upper body vasc

Computerized tomography with 3-dimensional reconstruction for the evaluation of renal size and arterial anatomy in the living kidney donor.

PURPOSE: Computerized tomography (CT) with 3-dimensional (3-D) reconstruction has gained acceptance as an imaging study to evaluate living renal donors. We report our experience with this technique in 199 consecutive patients to validate its predictions of arterial anatomy and kidney volumes. MATERIALS AND METHODS: Between January 1997 and March 2002, 199 living donor nephrectomies were performed at our institution using an open technique. During the operation arterial anatomy was recorded as well as kidney weight in 98 patients and displacement volume in 27. Each donor had been evaluated pre

Humeral Head Reconstruction in Anatomic Shoulder Arthroplasty: How to Assess It, How to Avoid Overstuffing, and Whether It Matters.

➢ Humeral head anatomy affects the tension and mechanics of the glenohumeral joint. Thus, aiming for anatomic reconstruction can help to avoid negative consequences of component malpositioning (such as "overstuffing") on soft-tissue tension and impingement-free range of motion.➢ The most common method to assess humeral head reconstruction is comparing the prosthetic humeral articular surface with the "perfect circle" incorporating the lateral cortex of the greater tuberosity, the medial greater tuberosity, and the medial calcar at the anatomic neck. Although this method is quick and helpful in

Hemodynamic Selection Bias in Observational Comparisons of EVAR Versus Open Repair for Ruptured Infrarenal Abdominal Aortic Aneurysm: A Systematic Review and Meta-Analysis.

BACKGROUND: Endovascular aneurysm repair (EVAR) is often associated with lower short-term mortality than open surgical repair (OSR) in observational studies of ruptured abdominal aortic aneurysm (rAAA), but this apparent benefit may reflect hemodynamic stability, anatomical suitability, and emergency treatment allocation. We aimed to reappraise EVAR-OSR comparisons by examining whether mortality estimates are influenced by treatment-selection bias. METHODS: PubMed, the Cochrane Library, China National Knowledge Infrastructure, and Wanfang Data were searched from inception to May 2026. Compara

Transcystic papillary balloon dilatation vs. laparoscopic choledochoscopy for choledocholithiasis: a retrospective observational propensity score-matched cohort study.

BACKGROUND: Single-stage management of choledocholithiasis during laparoscopic cholecystectomy can be performed using transcystic papillary balloon dilatation (TCPBD) or laparoscopic choledochoscopy (LCC). Direct comparative data between intraoperative clearance techniques remain limited. METHODS: We conducted a retrospective single-center observational cohort study of 132 consecutive adult patients undergoing documented one-stage minimally invasive treatment for choledocholithiasis between January 2020 and January 2026. Ninety-three patients underwent TCPBD and 39 underwent LCC. The primary

Comparative surgical outcomes of pterional, supraorbital, and endoscopic endonasal approaches for tuberculum sellae meningiomas: a systematic review and network meta-analysis.

BACKGROUND: The optimal surgical corridor for tuberculum sellae meningiomas (TSMs) remains a subject of ongoing debate. While the pterional (PT) approach is the traditional gold standard, the endoscopic endonasal approach (EEA) and supraorbital (SO) keyhole approach have emerged as viable alternatives. OBJECTIVE: To comparatively evaluate the surgical outcomes and complication profiles of the PT, SO, and EEA techniques for the management of TSMs. METHODS: A systematic literature search was conducted across PubMed, Embase, Scopus, Web of Science, and Cochrane databases through March 2026. Morphofunctional Space of the Forelimb in Caraguatypotherium munozi (Notoungulata; Mesotheriidae): Insights Into Wrist-Powered Digging.

Understanding how extinct animals moved is a central goal in paleobiology, yet interpreting locomotor function from anatomy alone is complicated by convergent and divergent morphologies. One promising approach is the construction of morphofunctional spaces (MFSs), which integrate multiple biomechanical indices and comparative statistics to refine functional inference. This study investigates forelimb adaptations for digging in Caraguatypotherium munozi (Notoungulata, Mesotheriidae), a mid-sized Miocene notoungulate lacking extant analogs. We developed an MFS based on osteological measurements

Timing of Ansa Cervicalis-to-recurrent Laryngeal Nerve Reinnervation: A Systematic Review.

BACKGROUND: Ansa cervicalis-to-recurrent laryngeal nerve anastomosis (ARA) is an established technique for the treatment of recurrent laryngeal nerve (RLN) injury after head and neck surgery. However, the optimal timing of ARA remains unclear, and the evidence bases for ARA performed at each timepoint after RLN injury have not previously been clearly distinguished. We conducted a systematic review of the literature to evaluate the efficacy of ARA performed at different timepoints on postoperative voice outcomes. METHODS: A review of English-language journal articles published in the last 20 y

Histology and structural integration of the major morphologies of the Cypriniform Weberian apparatus.

The Weberian apparatus, a diagnostic feature of otophysan fishes, is a novel hearing adaptation integrating several developmental and morphological systems (ear-vertebral column-swim bladder). Otophysan fishes are one of the largest and most successful freshwater clades, with over 10,000 species across most continents. The largest otophysan order, Cypriniformes, dominates the freshwaters of Asia, Europe, North America, and Africa. Spanning such a wide variety of environments, the Weberian apparatus undergoes morphological modifications to maintain functionality. Within Cypriniformes, we propos

Range of Normal and Abnormal Syndesmotic Measurements Using Weightbearing CT.

BACKGROUND: Early recognition of syndesmotic instability is critical for optimizing clinical outcome. Injuries causing a more subtle instability, however, can be difficult to diagnose. The purpose of this study was to evaluate both distal tibiofibular articulations using weightbearing computed tomography (CT) in patients with known syndesmotic instability, thereafter comparing findings between the injured and uninjured sides. We also aimed to define the range of normal measurement variation among patients without syndesmotic injury. METHODS: Patients with unilateral syndesmotic instability re

Coronary echocardiography in 406 patients with d-loop transposition of the great arteries.

OBJECTIVES: The reliability of two-dimensional echocardiography for determining the proximal coronary artery anatomy in d-loop transposition of the great arteries was investigated in 406 infants who underwent surgical repair at one institution. BACKGROUND: The origin and proximal course of the main coronary arteries can affect the surgical results of the arterial switch operation. Preoperative determination of the coronary artery anatomy appears to be advantageous for the surgeon. METHODS: All infants with d-loop transposition who underwent a two-dimensional echocardiogram and primary surgic

Surgical anatomy of multipart fractures of the proximal humerus.

The gross fracture anatomy was recorded in a cohort of 22 consecutive patients who underwent open surgery for 3-part and 4-part fractures of the proximal humerus and was compared with preoperative radiographs in a blind manner. It was noted that 8 of the 22 fractures did not correspond to any category of the Neer or AO/Orthopaedic Trauma Association/Jakob classification. These fractures had 3 displaced segments in terms of surgical anatomy, but the humeral head was free of soft tissue. In addition, the soft-tissue attachments to the head were closely related to the articular surface orientatio

Modified Fricke's Cheek Flap for Periorbital Reconstruction: A Regional Alternative to Forehead-Based Flaps.

INTRODUCTION: Periorbital reconstruction remains challenging due to the region's complex anatomy and essential roles in eyelid function, globe protection, and facial aesthetics. Traditional reconstructive options, such as forehead-based Fricke's flaps, may result in eyebrow distortion and poor color match. The modified Fricke's cheek flap offers a regional alternative with improved mobility, vascularity, and aesthetic integration. This study evaluates flap survival, functional eyelid outcomes, aesthetic results, and complications of this technique in traumatic periorbital defects. MATERIALS A

The use of the Neviaser portal for antegrade intramedullary nailing in humerus fractures: a scoping review.

PURPOSE: Proximal humerus and humeral shaft fractures are common orthopaedic injuries that can be managed using antegrade intramedullary nailing. The Neviaser portal allows a more medial start point at the apex of the humeral head. The published literature on humeral intramedullary nailing through the Neviaser portal is limited and heterogeneous. This scoping review aims to map the existing literature on the use of the Neviaser portal for antegrade intramedullary nailing of humeral fractures, summarise reported indications, surgical techniques, radiographic and functional outcomes, complicatio

Low interobserver agreement among expert surgeons using a common morphological classification for pectus excavatum.

BACKGROUND: Since the initial publication of a visual classification of pectus excavatum in 2006-later revised in 2013-its use has gained widespread international popularity. This system categorizes pectus excavatum into three morphological types: cup-shaped, saucer-shaped, and trench-shaped. However, concerns remain regarding its reproducibility. We therefore conducted a two-stage evaluation of anonymized chest wall photographs: first within our surgical team, and subsequently with a panel of internationally recognized chest wall surgeons, to assess interobserver consistency. METHODS: Compar

Molecular and morphological characterization of third instar Palaearctic horse stomach bot fly larvae (Oestridae: Gasterophilinae, Gasterophilus).

Species of Gasterophilus are obligate parasites of equids and may induce severe, even lethal myiasis. However, identification of the third instar Gasterophilus larva at the species level is still problematic predominantly due to a shortage of diagnostic morphological features and incomplete molecular libraries. Testing the suitability of three different molecular markers showed that the traditional 650 bp barcode region near the 5' terminus of the cytochrome c oxidase subunit I (COI) served as a better tool for species-level identification than a 663 bp region near the 3' terminus of COI and a
