Increased platelet reactivity in unstable angina patients is not related to C-reactive protein levels.

Platelets are a major component of thrombi, and coronary thrombosis plays a key role in the pathogenesis of unstable angina (UA). Whether platelet aggregability is increased in UA patients however, is not known. Furthermore, no study has investigated the relationship between platelet reactivity and inflammation in UA patients In this study, venous blood samples were collected at admission in coronary care unit in 37 patients with unstable angina (Braunwald class IIIB) and in 37 sex- and age-matched patients with chronic stable angina (CSA). Patients taking thienopyridine or anticoagulant drugs

Reduction of C-reactive protein by a single 80 mg of simvastatin in patients with unstable angina.

BACKGROUND: Increased concentrations of C-reactive protein (CRP) became widely accepted as a risk factor of the higher incidence of coronary events, and rapid lowering CRP by administration of drugs may produce early benefit to the coronary endothelium in patients with coronary heart disease and reduce angina and coronary events after revascularization. Limited information has been available, however, with respect to evaluating a potential effect of a single high-dose simvastatin on CRP in patients with unstable angina (UA) within 48 h. We investigated whether a rapid CRP reduction can be achi

Establishment of a high sensitivity plasma assay for human pentraxin3 as a marker for unstable angina pectoris.

OBJECTIVE: Plasma pentraxin 3 (PTX3) levels are increased in patients with acute myocardial infarction, yet its involvement in unstable angina pectoris (UAP) remains unclear. To critically evaluate the role of PTX3 in UAP, a sensitive and precise measurement of PTX3 concentration is needed. METHODS AND RESULTS: We established a high sensitive plasma ELISA assay system for the detection of PTX3 using monoclonal antibodies. The lower limit of detection of our ELISA was 0.1 ng/mL, sensitivity far greater than the current commercially available kit. Plasma samples were obtained from 162 consecuti

In-hospital and late outcomes after coronary stenting in patient with unstable angina and myelodysplastic syndrome.

Sixty-one-year-old male patient with diagnosis of myelodysplastic syndrome and unstable angina was submitted to coronary angiography and implant of stent. His Blood cell count revealed 40,000 platelets/mm3. Coronary angiography with previous platelet transfusion showed obstruction of 80% of the right coronary artery (RCA). Following the administration of clopidogrel, the patient was submitted to another platelet transfusion and stent implantation in the RCA lesion. No bleeding was observed after the introducers removal. After 6 months, treadmill test was positive and new coronary aniography, i

Preoperative statin use and in-hospital outcomes following heart surgery in patients with unstable angina.

OBJECTIVE: Unstable angina (UA) is characterized by a state of coronary artery vascular inflammation and endothelial dysfunction. Statins mitigate inflammation and endothelial dysfunction and decrease mortality associated with percutaneous interventions for UA. We determined whether preoperative statin use is associated with decreased mortality and morbidity following coronary artery bypass+/-valve surgery for UA. METHODS: Patients with CCS Class IV angina having CABG+/-valve surgery were identified (n=1706). A logistic regression model determined the association of preoperative statin use wi

[Clinical study of pravastatin with different dosage in early admission period in patients with unstable angina].

OBJECTIVE: To investigate the effect of early intervention by pravastatin with two different dosage on inflammatory factors and endothelial vasodilator function in patients with unstable angina (UA). METHODS: 108 patients with UA were investigated consecutively and divided randomly into three groups (group 20 mg, n = 37; group 10 mg, n = 37; group control, n = 34). Blood samples were examined at admission and 4, 8 weeks after the therapy of pravastatin. Fourty patients of UA were chosen from those three groups (15, 15 and 10 cases respectively). The endothelium-dependent vasodilation and the

Severe thrombocytopenia after heparin therapy in a patient with unstable angina and recent stent implantation.

Heparin-induced thrombocytopenia represents a serious side effect of heparin therapy. Immune-mediated platelet activation results in thrombocytopenia, endothelial thrombin release and development of thrombosis, mainly venous. We report the case of a man with a history of coronary artery disease and recent stent implantation. This patient developed severe heparin-induced thrombocytopenia type II after low molecular weight heparin administration because of unstable angina which occurred two months after stent implantation. The patient was treated with a new anticoagulant regimen, fondaparinux so

Determinants of ELISA D-dimer sensitivity for unstable angina pectoris as defined by coronary catheterization.

Unstable angina pectoris is associated with elevated D-dimer levels. However, the operating characteristics (sensitivity, specificity, positive and negative predictive value) of the D-dimer assay for the diagnosis of coronary artery disease (CAD) are unknown. Using a prospective, observational design, we collected blood from 54 patients with unstable angina pectoris at admission and assayed for ELISA D-dimer levels. The sensitivity, specificity, and negative and positive prediction values for angiographically determined coronary artery disease were calculated at multiple discriminate levels. A

[Utility of BNP measurement in the emergency room in patients with suspected unstable angina with a normal ECG].

Unstable angina is a serious condition, difficult to diagnose in the emergency room. Clinical, electrocardiographic and biological signs (increased troponine) are not sensitive. The authors set out to assess whether measuring B natiuretic peptide in the emergency room was more sensitive for identifying symptomatic coronary lesions. One hundred and twenty patients admitted to the emergency room for chest pain compatible with the diagnosis of unstable angina and a normal ECG were included in this prospective study. All patients underwent coronary angiography during their hospital admission. The

Myocardial uptake and release of substrates in patients operated for unstable angina: impact of glutamate infusion.

OBJECTIVE: To study cardiac uptake and release of substrates and the influence of intravenous glutamate in patients operated for unstable angina requiring intravenous nitrates. DESIGN: Nineteen patients were randomized to blinded infusion of glutamate or saline. Arterial-coronary sinus differences of substrates were measured before cardiopulmonary bypass (CPB) and during early reperfusion. RESULTS: Before CPB the only major substrates that were extracted by the heart in the saline group were free fatty acids (FFAs). During reperfusion uptake of glucose and glutamate was found but FFAs remain

Application of current guidelines to the management of unstable angina and non-ST-elevation myocardial infarction.

Unstable angina/non-ST-elevation myocardial infarction (UA/NSTEMI) is a common but heterogeneous disorder with patients exhibiting widely varying risks. Early risk stratification is at the center of the management program and can be achieved using clinical criteria and biomarkers, or a combination. In addition to anti-ischemic therapy and aspirin, the thienopyridine clopidogrel is indicated except in patients who are potential candidates for urgent coronary artery bypass grafting (CABG). Platelet glycoprotein (GP) IIb/IIIa antagonists are indicated in high-risk patients likely to undergo percu

[Oxygen dependent metabolism of neutrophils and monocytes in patients with unstable angina and its correction with preductal].

Oxygen-dependent metabolism of neutrophils and monocytes and antioxidant defense and heparin and trimetasidine effects on them were studied in 39 patients with unstable angina. All the patients showed a significant activation of neutrophils and monocytes by NBT-test, insignificant one by chemiluminescence in active serum superoxide dismutase and reduced total antioxidative serum potential. There was also a moderate rise in the level of CIC in enhanced HBT-test. Heparin in unstable angina pectoris produced stimulation of oxygen-dependent metabolism of neutrophils and monocytes, ischemic damage

Delayed splenic rupture presenting as unstable angina pectoris: case report and review of the literature.

The diagnosis of delayed rupture of the spleen can be challenging if the history of trauma is remote, or initially missed. Delayed rupture of the spleen can occur in approximately 1% of blunt force injuries to the spleen. We present a difficult case of delayed rupture of the spleen from remote, minor trauma and discuss the literature associated with this injury.

Prothrombotic response to coronary angioplasty in patients with unstable angina and raised C-reactive protein.

BACKGROUND: To better understand the mechanisms linking C reactive protein (CRP) to the risk of coronary thrombosis, we investigated the relation between inflammatory state and hemostatic response to coronary angioplasty in patients with either stable or unstable angina. METHODS: Plasma levels of von Willebrand factor (vWF) antigen, vWF collagen binding activity, vWF-cleaving-protease, CRP, and whole-blood platelet reactivity were measured before and 24 h after angioplasty in 18 patients with recent unstable angina and in 17 with stable angina. Each group was divided according to preprocedura

Measurement of plasma lipids in patients admitted with acute myocardial infarction or unstable angina pectoris.

We assessed the reliability of early (first day) plasma lipid measurements in patients admitted with acute myocardial infarction or unstable angina pectoris. In 55 such patients, plasma levels of cholesterol and triglycerides measured within the first 24 hours after admission were significantly lower than the corresponding values at 6 weeks, which means that even very early in-hospital lipid measurements could lead to potentially serious underestimation of the lipid risk in these patients ad defined by the current criteria.

Oral glycoprotein IIb/IIa antagonists for unstable angina--is there still a chance for the oral substances?

The intravenous glycoprotein IIb/IIIa antagonists abciximab, tirofiban and eptifibatide are well accepted for the therapy of patients with unstable angina and/or as concomitant medication during coronary interventions. Despite the fact that these drugs are not used in all patients presenting with unstable angina during coronary interventions, the scientific evidence is overwhelming including the substantial reduction in mortality 3 years after utilisation of abciximab for coronary interventions in patients with unstable angina. In addition to these two indications, intravenous glycoprotein IIb
