Suffocation by massive blood aspiration due to intraoral gun-shot injury -the oldest museum specimen in the Institute of Forensic Medicine in Belgrade.

The forensic museum specimen presented in this paper is the oldest specimen in the collection of the Institute of Forensic Medicine in Belgrade. It comprises a jar containing six cervical vertebras connected to a small amount of dry connective tissue. During the autopsy, the cervical part of the spine was completely opened posteriorly: the base of the odontoid process of the axis was crushed, but the transverse and posterior longitudinal ligaments and the spinal cord were intact. Attached to the specimen, there is a partly cored piece of lead which looks like a severely deformed handgun projec

A new system of classification for spinal injuries.

BACKGROUND CONTEXT: A comprehensive classification for spinal trauma has proved difficult to achieve as attested to by the number of systems in use today. In addition, few systems can be applied to all levels of the spine, and the first two cervical vertebrae are usually treated as altogether separate from the remainder of the spine. Consequently, outcome data and comparison of published data are difficult, at best. PURPOSE: The purpose of this presentation is to introduce a new system for classification of spine trauma applicable to all levels of the spine. STUDY DESIGN/SETTING: A different

Double insurance atlantoaxial fixation.

BACKGROUND: An alternative technique of atlantoaxial fixation is described, which combines the trans-articular method of fixation described by Magerl in 1982 and the interarticular technique described by us in 1988. METHODS: Between January 2001 and January 2005, 18 patients underwent the discussed method of fixation at the Department of Neurosurgery at King Edward VII Memorial Hospital in Mumbai, India. Fifteen patients had congenital craniovertebral anomaly and 3 patients had posttraumatic atlantoaxial instability. Fourteen patients had basilar invagination with "fixed" atlantoaxial disloca

Os odontoideum with bipartite atlas and segmental instability: a case report.

We report on the case of a 15-year-old adolescent who presented with a transient paraplegia and hyposensibility of the upper extremities after sustaining a minor hyperflexion trauma to the cervical spine. Neuroimaging studies revealed atlantoaxial dislocation and ventral compression of the rostral spinal cord with increased cord signal at C1/C2 levels caused by an os odontoideum, as well as anterior and posterior arch defects of the atlas. The patient underwent closed reduction and posterior atlantoaxial fusion. We describe the association of an acquired instability secondary to an os odontoid

Predictive model for achieving good clinical and radiographic outcomes at one-year following surgical correction of adult cervical deformity.

BACKGROUND: For cervical deformity (CD) surgery, goals include realignment, improved patient quality of life, and improved clinical outcomes. There is limited research identifying patients most likely to achieve all three. OBJECTIVE: The objective is to create a model predicting good 1-year postoperative realignment, quality of life, and clinical outcomes following CD surgery using baseline demographic, clinical, and radiographic factors. METHODS: Retrospective review of a multicenter CD database. CD patients were defined as having one of the following radiographic criteria: Cervical sagitta

Translaminar screws of the axis--an alternative technique for rigid screw fixation in upper cervical spine instability.

C2 pedicle screws or transarticular atlantoaxial screws are technically demanding and carry an increased risk of vertebral artery injury. In up to 20% of cases, pedicle and transarticular screw placement is not possible due to a high-riding vertebral artery or very small C2 pedicles in addition to other anatomical variations. Translaminar screws have been reported to rigidly capture posterior elements of C2 and therefore appear to be a suitable alternative. We present our first experiences and clinical results with this new method in two neurosurgical spine centers. Twenty-seven adult patients

Unrecognized ligamentous instability due to high-energy, low-velocity mechanism of injury.

We report a unique mechanism of injury and illustrate the difficulties of diagnosing purely ligamentous injuries to the cervical spine. To our knowledge, there have been no previous reports of this type of high-energy, low-velocity mechanism of injury. The pattern of injury is also unusual, consisting of anterolisthesis of L4 on L5 with bilateral perched facet joints, atlantoaxial instability and bilateral lower limb fractures. We present a 49-year-old man who experienced a high-energy, low-velocity crush injury that led to extreme hyperflexion of his spine. Standard trauma protocols were carr

Posterior elements disruption with hybrid constructs in AIS patients: is there an impact on proximal junctional kyphosis?

PURPOSE: Proximal junctional kyphosis (PJK) is a frequent proximal adjacent segment disease following spinal fusion in adolescent idiopathic scoliosis (AIS) and its rate has been estimated to 28% in the literature. The etiology is multifactorial, and risk factors associated with PJK are controversial. The aim of this study was to demonstrate that the disruption of muscular and bony tissue above the upper instrumented vertebra (UIV) during surgery does not increase the rate of PJK in patients undergoing posterior fusion for adolescent idiopathic scoliosis. MATERIAL AND METHOD: 50 patients with

Regional instability following cervicothoracic junction surgery.

OBJECT: The cervicothoracic junction (CTJ) is the transitional region between the cervical and thoracic sections of the spinal axis. Because it is a transitional zone between the mobile lordotic cervical and rigid kyphotic thoracic spines, the CTJ is a region of potential instability. This potential for instability may be exaggerated by surgical intervention. METHODS: A retrospective review of all patients who underwent surgery involving the CTJ in the Department of Neurosurgery at the Cleveland Clinic Foundation during a 5-year period was performed. The CTJ was strictly defined as encompassi

[Atloido-axoid surgery by a transoral approach].

The author has approached 18 atlanto-axial joints in 15 years by a transoral approach. Fourteen of the lesions were post traumatic, two congenital, one rheumatoid and one a tumour. It is concluded that the approach is a valuable one provided that there is adequate surgical technique, good antiseptic preparation, antibiotic therapy, provisional peroperative tracheostomy and special retractors. Use of adequate suture is also important. In some cases, magnification was needed. Medullary monitoring may also be helpful. The author estimates that the approach is more effective than extrabuccal techn

Comprehensive drilling of the C1-2 facets to achieve direct posterior reduction in irreducible atlantoaxial dislocation.

OBJECT: The cause of irreducibility in irreducible atlantoaxial dislocation (AAD) appears to be the orientation of the C1-2 facets. The current management strategies for irreducible AAD are directed at removing the cause of irreducibility followed by fusion, rather than transoral decompression and posterior fusion. The technique described in this paper addresses C1-2 facet mobilization by facetectomies to aid intraoperative manipulation. METHODS: Using this technique, reduction was achieved in 19 patients with congenital irreducible AAD treated between January 2011 and December 2013. The C1-2

Atlantoaxial fusion: a biomechanical analysis of two C1-C2 fusion techniques.

BACKGROUND CONTEXT: Different atlantoaxial fusion techniques are used for instability. Transarticular screws are biomechanically superior to wiring techniques and equivalent to C1 lateral mass to C2 pedicle (C1LM-C2P) fixation. Recently, C1 lateral mass to C2 laminar (C1LM-C2L) fixation has been shown to have flexibility similar to C1LM-C2P fixation in flexion, extension, lateral bending, and axial rotation. PURPOSE: Compare the stiffness of C1LM-C2P with C1LM-C2L screw rod fixation. STUDY DESIGN: In vitro biomechanical study. OUTCOME MEASURES: Stiffness in flexion/extension, lateral bendin

[Transaxial spongiosa-plasty and ventral, temporary atlanto-axial fixation for therapy of dens pseudarthrosis].

PURPOSE: According to literature there is a pseudarthrosis rate of about 1% up to 64% depending on the treatment of the dens fracture (16). Generally the treatment of dens pseudarthrosis consists of the fusion of the joint C1/C2 with or without dens resection. Now, a method is presented whereby, on the one hand the pseudarthrosis is treated, while on the other hand the anatomical structures and physiological function of the joint C1/ C2 are restored. METHOD: The operation consists of a gradual outboring of the base of the dens and the dens axis and filling with autologous cancellous bone. The

Injuries of the atlas and axis. A follow-up study of 85 axis and 10 atlas fractures.

In a follow-up study of ten atlas and 85 axis fractures, 12% had residual symptoms in the form of local and radiating cervical pain. The residual symptoms were interpreted as a sign of a mild demyelinating process initiated by the trauma of the medulla. At least 22% of the accidents occurred while the person was under the influence of drugs or alcohol. Therefore, a wide range of residual symptoms (progressive or nonprogressive) could be attributed in part to abuse of drugs and alcohol. A permanent measurable loss of motion occurred following injury to the atlas or axis, irrespective of the mod

Transection of spinal cord. A rare obstetrical complication of cephalic delivery.

A newborn infant, delivered following mid-forceps rotation, presented with apnoea, anaesthesia below the level of the mid-neck, and flaccid quadriplegia. At necropsy there was transection of the cord, and atlanto-occipital and atlantoaxial dislocations. Cord injury usually follows breech presentation, the lesion is in the lower cervical or upper thoracic segments, and results from excessive traction. By contrast, in the rare cases following cephalic delivery, the lesion is most often in the upper cervical cord and probably results from rotational forces.

The management of traumatic spondylolisthesis of the axis.

Fifty-two patients with traumatic spondylolisthesis of the axis were admitted to the University of Maryland Spinal Injury Center between 1977 and 1982. There were fifteen Type-I fractures, twenty-nine Type-II fractures, three Type-IIa fractures, and five Type-III fractures. Associated neurological deficits were found in only four patients, although unassociated neurological deficits such as closed head injury were seen in eleven patients. Thirteen patients had other fractures of the cervical spine. Type-I fractures were stable injuries and were treated with collar protection. Most Type-II inju
