Role of Additional Inferomedial Supporting Screws in Osteoporotic 3-Part Proximal Humerus Fracture: Finite Element Analysis.

INTRODUCTION: Importance of inferomedial supporting screws in preventing varus collapse has been investigated for the proximal humerus fracture. However, few studies reported the results of osteoporotic complex fracture. This study aimed to demonstrate the stress distribution pattern, particularly in osteoporotic 3-part proximal humerus fractures involving greater tuberosity (GT) with different screw configurations. MATERIALS AND METHODS: Using the computed tomography (CT) images of 2 patients, who had osteoporosis and the other had normal bone density, 3-part fractures involving the GT, with

[Animal experiment study of tensile strength of hydroxylapatite-coated AO/ASIF Schanz screws for external fixators].

The removal torque and the loosening and infection rates of AO/ASIF 4.5 mm Schanz screws (MS) have been compared with those of hydroxyapatite-coated (HA) and hydroxyapatite/silver phosphate-coated (HA/P) AO/ASIF Schanz screws in vivo. In sheep, the left tibia was fixed after a midshaft osteotomy by means of unilateral fixator with 6 Schanz screws in an antero-lateral position. The osteotomy gap was filled with bone wax to avoid bony overgrowth. After operation, the animals were allowed to walk around immediately. The loosening rate was 2.8% at the site of the two HA-coated screws and 36% at th

A new concept for implant fixation: bone-to-bone biologic fixation.

Many attempts have been made to reduce complications of bone implant, such as pedicle screw loosening. To address this problem, the authors suggest a new concept of bone-to-bone biologic fixation using recombinant human bone morphogenetic protein-2 (rhBMP-2)-loaded cannulated pedicle screws. Recombinant human bone morphogenetic protein-2 is an osteoinductive cytokine. Four types of titanium pedicle screws were tested (uncannulated, cannulated with no loading, beta-tricalcium phosphate (TCP)-loaded, and TCP/BMP2 loaded) using 16 miniature pigs. Radiological evaluation was conducted to assess th

The "Rescue screw" as a freehand revision strategy for implant loosening: a technical note and comparative biomechanical study on human cadavers.

BACKGROUND CONTEXT: Depending on bone mineral density, implant loosening occurs in up to 54% of cases after spinal instrumentation. This leads to compromised healing, instability and mechanical pain, often requiring surgical revision. Revision strategies strongly depend on the remaining bone stock within the pedicle and include the use of larger diameter or dual pitch screws, implant cementation, alternative screw trajectories such as the cortical bone trajectory (CBT), impaction grafting, and extension to the intact adjacent segment. PURPOSE: This study biomechanically compares screws with a

Biomechanical evaluation of titanium, biodegradable plate and screw, and cyanoacrylate glue fixation systems in craniofacial surgery.

Choice of appropriate fixation after reduction of displaced bone fragments or advancement of osteotomized segments requires knowledge of the maximal force to which these segments can be subjected. The present study was performed to obtain a biomechanical comparison of a variety of resorbable fixation systems as an alternative to metal plates and screws. Sheep cadaver parietal bone segments were osteotomized and fixed with one of six methods of fixation: (A) titanium plates and screws consisting of (1) miniplates and 2.0-mm-diameter screws; (2) midface plates and 1.5-mm screws; (3) microplates

Study of stress distribution in pedicle screws along a continuum of diameters: a three-dimensional finite element analysis.

OBJECTIVE: To create a three-dimensional finite element model to identify the biomechanically optimal diameter of pedicle screws for placement in L1 vertebral bone. METHODS: The effects of pedicle screws with different diameters on the maximum Von Mises stress in L1 vertebra bone were evaluated by a finite element method. Pedicle screw diameters ranging from 4.0 to 6.5 mm were assessed. RESULTS: The simulation showed that, under axially oriented pullout forces, stress was decreased in all models when the diameter of screws was in the range of 4.0 mm to 6.5 mm. With a standard external load a

Fracture risk and initial fixation of a cementless glenoid implant: the effect of numbers and types of screws.

The initial fixation of an anatomical cementless glenoid component, provided by different numbers and types of screws, and the risk of bone fracture were evaluated by estimating the bone-implant interface micromotions and the principal strains around the prosthesis. Four different fixation configurations using locking or compression screws were tested. Estimation of the micromotions at the bone-implant interface was performed both experimentally, using an in vitro model, and computationally, using a numerical model. Principal bone strains were estimated using the numerical model. Subject varia

A novel model of locking plate and locking spongious screw: a biomechanical in vitro comparison study with classical locking plate.

BACKGROUND: Locking plates are commonly used for the fixation of comminuted, periprosthetic and osteoporotic bone fractures. These plates are secured to the bone with screws, creating a stable connection with fixed angle between the plate and the screws. In this biomechanical in vitro study, our aim is to evaluate and compare the novel locking plate-locking spongious screw model with FDA approved classical locking plate. METHODS: Sawbone PCF-15 osteoporotic bone model was utilized to simulate osteoporotic bone conditions. Two screws were used to attach both the classical locking plate and the

Cytotoxicity of a new antimicrobial coating for surgical screws: an in vivo study.

INTRODUCTION: The risk of surgery-related infection is a persistent problem in orthopaedics and infections involving implants are particularly difficult to treat. This study explored the responses of bone and soft tissue to antimicrobial-coated screws. We investigated whether such screws, which have never been used to fix bony tissues, would result in a cytotoxic effect. We hypothesised that the coated screws would not be toxic to the bone and that the likelihood of infection would be reduced since bacteria are not able to grow on these screws. METHODS: Titanium screws were inserted into the

Failure analysis of broken pedicle screws on spinal instrumentation.

Revised spinal surgery is needed when there is a broken pedicle screw in the patient. This study investigated the pedicle screw breakage by conducting retrieval analyses of broken pedicle screws from 16 patients clinically and by performing stress analyses in the posterolateral fusion computationally using finite element (FE) models. Fracture surface of screws was studied by scanning electron microscope (SEM). The FE model of the posterolateral fusion with the screw showed that screws on the caudal side had larger axial stress than those on the cephalic side, supporting the clinical findings t

Poly(D,L-lactide) coating is capable of enhancing osseous integration of Schanz screws in the absence of infection.

Pin loosening is a major complication in external fixation. Biological and mechanical conditions play an important role in the maintenance and enhancement of the implant-bone interface in fracture fixation. It is thought that biodegradable coatings may be capable of preventing pin track infection and pin loosening. The goal of this study was therefore to analyze the influence of a biodegradeable coating on the osseous integration of Schanz' screws during fracture treatment. Standardized osteotomies (3-mm fracture gap) of the right tibiae were performed on 16 sheep and stabilized by an AO mono-

Magnesium alloys as a biomaterial for degradable craniofacial screws.

Recently, magnesium (Mg) alloys have received significant attention as potential biomaterials for degradable implants, and this study was directed at evaluating the suitability of Mg for craniofacial bone screws. The objective was to implant screws fabricated from commercially available pure Mg and alloy AZ31 in vivo in a rabbit mandible. First, Mg and AZ31 screws were compared to stainless steel screws in an in vitro pull-out test and determined to have a similar holding strength (∼40N). A finite-element model of the screw was created using the pull-out test data, and this model can be used f…

Pullout strength of monocortical and bicortical screws in metaphyseal and diaphyseal regions of the canine humerus.

OBJECTIVE: Monocortical screws are commonly employed in locking plate fixation, but specific recommendations for their placement are lacking and use of short monocortical screws in metaphyseal bone may be contraindicated. Objectives of this study were to evaluate axial pullout strength of two different lengths of monocortical screws placed in various regions of the canine humerus compared to bicortical screws, and to derive cortical thickness and bone density values for those regions using quantitative computed tomography analysis (QCT). METHODS: The QCT analysis was performed on 36 cadaveric

Hollow screw fixation of type II avulsion fractures of the calcaneal tuberosity using the finite element analysis.

We aimed to establish a model of type II avulsion fractures of the calcaneal tuberosity treated with 2 hollow screws implanted in different directions and to analyze the biomechanical properties of the model using the finite element method. The Dicom data of the calcaneal bone obtained after computed tomography scan were inputted into Mimics 21.0 software and Geomagic Studio software to generate a 3D finite element digital model of the calcaneal bone. The model was then imported into SOLIDWORKS 2020 software. Based on the Beavis theory, the calcaneal bone was cut to build a type II avulsion fr
