A Liver's Iliad: Liver Graft Retransplant to a Second Recipient After Brain Death in a First Recipient.

With the current increased incidence of cirrhosis, the demand for liver transplant continues to grow. Here, we performed a systematic review … e hospital after an uneventful postoperative course. Among the 19 studies, all donors had confirmed brain death and all transplants included the whole liver, except for 2 cases of auxiliary liver grafts (reduced liver transplant) and 1 case of extended right living-donor liver graft (donor and recipient being related sisters). Overall, among first recipients, …

Postmortem urine concentration of N-terminal pro-brain natriuretic peptide in relation to the cause of death.

UNLABELLED: The utility of biochemical marker analysis in forensic autopsy cases is still uncertain due to the postmortem changes which they undergo. Thus, research is required to elucidate alternative samples and biochemical markers which are less affected by postmortem changes. Levels of N-terminal pro-brain natriuretic peptide (NT-proBNP) are known to be elevated in congestive heart failure (CHF), acute myocardial infarction (AMI), and sepsis patients. Although NT-proBNP is reportedly excreted into the urine, no study has previously evaluated the diagnostic efficacy of urinary concentration

Hemodynamic instability associated with increased risk of death or brain injury in neonates with hypoxic ischemic encephalopathy.

OBJECTIVE: To determine the association between hemodynamic instability requiring inotropes and brain injury or death in neonates with hypoxic ischemic encephalopathy (HIE). METHODS: Retrospective cohort study of 221 neonates with HIE. Brain injury was defined using four HIE patterns based on MRI diffusion or T1 changes. The primary outcome was death or brain injury. Secondary outcomes were abnormal MRI, death, and abnormal EEG. Logistic regression was used to examine the risk of death or brain injury with the use of inotropes while adjusting for confounding factors. RESULTS: Brain injury or

A standardized model of brain death, donor treatment, and lung transplantation for studies on organ preservation and reconditioning.

BACKGROUND: We set a model of brain death, donor management, and lung transplantation for studies on lung preservation and reconditioning before transplantation. METHODS: Ten pigs (39.7 ± 5.9 Kg) were investigated. Five animals underwent brain death and were treated as organ donors; the lungs were then procured and cold stored (Ischemia). Five recipients underwent left lung transplantation and post-reperfusion follow-up (Graft). Cardiorespiratory and metabolic parameters were collected. Lung gene expression of cytokines (tumor necrosis factor alpha (TNFα), interleukin-1 beta (IL-1β), interleu…

Multiple interacting cell death mechanisms in the mediation of excitotoxicity and ischemic brain damage: a challenge for neuroprotection.

There is currently no approved neuroprotective pharmacotherapy for acute conditions such as stroke and cerebral asphyxia. One of the reasons for this may be the multiplicity of cell death mechanisms, because inhibition of a particular mechanism leaves the brain vulnerable to alternative ones. It is therefore essential to understand the different cell death mechanisms and their interactions. We here review the multiple signaling pathways underlying each of the three main morphological types of cell death--apoptosis, autophagic cell death and necrosis--emphasizing their importance in the neurona

Vasopressor agents after experimental brain death: effects of dopamine and vasopressin on vitality of the small gut.

BACKGROUND: Both congenital and acquired short bowel syndrome frequently leads to the necessity for long-term parenteral nutrition, which in turn may lead to any of several complications or death. Transplantation of the small bowel from brain-dead organ donors has been successfully performed over the last years. However, systemic blood pressure and blood perfusion to the splanchnic area decrease rapidly after brain death, which comprises the vitality of the small bowel. OBJECTIVE: To evaluate the differences between dopamine and low-dose vasopressin on perfusion and vitality of the small bowe…

The role of cellular development and cell death in neurochemical organization and integrative brain functions--normal and pathological.

In terms of systemic aspects of common molecular mechanisms of development, important part of which is the process of cellular death and integrative activity of nervous system, a complex clinical-experimental study of effects of various neurotrophic and apoptotic factors (proteins S100b, HLDF, brain lectins CSL and R1) on learning and memory and ischemic stroke was performed. Data concerning specific and heterochronic participation of these factors in neurochemical mechanisms of learning and memory in mechanisms of ischemic stroke formation were established. Changes of examined factors and the

[Diagnosis of brain death in the admission department of the Neurologic Clinic of Erlangen University from 1984 to 1994].

OBJECTIVE: The reliable diagnosis of brain death is a precondition of organ removal for subsequent transplantation. As demographic data, information on the underlying cause and written consent to organ removal from brain-dead patients are largely unknown; protocols relating to the determination of brain death were analysed to obtain these data. PATIENTS AND METHODS: The study was based on an examination of the protocols of 547 consecutive and unselected patients who between 1984 and 1994 had been examined for possible brain death by experienced members of the Neurological Department of Erlang…

Relationships between neuronal loss, age of death, disease duration and MRI brain volumes in progressive supranuclear palsy.

BACKGROUND: Neuronal loss and the presence of tau lesions in specific brain regions is required for a definitive diagnosis of progressive supranuclear palsy (PSP). We previously found tau burden in basal ganglia and red nucleus to be associated with disease duration and age at death. It is currently unknown whether neuronal loss is also associated with disease duration and age at death, or with antemortem brain volume loss in PSP. OBJECTIVE: To investigate whether neuronal loss is associated with tau burden, age at death, disease duration, or brain volume loss in PSP. METHODS: Forty-one path

Practice of Apnea Testing and Determination of Brain Death During Extracorporeal Membrane Oxygenation: A Multicenter Retrospective Analysis.

To explore whether reducing airflow and maintaining blood flow can improve the success rate of the apnea test (AT) and whether two auxiliary test strategies can improve the completion rate of brain death determination (BDD). To perform a multicenter retrospective analysis of the effect of reducing air flow and maintaining constant blood flow during extracorporeal membrane oxygenation (ECMO) on AT parameters and the role of auxiliary detection in the absence of an AT. Among the 31 BDD patients, 18, 10, and 3 had hypoxic-ischemic encephalopathy, stroke, and traumatic brain injury, respectively. …

Inflammatory Signaling Induces Mitochondrial Dysfunction and Neuronal Death in Traumatic Brain Injury via Downregulation of OXPHOS Genes.

Traumatic brain injury (TBI) is a major cause of neurological dysfunction and disability. This study aimed to investigate the transcriptomic changes and the functional consequences in TBI, focusing on the interplay between inflammation and mitochondrial impairment. Brain tissue samples from TBI patients and healthy controls were subjected to RNA-sequencing analysis. Mouse hippocampal HT-22 cells were treated with inflammatory cytokine and the PGC-1α activator ZLN005. Mitochondrial function, oxidative stress, and apoptosis were assessed using Seahorse respirometry, electron microscopy, flow cyt

Brain Death and Its Prediction in Out-of-Hospital Cardiac Arrest Patients Treated with Targeted Temperature Management.

Evolution toward brain death (BD) in out-of-hospital cardiac arrest patients with targeted temperature management (TTM) provides opportunities for organ donation. However, knowledge regarding BD in these patients is limited. We retrospectively analyzed the TTM registry of one hospital where life-sustaining therapy was not withdrawn. In-hospital death patients were categorized into BD and non-BD groups. We explored the process of evolution toward BD and its predictors by comparing the serial measurements of clinical variables and the results of various prognostic tests between the two groups. O…

Infratentorial Brain Injury Among Patients Suspected of Death by Neurologic Criteria: A Systematic Review and Meta-analysis.

BACKGROUND AND OBJECTIVES: There is a paucity of data on the frequency and prognosis of infratentorial brain injury among patients suspected … l responses to somatosensory evoked potentials. At the latest follow-up, 28 had progressed to whole-brain death. DISCUSSION: Studies with moderate to high risk of bias suggest that infratentorial brain injury is relatively uncommon among patients suspected of DNC. Isolated brainstem death is rarer and seems to carry a high risk of progression to whole-brain …
