Bilateral vocal cord palsy due to brain-stem cavernoma--a diagnostic dilemma!

Vocal cord palsy may result from varied causes. However, cord palsy resulting from a brain-stem cavernoma has never been reported. We report a patient with vocal cord palsy in a 30-year-old male resulting from the brain-stem lesion. The patient became symptomatic each time the lesion bled and improved gradually when the bleeding resolved. Repeated insults on the brain-stem produced permanent cord palsy. Although a rare presentation, brain-stem cavernoma may be considered in the aetiology of cord palsy.

Genetically engineered human neural stem cells for brain repair in neurological diseases.

Neural stem cells (NSCs)of the central nervous system (CNS) have recently received a great deal of attention and interest for their therapeutic potential for neurological disorders. NSCs are defined as CNS progenitor cells that have the capacity for self-renewal and multipotent potential to become neurons or glial cells. Recent studies have shown that NSCs isolated from mammalian CNS including human can be propagated in vitro and then implanted into the brain of animal models of human neurological disorders. Recently, we have generated clonally derived immortalized human NSC cell lines via a r

Restoration of brain stem auditory-evoked potential in maple syrup urine disease.

OBJECTIVE: The purpose of this study was to report a unique case of reversible brain stem auditory-evoked potential (BAEP) involving wave I in maple syrup urine disease (MSUD). The possible mechanism of the hearing loss and reversibility of wave I is proposed. PATIENTS: Single case report. INTERVENTION: Treatment of MSUD with dialysis and diet devoid in branched-chain amino acids. MAIN OUTCOME MEASURE: BAEPs. RESULTS: Initial presentation of MSUD in a 14-week-old child included failure to thrive, present otoacoustic emissions, and absent BAEPs. After treatment with dialysis and an MSUD-app…

Neuronal and glioma-derived stem cell factor induces angiogenesis within the brain.

Stem cell factor (SCF) is overexpressed by neurons following brain injury as well as by glioma cells; however, its role in gliomagenesis remains unclear. Here, we demonstrate that SCF directly activates brain microvascular endothelial cells (ECs) in vitro and induces a potent angiogenic response in vivo. Primary human gliomas express SCF in a grade-dependent manner and induce normal neurons to express SCF in brain regions infiltrated by glioma cells, areas that colocalize with prominent angiogenesis. Downregulation of SCF inhibits tumor-mediated angiogenesis and glioma growth in vivo, whereas

Isolated Dandy-Walker malformation associated with brain stem dysgenesis in male sibs.

We describe two brothers with isolated Dandy-Walker malformation (DWM). Interestingly, brain stem dysgenesis and abnormal gyral pattern were also observed in the sibs. They presented with psychomotor retardation and macrocrania. Both suffered from hypotonia with brisk deep tendon reflexes and ataxic gait. They had bilateral optic atrophy and the visual evoked potentials documented prolonged latencies. Further, motor and sensory conduction velocities were normal. Chromosomal examinations for the sibs and their parents showed normal results. The majority of cases are sporadic but rare reports of…

Can mobile phone emissions affect auditory functions of cochlea or brain stem?

PROBLEMS ADDRESSED: Despite their abundant spread, mobile phones are suspected by a major share of the population to cause adverse effects on health and welfare. The ear as the sense organ next to the individual device has rarely been investigated for short-term effects in this regard. In a previous article, we could not prove any impact on the vestibular part of the inner ear. Our present examinations are concerned with the question whether mobile phone emissions could affect cochlear or auditory brain stem functions. METHODS AND MEASURES: In 12 healthy test persons with normal hearing, audi…

Using endogenous neural stem cells to enhance recovery from ischemic brain injury.

The use of cell-based therapy may be a valid therapeutic approach to ischemic brain injury. Stem cells have been proposed as a new form of cell based therapy in a variety of disorders, including acute and degenerative brain diseases. Up to date most efforts have concentrated on transplantation of embryonic stem cells (ESC) or neural stem cells (NSCs) obtained from immortalized cell lines into the diseased brain. These procedures require harvesting the appropriate stem cell, expansion in vitro and transplantation. Endogenous NSCs have been identified in the central nervous system where they res

Quantitative proton MR spectroscopic imaging of normal human cerebellum and brain stem.

Quantitative, multislice proton MR spectroscopic imaging (MRSI) was used to investigate regional metabolite levels and ratios in the normal adult human posterior fossa. Six normal volunteers (36 +/- 3 years, five male, one female) were scanned on a 1.5 T scanner using multislice MRSI at long echo time (TE 280 msec). The entire cerebellum was covered using three oblique-axial slice locations, which also included the pons, mid-brain, insular cortex, and parieto-occipital lobe. Concentrations of N-acetylaspartate (NAA), choline (Cho), and creatine (Cr) were estimated using the phantom replacement

Effects of cholinergic blockers on auditory brain-stem evoked potentials in rats.

The pharmacology of auditory brain-stem evoked potentials (ABEP) pathways is poorly understood. There are anecdotal reports on the involveme … ed for each animal. Atropine reduced the amplitudes of waves P1, P3 and P4 and increased mildly the brain stem transmission time. d-Tubocurarine reduced the amplitudes of P1 and P4 with no significant effect on the peak latencies. Saline injection had no effect on any of the parameters. These results show that both cholinergic systems are involved in ABEP ge …

Diffuse brain stem tumor in an adolescent with multiple enchondromatosis (Ollier's disease).

Among patients with enchondromatosis, those with Ollier's disease are usually considered to be at a lower risk for extra-osseous malignancy than those with Maffucci's disease. However, several reports suggest that Ollier's disease may also be associated with gliomas. We report here the youngest patient in the literature (16 years) to be detected with a brain tumor and Ollier's disease. This is also the first case with diffuse brain stem involvement. Thus, counselling of patients with Ollier's disease may become more difficult than initially thought.

Three-Hertz postural oscillation in patients with brain stem or cerebellar lesions.

Postural instability was quantitatively studied in patients with lesions in the 1) pontine tegmentum, 2) deep cerebellar nuclei, or 3) cerebellar hemisphere. As compared to controls, patients with each lesion demonstrated a characteristic 3 Hertz (Hz) body oscillation. Cross-correlation functions revealed a strong correlation between the body sway and activities in the lower leg muscles. Based on these findings, we conclude that the 3 Hz body oscillation may be the result of any disturbance in the loop of long-latency reflexes mediated by the cerebellum.

Brain-stem trigeminal and auditory evoked potentials in multiple sclerosis: physiological insights.

Thirty-six patients with multiple sclerosis were evaluated by means of brain-stem trigeminal and auditory evoked potentials. The brain-stem auditory evoked potentials (BAEPs) were abnormal in 26 patients (72.2%). Brain-stem trigeminal evoked potentials (BTEPs) yielded similar results, showing distorted waveforms and/or prolonged latencies in 25 patients (69.4%). As expected, the MRI proved to be the most efficient single test, revealing plaques in 86.4% of the patients evaluated. However, the diagnostic accuracy of MRI was lower than that provided by the combination of the BTEP and BAEP (88.9%

CO2 response for the brain stem artery blood flow velocity in man.

We examined changes in the blood flow velocity of brain stem artery (BSA) and middle cerebral artery (MCA) in response to hypercapnic, normocapnic and hypocapnic hyperventilation in seven awake subjects with a transcranial Doppler to determine if there are differences in blood flow control in regional brain perfused by these respective arteries, and to separate the effects of CO2 and ventilation itself on blood flow velocity during CO2 loading. During hypercapnic hyperventilation, BSA flow velocity increased linearly with an increase in end-tidal partial pressure of CO2 (PETCO2). During hypoca…

Multichannel auditory brain stem implant: case studies comparing fitting strategies and results.

A new 8-electrode prosthesis has been developed for individuals deafened by bilateral auditory nerve tumors (neurofibromatosis-2). Twelve patients have received the multichannel auditory brain stem implant at House Ear Institute since 1992, 11 of whom receive useful auditory sensations. Processor fitting includes scaling and ranking of electrode-specific pitch sensations, and determination of any nonauditory sensations. Two representative patients differing in experience and number of usable electrodes are compared in terms of range of auditory sensations and speech perception performance. An …

MRI of the brain stem using fluid attenuated inversion recivery pulse sequences.

Heavily T2-weighted fluid-attenuated inversion recovery (FLAIR) sequences with inversion times of 2000-2500 ms and echo times of 130-200 ms were used to image the brain stem of a normal adult and five patients. These sequences produce high signal from many white matter tracts and display high lesion contrast. The corticospinal and parietopontine tracts, lateral and medial lemnisci, superior and inferior cerebellar peduncles, medial longitudinal fasciculi, thalamo-olivary tracts and the cuneate and gracile fasciculi gave high signal and were directly visualised. The oculomotor and trigeminal ne…
