Intermittently scanned continuous glucose monitoring compared with blood glucose monitoring is associated with lower HbA1c and a reduced risk of hospitalisation for diabetes-related complications in adults with type 2 diabetes on insulin therapies.

AIMS/HYPOTHESIS: We assessed the impact of initiating intermittently scanned continuous glucose monitoring (isCGM) compared with capillary blood glucose monitoring (BGM) on HbA1c levels and hospitalisations for diabetes-related complications in adults with insulin-treated type 2 diabetes in Sweden. METHODS: This retrospective comparative cohort study included adults with type 2 diabetes who had a National Diabetes Register initiation date for isCGM after 1 June 2017. Prescribed Drug Register records identified subgroups treated with multiple daily insulin injections (T2D-MDI) or basal insulin…

Development of a glycated hemoglobin prediction model using continuous glucose monitoring metrics in pediatric type 1 diabetes mellitus: insights into average glucose and recent glycemic trends.

PURPOSE: Given the limitations of glycated hemoglobin (HbA1c), continuous glucose monitoring (CGM) metrics have been proposed as complementary indicators of glycemic control. This study evaluated the association between CGM metrics and HbA1c and developed HbA1c prediction models in Korean pediatric patients with type 1 diabetes mellitus (T1DM). METHODS: We retrospectively analyzed CGM data from 85 patients aged 2-18 years using real-time CGM systems (G6 or G7). CGM records over 12 weeks were segmented into 5 intervals (0-2, 0-4, 4-8, 8-12, and 0-12 weeks) prior to HbA1c measurement. Metrics i…

Continuous glucose monitoring for the prediction of posttransplant diabetes mellitus and impaired glucose tolerance on day 90 after kidney transplantation-A prospective proof-of-concept study.

Posttransplant diabetes mellitus (PTDM) and prediabetes represent serious complications after kidney transplantation and are associated with increased cardiovascular morbidity and mortality. We assessed the predictive performance of continuous glucose monitoring (CGM) compared with plasma glucose and hemoglobin A1c in 46 kidney transplant recipients (KTRs) without known preexisting diabetes mellitus. CGM (14-day recording duration) was performed on days 8, 30, 45, 60, 90, and 180 posttransplant. Eight patients (17%) developed PTDM and nine (20%) impaired glucose tolerance (IGT), as diagnosed b…

Glucose-lowering effects of 7-day treatment with SGLT2 inhibitor confirmed by intermittently scanned continuous glucose monitoring in outpatients with type 1 diabetes. A pilot study.

The present study used intermittently scanned continuous glucose monitoring (isCGM) in 10 patients with type 1 diabetes mellitus (T1DM) to evaluate the efficacy and safety of 7-day outpatient treatment with the combination of intensive insulin therapy and sodium-glucose transporter 2 inhibitor (SGLT2-I). All participants wore isCGM and were treated with either 50 mg/day ipragliflozin or 5 mg/day dapagliflozin. The primary outcome, percent time with glucose at 70-180 mg/dL (TIR: time in range), improved significantly following the addition of SGLT2-I (p = 0.005). TIR increased from 36.0% before…

Threshold and Predictive Alerts of Continuous Glucose Monitoring and Glycemic Control in Hospitalized Adults With Diabetes: A Randomized Clinical Trial.

IMPORTANCE: Interest in use of continuous glucose monitoring (CGM) in the hospital is increasing, yet clear guidance on the optimal implementation of CGM glucose alerts is lacking. It also remains unclear if alerts themselves or the fact that sensor values are available in real time are the main reason for improved glucose outcomes observed with inpatient CGM use. OBJECTIVE: To determine the incremental value of CGM threshold and predictive alerts for glucose management among hospitalized adults with diabetes. DESIGN, SETTING, AND PARTICIPANTS: This single-center, open-label, randomized clin…

Real-world use of continuous glucose monitoring in people with type 2 diabetes and chronic kidney disease or on dialysis.

INTRODUCTION: Continuous glucose monitoring (CGM) has become the standard of care for people with type 1 diabetes (T1D) and some people with type 2 diabetes (T2D). However, there is a lack of data regarding CGM use in people with T2D and chronic kidney disease (CKD), who are at increased risk of hypoglycaemia. We assess the use of CGM and glucose outcomes in this cohort. METHODS: Retrospective, observational analysis of adults on CGM attending a tertiary diabetes renal clinic between January 2023 and September 2024. People with T2D on multiple daily insulin injections (defined as 2 or more in…

Advances in Continuous Glucose Monitoring and Integrated Devices for Management of Diabetes with Insulin-Based Therapy: Improvement in Glycemic Control.

Continuous glucose monitoring (CGM) technology has evolved over the past decade with the integration of various devices including insulin pumps, connected insulin pens (CIPs), automated insulin delivery (AID) systems, and virtual platforms. CGM has shown consistent benefits in glycemic outcomes in type 1 diabetes mellitus (T1DM) and type 2 diabetes mellitus (T2DM) treated with insulin. Moreover, the combined effect of CGM and education have been shown to improve glycemic outcomes more than CGM alone. Now a CIP is the expected future technology that does not need to be worn all day like insulin…

Continuous Glucose Monitors and Activity Trackers to Inform Insulin Dosing in Type 1 Diabetes: The University of Virginia Contribution.

Objective: Suboptimal insulin dosing in type 1 diabetes (T1D) is frequently associated with time-varying insulin requirements driven by various psycho-behavioral and physiological factors influencing insulin sensitivity (IS). Among these, physical activity has been widely recognized as a trigger of altered IS both during and following the exercise effort, but limited indication is available for the management of structured and (even more) unstructured activity in T1D. In this work, we present two methods to inform insulin dosing with biosignals from wearable sensors to improve glycemic control
