Saccadic velocity analysis in patients with divergence paralysis.

Twelve patients with clinical findings of divergence paralysis underwent horizontal saccadic velocity testing. Peak velocities were measured using digitally sampled electro-oculography during 10 degrees, 20 degrees, and 30 degrees saccades. Results were compared with those of 12 age-matched controls. The peak saccadic velocities of each eye in adduction and 10 degrees abduction did not differ from those of age-matched controls (P > .05). The 20 degrees and 30 degrees abducting saccades showed mildly reduced saccadic velocities when compared with controls (P < .05). The finding of only mildly r

[Postures and abnormal paroxysmal movements during sleep: hypnogenic paroxysmal dystonia or partial epilepsy?].

In recent years, a differentiation has been made between two syndromes that are characterized by brief abnormal paroxysmal movements occurring principally at night: 1, hypnogenic paroxysmal dystonia (HPD), sometimes considered a particular form of dystonia similar to paroxysmal kinesigenic choreoathetosis, and 2, mesiofrontal epilepsy. Whether HPD is a distinct syndrome is not clear. Twenty-three patients, 11 men and 12 women, were hospitalized between 1985 and 1989 for examination of this type of abnormal paroxysmal movements (APM) occurring at night. In order to clarify the physiopathology o

[Contribution of oculography and magnetic resonance imaging to the clinico-topographic correlates in ischemic vascular disorders of the brainstem].

6 patients with brainstem infarction and oculomotor signs were studied. One case was consistent with a mesencephalic infarct (internuclear ophthalmoplegia); 4 cases were consistent with a protuberantial infarct (3 cases of "one and a half" syndrome and one case of pontine reticular syndrome associated with a palsy of the root of the VIth nerve. One case was a laterobulbar syndrome. These 6 patients were studied with CT scan and magnetic resonance imaging (MRI CGR Magniscan 5000 with supraconductor magnet of 0.5 Tesla) with T2 weighted images (TR = 2000 ms, TE = 60; 120 ms) in joined section of

Looking but not seeing: attention, perception, and eye movements in simultanagnosia.

We studied three subjects who reported the apparent "disappearance" of stationary objects from direct view. They had simultanagnosia caused by CT-verified bilateral superior occipital lobe lesions. They had no abnormalities of visual acuity or fields to explain their defect. EOG with computer analysis showed intact motility and scanning. Most important, the subjects reported intermittent disappearance of a light target during EOG-verified fixation--ie, they were looking but not seeing. Results indicate that attention mechanisms that permit sustained awareness of visual targets depend on the su

Lithium effects on ophthalmological-electrophysiological parameters in young healthy volunteers.

The effects of lithium on the visual system in man have been tested. Ten healthy volunteers followed a 10-day lithium regimen. Lithium plasma levels in the range of 0.6-0.8 mmol/l were obtained. Clinically used ophthalmological-electrophysiological tests were performed. Most of the tests showed no significant change during the lithium treatment. The lithium regimen resulted in a strong reduction of the slow phase. Also the "Arden-ratio", measured by electrooculogram (EOG), was influenced by lithium. The "Arden-ratio" reflects the corneo-fundal potential in retinal illumination and dark adaptat

Vitreous fluorophotometry in congenital stationary nightblindness.

Vitreous fluorophotometry was performed on eyes with three kinds of congenital stationary nightblindness: Oguchi's disease (two cases), X-linked congenital stationary nightblindness (two cases), and fundus albipunctatus (one case). The results were compared with the electroretinogram (ERG) and the electro-oculogram (EOG). In spite of the variable degree of abnormality of the ERG and EOG, all eyes showed normal values on vitreous fluorophotometry. These results strongly suggest that the blood-retinal barrier in congenital stationary nightblindness is intact and that the electrophysiological res

The effects of the "vestibular sedative" drug, Flunarizine upon the vestibular and oculomotor systems.

The effects of the "vestibular sedative" drug Flunarizine upon the oculomotor functions of pursuit and voluntary saccades and upon the vestibular response (to rotational stimuli) were assessed in twenty volunteer subjects. The study was then extended to three patients with chronic imbalance of central origin who had reported a beneficial symptomatic response to the drug. Three of the volunteer subjects were found to have a directional preponderance (presumed to arise from peripheral dysfunction). In the remaining seventeen normal subjects Flunarizine was found to reduce the amplitudes of fast

Assessment of obstructive sleep apnoea treatment success or failure after maxillomandibular advancement.

Maxillomandibular advancement (MMA) is an alternative therapeutic option that is highly effective for treating obstructive sleep apnoea (OSA). MMA provides a solution for OSA patients that have difficulty accepting lifelong treatments with continuous positive airway pressure or mandibular advancement devices. The goal of this study was to investigate the different characteristics that determine OSA treatment success/failure after MMA. The apnoea-hypopnoea index (AHI) was used to determine the success or failure of OSA treatment after MMA. Sixty-two patients underwent MMA for moderate and sever

Retinal features in Mulvihill-Smith syndrome.

BACKGROUND: Mulvihill-Smith syndrome is a rare progeroid syndrome of sporadic nature. Previously reported ophthalmological findings include astigmatism, myopia, endothelial dystrophy, keratoconus, cataract, band keratopathy, meibomian gland dysfunction, dry eye disease, amblyopia, and allergic conjunctivitis. MATERIALS AND METHODS: The proband, a 25-year-old male subject diagnosed with Mulvihill-Smith syndrome in childhood developed retinal changes with onset of adulthood. The retinal changes were monitored for progression with fundus photography, electrodiagnostic tests, and spectral domain

Visual symmetry in objects and gaps.

It is known that perceptual organization modulates the salience of visual symmetry. Reflectional symmetry is more quickly detected when it is a property of a single object than when it is formed by a gap between two objects. Translational symmetry shows the reverse effect, being more quickly detected when it is a gap between objects. We investigated the neural correlates of this interaction. Electroencephalographic data was recorded from 40 participants who were presented with reflected and translated contours in one- or two-object displays. Half of the participants discriminated regularity, h

Binasal visual field defects are not specific to vigabatrin.

This study investigated the visual defects associated with the antiepileptic drug vigabatrin (VGB). Two hundred four people with epilepsy were grouped on the basis of antiepileptic drug therapy (current, previous, or no exposure to VGB). Groups were matched with respect to age, gender, and seizure frequency. All patients underwent objective assessment of electrophysiological function (wide-field multifocal electroretinography) and conventional visual field testing (static perimetry). Bilateral visual field constriction was observed in 59% of patients currently taking VGB, 43% of patients who p

Inter-hemispheric asynchrony of the brain during events of apnoea and EEG arousals.

Sleep apnoea hypopnea syndrome (SAHS) is a serious sleep disorder affecting a large percentage of the population. Apnoea/hypopnea and electroencephalographic-arousal (EEGA) events occur frequently in SAHS patients. These events significantly disturb the sleep architecture, as revealed through nocturnal EEG signals. Even though EEG carries vital information on the state of the brain, its use in clinical SAHS diagnosis is limited mainly to routine sleep staging. In this paper, we address this issue. We propose a novel measure, called the inter-hemispheric asynchrony (Psi(a-->b)), to capture EEG-
