The longitudinal dispositions of people diagnosed with adjustment or severe stress disorders.

BACKGROUND: Adjustment and stress-related disorders are prevalent among psychiatric service users. Despite their prevalence, little is known about their prognosis. To reduce that gap, the present article documents the service use and diagnostic outcomes of people with adjustment or stress-related disorders presenting at Singapore's largest psychiatric emergency department. METHODS: Administrative data from 2014 to 2021 was retrieved to follow a group of 683 service users whose first-ever psychiatric presentation in 2014 warranted a diagnosis of adjustment or stress-related disorder. People we

Lack of emergency hand surgery: discrepancy between elective and emergency hand care.

Wrist, hand, and finger trauma are the most common injuries presenting to emergency departments. Shortage of emergency hand care is an emerging problem, as on-call hand coverage declines. This study evaluates the availability of elective and emergency hand surgery services in Tennessee, with the use of telephone surveys administered to emergency department and operating facility management. One hundred eleven Tennessee hospitals completed the surveys (93% response rate). In all, 77% of hospitals offer elective hand surgery, 58% offer basic emergency hand services, 18% offer occasional hand spe

Overcoming Legislative and Institutional Barriers to Implementing Methadone Dispensing in a California Health System.

BACKGROUND: Opioid overdose deaths increased markedly during the COVID-19 pandemic, highlighting the need to optimize access to medications for opioid use disorder (MOUD). Hospitalization presents a key opportunity to initiate treatment. However, methadone access is uniquely limited by federal and state regulations requiring dispensing through Opioid Treatment Programs (OTPs). This frequently delays hospital discharge, prolongs inpatient length of stay, or necessitates return emergency department (ED) visits when OTPs are inaccessible. In March 2022, the Drug Enforcement Administration permitt

The CCEDRRN COVID-19 Mortality Score to predict death among nonpalliative patients with COVID-19 presenting to emergency departments: a derivation and validation study.

BACKGROUND: Predicting mortality from COVID-19 using information available when patients present to the emergency department can inform goals-of-care decisions and assist with ethical allocation of critical care resources. The study objective was to develop and validate a clinical score to predict emergency department and in-hospital mortality among consecutive nonpalliative patients with COVID-19; in this study, we define palliative patients as those who do not want resuscitative measures, such as intubation, intensive care unit care or cardiopulmonary resuscitation. METHODS: This derivation

Direct admission to the intensive care unit for patients with prehospital septic shock is associated with a lower 30-day mortality rate.

In order to reduce sepsis mortality, early treatment implementation and referral to the most appropriate ward (emergency department (ED) or intensive care unit (ICU) are recommended. This multicentre retrospective study aims to study the relationship between 30-day mortality and the admission mode to ICU or ED among patients with septic shock cared for by a prehospital mobile Intensive Care Unit (mobile-ICU). From May 2016 to December 2022, septic shock patients cared for by a prehospital mobile-ICU were retrospectively analysed. To assess the relationship between 30-day mortality rate and the

Effect of pharmacist care on medication adherence and cardiovascular outcomes among patients post-acute coronary syndrome: A systematic review.

BACKGROUND: The impact of collaborative and multidisciplinary health care on the outcomes of care in patients with acute coronary syndromes (ACS) is well-established in the literature. However, there is lack of high quality evidence on the role of pharmacist care in this setting. OBJECTIVE: This systematic review aimed to evaluate the impact of pharmacist care on patient outcomes (readmission, mortality, emergency visits, and medication adherence) in patients with ACS at or post-discharge. METHODS: The following electronic databases and search engines were searched from their inception to Se

Avoid construction nightmares by taking charge of the process and setting rules.

A construction crew in your ED guarantees some level of disruption, but you can keep your department functioning well by hiring contractors familiar with health care settings and establishing ground rules up front. The ED manager must be firm about patient safety and unnecessary disruptions. You can reasonably expect contractors and their crews to behave appropriately in your ED. Don't stop construction work unless necessary, or you may incur costly penalties.

[Use of laparoscopic trocars in chest tube insertion: a rapid and safe technique].

Insertion of chest drain is always a life saving procedure in emergent conditions. Especially in crowded traffic accidents more than one patient has brought to the emergency service. In this study we review our experience in chest drainage by using classic laparoscopic trocars. During the period between August 1997, and March 2000 chest tubes were inserted using laparoscopic trocars in 27 patients in emergency service, Yozgat State Hospital. We have no complications related to use of this technique. We concluded that use of laparoscopic trocars in chest tube insertion in emergency conditions i

Updated European Society for Emergency Medicine Guidelines for acute pain management in emergency departments and prehospital care.

Acute pain management in European emergency settings faces persistent challenges, including inadequate clinical knowledge, training deficits, heightened emphasis on opioid stewardship, and concerns regarding drug-seeking behaviors. Despite previous guidelines, oligoanalgesia remains prevalent, with many patients experiencing suboptimal pain control. The evolving clinical landscape necessitated updating the 2020 European Society for Emergency Medicine (EUSEM) guidelines, considering emergent technological advances, the ongoing opioid crisis, aging populations, and continued pressure on emergenc

A survey of national physicians working in an active conflict zone: the challenges of emergency medical care in Iraq.

INTRODUCTION: There has been limited research on the perspectives and needs of national caregivers when confronted with large-scale societal violence. In Iraq, although the security situation has improved from its nadir in 2006-2007, intermittent bombings, and other hostilities continue. National workers remain the primary health resource for the affected populace. PROBLEM: To assess the status and challenges of national physicians working in the Emergency Departments of an active conflict area. METHODS: This study was a survey of civilian Iraqi doctors working in Emergency Departments (EDs)

Prediction Model for Children With Anaphylaxis Who May Not Require Emergency Department Care: A Multicenter Retrospective Cohort Study.

BACKGROUND: The 2023 US Anaphylaxis Practice Parameter indicated that some patients receiving epinephrine in the community might avoid emergency department (ED) visits; however, the criteria and safety of this approach remain unproven. OBJECTIVE: To derive and validate a clinical prediction model for identifying children at risk of receiving epinephrine after arriving at the ED. METHODS: We conducted a 31-center retrospective cohort study of children aged 6 months to less than 18 years who received 1 prehospital dose of epinephrine for allergic reactions between 2016 and 2019. The outcome wa

Clinical management where medicine meets management. All of a rush.

The time between a myocardial infarction patient arriving at hospital and receiving life-saving drugs is decreasing. Schemes to speed up care include paramedics administering thrombolytic drugs. Hospitals are also finding ways of speeding up admission patients.

Emergency department use during pregnancy: a prospective observational study in a single center institution.

PURPOSE: Gynecologists working in emergency department services in hospitals lately have the impression that the number of pregnant women who present themselves because of anxiety or uncertainty is increasing. Hence, the aim of this study was to assess reasons of pregnant women for presenting themselves to an emergency department. Of special interest was how far a diagnosis could be confirmed for the symptoms pregnant women complained about. METHODS: This is a prospective questionnaire-based study conducted between April 2015 and April 2016 in the Department of Gynecology and Obstetrics of th

Unplanned Healthcare Utilization After Emergency Laparotomy: A Systematic Review and Meta-Analysis.

BACKGROUND: Readmissions following emergency laparotomy are frequent, and mainly a result of late-onset postoperative complications, worsening of pre-existing comorbidities, and non-compliance with care instructions. This review aimed to describe current evidence on readmission after emergency laparotomy. METHODS: A systematic search was performed in MEDLINE, Embase, and the Cochrane Library. Studies were included if they reported readmission incidence following non-trauma emergency laparotomy for perforated viscera, intestinal obstruction, mesenteric ischemia, or intraabdominal bleeding in a

SARS-CoV-2 and Adolescent Psychiatric Emergencies at the Tübingen University Hospital: Analyzing Trends, Diagnoses, and Contributing Factors.

Psychiatric emergencies have increased in recent decades, particularly with the onset of the SARS-CoV-2 pandemic, and far too little is known about the backgrounds of these emergencies. In this study, we investigated the extent to which the number of psychiatric emergencies changed during and in the aftermath of the SARS-CoV-2 pandemic at the Child and Adolescent Psychiatry (CAP) Tübingen. We considered age and psychiatric diagnoses. Additionally, we evaluated the backgrounds of the emergencies. We applied a mixed- (quantitative and qualitative) methods approach to data on emergency presentati

Evaluating effect of implementing Gibbs' reflective model on quality of nurses' documentation in emergency departments: a clinical trial.

BACKGROUND: Emergency nurses are responsible for providing and documenting immediate patient care. Documentation is a fundamental component of their professional practice; however, evidence indicates that documentation standards are often not adequately met. Therefore, innovative training approaches should be employed to improve the quality of nurses' documentation. The present study aims to evaluate the effect of implementing Gibbs' reflective model on the quality of documentation by emergency department nurses. METHODS: This cluster-randomized clinical trial was conducted on 68 nurses worki
