Xanthomatous hypophysitis causing hypogonadotropic hypogonadism resulting in delayed presentation of slipped capital femoral epiphysis.

An 18-year-old man who underwent bilateral pinning of his hip joints after a left unstable Slipped Capital Femoral Epiphysis (right pinned prophylactically) was noted to have delayed secondary sexual characteristics and post-operative diabetes insipidus. The patient also described a history of fatigue, headache and polydipsia for the past 4 years. Endocrine investigations revealed reduced androgen levels, hypocortisolism, a borderline normal Serum ACE and secondary hypothyroidism. Magnetic Resonance Imaging of the pituitary gland identified an enhancing mass and a thickened stalk which trans-n

Patterns of Pain in Adolescents with Slipped Capital Femoral Epiphysis.

OBJECTIVE: To prospectively characterize pain locations in slipped capital femoral epiphysis (SCFE) and evaluate pain locations as predictors of a delay in diagnosis. STUDY DESIGN: This was an institutional review board approved prospective study of 110 children who underwent surgery for SCFE at a tertiary children's hospital between 2009 and 2015. Standardized pain diagrams were completed by 107 children. Pain zones were designated via a composite diagram. Hips without hip pain were categorized as atypical; hips with hip pain were typical. RESULTS: In total, 122 hips were eligible for pain

Revised modified Oxford bone score: a simpler system for prediction of contralateral involvement in slipped capital femoral epiphysis.

BACKGROUND: The decision to prophylactically treat the uninvolved hip in slipped capital femoral epiphysis (SCFE) remains controversial. The modified Oxford bone score (mOBS) is predictive of future contralateral involvement in slipped capital femoral epiphysis. The scoring method for this system is challenging to remember because out-of-context irrational number sequences and total score range (16 to 26) are used. This study was performed to evaluate intraobserver and interobserver reliability of the mOBS and to determine whether revising the scoring sequence to 0 to 2 for all 5 categories (t

[Rôle of growth hormone therapy in the genesis of slipped capital femoral epiphysis].

The authors report two cases of children treated with growth hormone who had a secondary slipped capital femoral epiphysis. Endocrine disease, radiotherapy are some well-known etiological factors reported in literature, but growth hormone influence is still discussed. This hormone could have an indirect incidence on this pathology because of its effects on growth cartilage. The increased number of treatment with growth hormone is certainly a risk factor for slipped capital femoral epiphysis. Regular checking from "France Hypophyse" allows an early diagnosis of epiphysis and surgical fixation c

The management of severe avascular necrosis following slipped capital femoral epiphysis by transtrochanteric rotational osteotomy. Results of successful treatment in two cases with longterm follow-up.

We present two cases of unstable slipped capital femoral epiphysis complicated by avascular necrosis that were successfully treated with transtrochanteric anterior rotational osteotomy. These two cases both had severe avascular necrosis and were felt to be unsalvageable. We recommend this surgical technique as a viable alternative to other forms of treatment in this selected patient population.

Slipped capital femoral epiphysis. Results of treatment with a single cannulated screw.

Thirty patients with 36 slipped capital femoral epiphysis underwent pinning with a single cannulated screw. All patients were pinned in situ, allowed immediate weight bearing as tolerated, discontinued crutches at 6 weeks, and had a minimum of 1 year follow up (average 21 months). One patient had avascular necrosis, one patient had bilateral chondrolysis, and two patients required repinning.

Proximal femoral epiphysiolysis in the neonate.

Seven neonates with eight proximal femoral epiphysiolyses were reviewed. Pain elicited by leg motion was the common diagnostic finding in all. Radiography showed lateralization of the shaft similar to congenital hip dysplasia. However, the acetabular index was normal in all but one child. Arthrography confirmed the diagnosis, showing a located proximal femoral epiphysis and a laterally displaced, externally rotated proximal metaphysis. Treatment usually consisted of traction followed by abduction splinting. The long-term results were excellent in six fractures. Growth complications included an

Adolescent traumatic dislocations of the shoulder with open epiphyses.

Nine of 212 cases of traumatic anterior shoulder dislocations occurred in children with clearly open epiphyses (4.7% incidence). All cases associated with psychological or physical abnormalities were excluded. A high recurrence rate of 80% (8 of 10) was noted requiring operative intervention. Two of the three remaining unoperated cases had a history of "subluxation" after their initial dislocation.

A retrospective study of 125 cases of slipped capital femoral epiphysis.

A series of 125 cases of slipped capital femoral epiphysis in 94 patients was followed an average of 4.7 years. Initial treatment consisted of closed reduction with internal fixation in 13 cases, cervical cuneiform osteotomy with internal fixation in nine, and in situ fixation in the remainder. The end-results were good in 82% of the cases. Of the nine failures, eight were due to avascular necrosis or chondrolysis. Closed reduction should be performed only in acute or acute-on-chronic slippings. Chronic slippings should be nailed in situ. The use of more involved procedures, such as cuneiform

[Southwick osteotomy in stable slipped capital femoral epiphysis: a long-term outcome study].

OBJECTIVES: Moderate to severe chronic stable slipped capital femoral epiphysis (SCFE) produce permanent irregularities in the femoral head and acetabulum. We report a single centre long-term outcome study of Southwick's procedure for the management of moderate or severe stable chronic SCFE with irregularity of the femoral head. METHODS: In the period from January 1978 to January 1990, 20 patients (16 girls, 4 boys; mean age 13 years; range 11 to 15 years) underwent 22 Southwick osteotomies for chronic stable SCFE with closure or partial closure of the growth plate and a slip between 30 degre

[Pediatric Hip Disorders].

Developmental dysplasia of the hip is a condition characterized by hip joint instability due to acetabular dysplasia in infancy, necessitating precise ultrasound examination. Legg-Calvé-Perthes disease is caused by a temporary disruption in blood flow to the femoral head during childhood, progressing through avascular, fragmentation, re-ossification, and residual stages. Slipped capital femoral epiphysis is a condition where the femoral head shifts medially along the epiphyseal line during adolescence due to stress, such as weight-bearing. Differentiating between transient hip synovitis and se

Slipped capital femoral epiphysis with hypopituitarism in adults: A case report and literature review.

RATIONALE: Slipped capital femoral epiphysis (SCFE) is a common disease in pediatric orthopedics. Most research on SCFE has focused on high-risk groups or the whole population, and studies focusing on adult SCFE patients are rare. In the present study, we report the case of an adult patient with SCFE. PATIENT CONCERN: A 37-year-old man presented to our clinic with persistent pain that was poorly localized to both hips, groin regions, and thighs for more than 1 year. DIAGNOSES: A bilateral hip X-ray examination was performed, and the femoral epiphyses were found to be unfused on both sides. L

End-stage coxarthrosis and gonarthrosis. Aetiology, clinical patterns and radiological features of idiopathic osteoarthritis.

OBJECTIVES: To determine and compare the aetiological background, clinical patterns and radiological features of idiopathic osteoarthritis (OA) of the hip and the knee warranting arthroplasty. METHODS: A total of 402 Caucasians consecutively undergoing total hip replacement (THR) or total knee replacement (TKR) for idiopathic OA at a major centre was surveyed. RESULTS: Previous joint injury was more common in the TKR group (P < 0.0001). However, both groups manifested a mixed occupational background, body mass indices similar to the general population and a predominance of females (F:M = 1.3

Arthroscopic osteochondroplasty in patients with mild slipped capital femoral epiphysis after in situ fixation.

PURPOSE: Mild slipped capital femoral epiphyses (SCFE) nevertheless show significant femoral head-neck deformities which may put cartilage and acetabular labrum at risk. Whether this deformity can be restored to normal has not yet been described in the literature. METHODS: In a prospective follow-up study, 14 patients with mild SCFE underwent in situ fixation with a single 6.5-mm cancellous, partially threaded screw. In 14 patients arthroscopic osteochondroplasty was performed, and in 13 patients pre- and postoperative measurements of the α-angle were made using antero-superior radial magneti
