Update on MR Imaging Techniques of the Hip.

MR imaging protocols and technical parameters should be optimized in order to achieve the best diagnostic results. Routine hip MR imaging includes a combination of T1-weighted and fluid-sensitive sequences, obtained in coronal, axial, and sagittal planes. MR arthrography of the hip can be used for assessment of intra-articular pathology involving intricate and signal-poor structures such as articular cartilage, labral fibrocartilage, and intra-articular bodies. Emerging quantitative MR imaging techniques such as T1ρ, T2 mapping, and delayed gadolinium-enhanced MR imaging of cartilage allow for…

Chondrocalcinosis of the temporomandibular joint: a rare but important diagnosis.

Patients with preauricular masses present with a wide range of pathologies, the most common of which are neoplasms of the parotid gland, metastatic or inflammatory disease, and enlarged periparotid lymph nodes. Other possibilities include lesions that arise from the skull base or the temporomandibular joint (TMJ). Chondrocalcinosis is a common age-related phenomenon that has a predilection for fibrocartilage. Although it can involve any joint, the knee is by far the most common site; involvement of the TMJ is very rare. We describe a case of chondrocalcinosis of the TMJ that manifested as a pr…

Galeazzi fractures and dislocations.

In 1934, fractures of the middle and distal third of the radius associated with instability of the distal radial ulnar joint (DRUJ) were described by Galeazzi. This type of lesion is characterized by its unstable nature and the need for open reduction and internal fixation to achieve a satisfactory functional outcome. A high index of suspicion should be maintained by the surgeon, and a thorough examination for instability of the DRUJ must be conducted. The marked instability of this fracture-dislocation complex is further enhanced by the disruption of the triangular fibrocartilage complex, eit…

Suture fixation as a treatment for acute traumatic osteochondral lesions.

This is a report on the treatment of traumatic osteochondral lesions with suture fixation. Equivocal results from traditional, conservative treatment modalities have resulted in a recent surge in research in this area. Literature reports of treatments for these lesions often rely on a fibrocartilage healing response that exhibits inadequate weight bearing, lubricating, and durability qualities. We describe here a method by which a large, intact osteochondral fragment over the articulating, weight-bearing portion of the patella may be repaired using suture. This method allows for the preservati…

Management of Ulnar Styloid Nonunions.

Ulnar styloid fractures commonly occur in the setting of distal radius fractures and often progress to asymptomatic nonunion. Displaced basi … matic outcomes can be achieved with nonunion repair or fragment excision with or without triangular fibrocartilage complex repair, depending on the location and size of the ulnar styloid fracture.

Albumin-Coated Polycaprolactone (PCL)-Decellularized Extracellular Matrix (dECM) Scaffold for Bone Regeneration.

With the emphasis on collagen and hydroxyapatite, the main structural components of bone tissue, synthetic grafts fall short of matching the clinical efficacy of autologous bone grafts. Excluded non-collagenous protein (NCPs) and carbohydrates also participate in critical cell signaling cascades and guide mineral deposition during intermediate stages of bone healing. By mimicking the native fracture repair process, polymeric scaffolds that incorporate calcium-binding moieties present in fibrocartilage can potentially enhance their bioactivity, mineralization, and bone growth. Likewise, coating…

The role of arthroscopy in the treatment of wrist injuries in the athlete.

Wrist arthroscopy provides an ideal means of evaluating intraarticular soft-tissue injuries of the wrist. Many lesions such as tears of the triangular fibrocartilage complex can be treated by arthroscopic means as well. Reduction of articular fractures of the distal radius and pin fixation, reduction of scaphoid fractures and intramedullary fixation, and arthroscopic reduction and transcutaneous pin stabilization of acute carpal dissociation patterns can all be accomplished with minimally invasive techniques under arthroscopic control. These measures often provide the athlete with shorter peri…

Ligamentous anatomy of the medial carpal region.

The purpose of this study is to elucidate the ligamentous anatomy of the medial carpus in order to characterize the pathologic basis of pisiform subluxation secondary to traumatic ligament rupture. Medial carpal ligamentous anatomy has been studied in the past by anatomic dissection and analyzed in terms of comparative morphology. A review of this literature reveals that the anatomy of the region requires further definition. We have performed cadaveric dissections that reveal that the ulnar or medial carpal ligament complex is composed of three structures: (1) the triangular fibrocartilage, (2…

Chondrocyte Viability 3.5 Years Following a Vascularized Medial Femoral Trochlea Osteocartilaginous Free Flap.

CASE: A 30-year-old woman underwent arthroscopy for wrist pain 3.5 years following a medial femoral trochlea (MFT) osteocartilaginous free flap for Kienböck disease, which provided the opportunity to examine the transferred cartilage in vivo. Arthroscopy revealed no evidence of chondrolysis, and histologic examination revealed uniformly viable chondrocytes within a matrix consistent with fibrocartilage. CONCLUSION: To the best of our knowledge, the long-term viability of chondrocytes following an MFT free flap has not been previously documented. This case provides early evidence that free tis…

[MRI in the diagnosis and differential diagnosis of radiocarpal diseases and injuries].

Advances in MRI technology have greatly improved visualization of the radio-carpal region. With todays high-resolution imaging, even very small anatomic structures like the triangular fibrocartilage may be seen. Established indications include the osteonecroses (scaphoid and lunate); however, MR images always should be evaluated in conjunction with plain radiographs. By delineating bone marrow edema, MRI is well suited for the detection of microfractures. Visualization of small soft tissue tumors, soft tissue extension of bone tumors and for staging of tumors in general may also be regarded as…

FSP1/S100A4-Expressing Stem/Progenitor Cells Are Essential for Temporomandibular Joint Growth and Homeostasis.

The temporomandibular joint (TMJ) is one of the most used joints in the body. Defects and wear in the cartilage of the joint, condyle, and fibrocartilage disc lie at the heart of many common TMJ disorders. During postnatal development, the condyle acts as a growth center for the mandible, with cells moving as a conveyor belt away from the top of the condyle as they differentiate. The superficial layers of the condyle have been proposed to contain stem/progenitor populations to allow growth and maintain homeostasis. Here we have focused on the role of fibroblast-specific protein 1 (FSP1; also k…

Increase of weight-bearing capacity of patients with lesions of the TFCC using a wrist brace.

STUDY DESIGN: Retrospective cross-sectional case series. BACKGROUND: Lesions of the triangular fibrocartilage complex (TFCC) can result in pain during axial load and unstable distal radioulnar joint (DRUJ). Conventional wrist orthoses decrease initial pain sufficiently but also prevent any movement during recovery and do not contribute to the stabilization of the DRUJ. PURPOSE: In this retrospective analysis, we tested if the weight-bearing capacity of patients with lesions of the triangular fibrocartilage complex was increased by wearing a brace that stabilizes the distal radioulnar joint. …

An immunohistochemical study of the tissue bridging adult spondylolytic defects--the presence and significance of fibrocartilaginous entheses.

UNLABELLED: Introduction Spondylolytic spondylolisthesis is an osseous discontinuity of the vertebral arch that predominantly affects the fi … t fibrocartilaginous entheses at both ends. Typically, these had zones of calcified and uncalcified fibrocartilage labelling strongly for type II collagen and aggrecan. Labelling was also detected around bony spurs that extended from the enthesis into the lysis-zone. The entheses also labelled for types I, III and VI collagens, chondroitin four and six sulfate, …

The treatment of osteochondral fractures and OCD lesions in the knee is controversial. Many new procedures and techniques have been develope … tion. Downregulation of SOX9 expression in developing entheses adjacent to intramembranous bone.

Entheses are classified into three types: fibrocartilaginous, fibrous, and periosteal insertions. However, the mechanism behind the development of fibrous entheses and periosteal insertions remains unclear. Since both entheses are part of the temporomandibular joint (TMJ), this study analyzes the TMJ entheses. Here, we show that SOX9 expression is negatively regulated during TMJ enthesis development, unlike fibrocartilage entheses which are modularly formed by SCX and SOX9 positive progenitors. The TMJ entheses was adjacent to the intramembranous bone rather than cartilage. SOX9 expression was…
