A Rare Case of Giant Cell Hepatitis Superimposed on Sickle Cell Hepatopathy.

Giant cell hepatitis (GCH) is a rare histopathologic pattern of liver injury characterized by multinucleated syncytial hepatocytes within inflamed parenchyma and is a pattern most commonly seen in infants rather than adults. We report a case of levofloxacin-induced GCH superimposed on sickle cell hepatopathy in a 20-year-old woman with sickle cell disease. She presented with recurrent transaminitis and severe hyperbilirubinemia shortly after initiation of levofloxacin. Liver biopsy revealed sinusoidal congestion and fibrosis consistent with sickle cell hepatopathy, along with portal inflammati

Incomplete cytokinesis and re-fusion of small mononucleated Hodgkin cells lead to giant multinucleated Reed-Sternberg cells.

Multinucleated Reed-Sternberg (RS) cells are pathognomonic for classical Hodgkin lymphoma (HL), and their presence is essential for diagnosis. How these giant tumor cells develop is controversial, however. It has been postulated that RS cells arise from mononucleated Hodgkin cells via endomitosis. Conversely, continuous single-cell tracking of HL cell lines by long-term time-lapse microscopy has identified cell fusion as the main route of RS cell formation. In contrast to growth-induced formation of giant Hodgkin cells, fusion of small mononuclear cells followed by a size increase gives rise t

Human dendritic reticulum cells of lymphoid follicles: their antigenic profile and their identification as multinucleated giant cells.

The B-dependent areas of human lymphoid tissue contain non-lymphoid, non-phagocytic cells known as dendritic reticulum cells (DRC). These ce … ns cells. DRC in touch imprints and suspensions prepared from hyperplastic tonsils were found to be giant cells often with 10 or more nuclei. In certain cases of follicular hyperplasia and of centroblastic-centrocytic lymphoma, DRC with several nuclei were also detectable in situ. These results show that (1) the phenotype of DRC differs from that of all other …

Recurrence Screening of Undifferentiated Pleomorphic Sarcoma with Giant Cells of the Breast.

BACKGROUND: Undifferentiated pleomorphic sarcoma (UPS) of the breast is a rare mesenchymal tumor that is not commonly encountered in clinical settings. Given its high recurrence rate, close monitoring, and follow-up are strongly recommended. METHODS: In this case, the recurrence of UPS with giant cells of the breast was screened through ultrasound imaging and tumor marker tests, including CA 15-3 and CA 72-4. RESULTS: Following recurrence, the patient achieved partial clinical remission after six cycles of epirubicin-based adjuvant chemotherapy. In accordance with the patient's preference, a…

Mast Cells Are Activated in the Giant Earlobe Keloids: A Case Series.

Mast cells and inflammatory cells are abundant in keloid and hypertrophic scar tissues. Even if the cause of physical injury is similar, such as piercing or scratching with hands, clinical findings show differences in the size of keloids in the same area. Hence, we performed histological studies on giant keloids larger than the earlobe, and other smaller keloids. We also examined the risk factors associated with the formation of giant lesions. No statistically significant differences in the association of the risk factors were observed. However, histological observations clearly showed a high

Omental leiomyosarcoma with unusual giant cells in a Beagle dog - Short communication.

A 10-year-old castrated male Beagle dog was presented with a 2-month history of intermittent vomiting and abdominal pain. The dog was referred to the Veterinary Teaching Hospital at Iwate University for further evaluation, and a splenic tumour was suspected on the basis of ultrasonography and computed tomography. Surgery identified a large, solid, light-pink mass on the greater omentum with blood-coloured ascites in the abdominal cavity, and resection was performed. Microscopically, the mass comprised spindle-shaped tumour cells and scattered osteoclast-like giant cells. Most spindle-shaped ce…

Differential expression of filamin B splice variants in giant cell tumor cells.

Giant cell tumor of bone (GCT) is the most commonly reported non-malignant bone tumor in Hong Kong. This kind of tumor usually affects people aged 20-40 years. Also, it is well known for recurrence locally, especially when the tumor cannot be removed completely. Filamins are actin-binding proteins which contain three family members, filamin A, B and C. They are the products of three different genes, FLNA, FLNB and FLNC, which can generate various transcript variants in different cell types. In this study, we focused on the effects of FLNBv2 and FLNBv4 toward GCT cells. The only difference betw

M2 polarized macrophages and giant cells contribute to myofibrosis in neuromuscular sarcoidosis.

The etiopathogenesis of sarcoidosis, a systemic granulomatous disease, still remains obscure. A multitude of organs have been described to be affected in systemic sarcoidosis. Skeletal muscles may also be affected, leading to myalgia and weakness. A workup of the specific immune response with emphasis on the macrophage response is provided herein. Affected muscle tissue from seven patients with systemic sarcoidosis was analyzed and compared with that from seven patients with other myopathies containing macrophagocytic infiltration. Monocytes/macrophages and giant cells in granulomas of muscle …

Long bones giant cells tumors: treatment by curretage and cavity filling cementation.

OBJECTIVE: Giant cell tumors (GCT) of bone are benign tumors with local aggressiveness that most of the time occur around the metaphyseal area of long bones, often in contact with the articular cartilage. Their treatment remains controversial because of their high recurrence rate. The authors report a retrospective series of 30 cases treated using curettage followed by cementation. They suggest demonstrating the mechanical and functional benefit of this technique, its benefit controlling the risk of recurrence, and of osteoarthritis potential. MATERIAL AND METHODS: Between 1992 and 2005, 30 p

Floret-like multinucleated giant cells in a neurofibromatosis type 1-associated neurofibroma.

I report herein a case of floret-like multinucleated giant cells (FMGCs) in a neurofibroma (NF) from a patient with neurofibromatosis type 1 (NF1). CD34-positive FMGCs have been described in 2 prior cases of cutaneous NF and in gynecomastia from patients with NF1. In the case of NFs with FMGCs, these CD34-positive cells may represent endoneurial dendritic cells distinct from both fibroblasts and Schwann cells and do not represent degenerative or malignant change. However, the exact origin of these cells is debatable. I encourage others to observe for the presence of CD34-positive FMGCs in NFs …

Giant cells in cortical tubers in tuberous sclerosis showing synaptophysin-immunoreactive halos.

We describe a characteristic pattern of immunoreactivity for synaptophysin in tuberous sclerosis. We analyzed cortical tubers from surgical specimens taken from six patients with tuberous sclerosis, which were obtained by surgical resections for the treatment of intractable seizures. The cortical tubers were characterized by blurred lamination of the cerebral cortex, hypercellularity, and gliotic changes. Neuropil in the cortex of cortical tubers showed reduced immunoreactivity for synaptophysin in all patients. 'Giant cells' were investigated in the cortex and white matter regions of cortical…

Intracranial mixed germ cell tumor with syncytiotrophoblastic giant cells and precocious puberty.

A 9-year-old male patient developed a germ cell tumor in the right basal ganglia which secreted beta-human chorionic gonadotropin (beta-HCG) and caused precocious puberty. Histology and immunohistochemical staining for placental alkaline phosphatase (PLAP), alpha-fetoprotein (alpha-FP), and beta-HCG showed a mixed population of neoplastic germinocytes, embryonal carcinoma, and syncytiotrophoblastic giant cells (STGC). Immunohistochemical double-staining for alpha-FP and beta-HCG revealed that these two markers were produced by different subsets of cells. Expression of the proliferation marker …

Multinucleated giant cells in acquired immunodeficiency syndrome encephalopathy. Origin from endogenous microglia?

Multinucleated giant cells that are characteristic of the encephalomyelopathy of acquired immunodeficiency syndrome are stained by a lectin histochemical method. The particular lectin used, Ricinus communis agglutinin I, has been shown to bind to endogenous microglial cells in human brain, as well as to endothelial cells of blood vessels. These results suggest that multinucleated giant cells in the brain of patients with acquired immunodeficiency syndrome may be derived from endogenous microglia.

Mammary carcinoma with osteoclast-like giant cells. Additional observations on six cases.

Six cases of mammary carcinoma with osteoclast-like giant cells are reported. In addition to the multinucleated giant cells and the prominent stromal angiogenesis reported in earlier reports, other characteristic features of the tumors were observed: (1) the gross appearance, characterized by a well-outlined, lump-shaped, dark brown tumor mass; (2) the round tumor shadow on the mammograms, originally interpreted as a cyst, a fibroadenoma, or a medullary carcinoma; and (3) the "adenocystic" pattern of the invasive tumor growth. The light and electron microscopic observations point to the intima…

Carboxylic activity of giant cells of human granuloma produced by Paracoccidioides brasiliensis.

The carboxilic activity of giant cells of human granuloma produced by P. brasiliensis was studied. The enzymatic activity was revealed by reddish-brown, purple red, and indigo-blue cytoplasmic precipitate, using the substrates alpha-naphthyl-acetate, naphthol-AS acetate and 5-bromo-4-chloro-indoxyl acetate respectively. The giant cells were intensely positive in all cases studied. We believe this esterasic activity is related to the lytic, lisosomic activity of the macrophages and giant cells in response to the activity by the P. brasiliensis in tissue.

[Bilateral giant cell central granuloma of the jaws in a Noonan syndrome: About one case with emphasizing on bone giant cell lesions of the jaws].

We report the case of a 10-year-old child with bilateral mandibular localization of a central giant cell granuloma occurring in the setting of Noonan syndrome. The histological appearance was classic with two intermigled components, one fibrous with non-atypical mononuclear cells, the other consisting of numerous osteoclast-like giant cells. This aspect is similar to that observed in the brown tumor as well as that of cherubism, which can also give multiple bone lesions. We will discuss the other lesions to consider in case of benign giant cell bone lesions affecting the jawbones, sometimes mu…
