When Is a Positive Test for Pediatric Growth Hormone Deficiency a True-Positive Test?

BACKGROUND: In most cases, the growth hormone stimulation test is a necessary component for the diagnosis of growth hormone deficiency (GHD) in children. Diagnostic testing can lead to unnecessary treatment of children with false-positive test results and omission of treatment in children with false-negative results. False-positive results are suggested by the absence of typical growth responses in treated children and false-negative results are suggested by continued growth failure in those left untreated. SUMMARY: The probability that a positive test result indicates the presence of the con

Growth Hormone Treatment for Patients with Noonan Syndrome.

Noonan syndrome (NS) is a genetic disorder, which can present clinically with a variable phenotype. Proportional post natal short stature is a common manifestation of NS, with the majority of affected patients having an adult height below the third percentile. Some investigators have reported minor abnormalities in GH secretion and/or action, suggesting that recombinant growth hormone (rhGH) therapy may be useful for the treatment of their short stature. Our review of the literature regarding rhGH therapy in children with NS indicates that this therapy improves height velocity, but relatively

Irradiation-induced growth failure.

GH deficiency, skeletal disproportion and early or precocious puberty may complicate irradiation to the head or axial skeleton in childhood. Certain cohorts of children are at particular risk, including those irradiated for brain tumours and various haematological malignancies. Both GH deficiency and impaired spinal growth may result in short stature, whereas the occurrence of early puberty in association with GH deficiency reduces the time available for GH therapy. The age of the child at irradiation is critical in that, in younger children, the central nervous system is more radiosensitive,

Exploring the mind-body connection from puberty to the interSEXtion of eating disorders and reproductive health: What mental health providers should know.

Eating disorders have potential to significantly impact growth and sexual development, particularly when associated with malnutrition. The hypothalamic-pituitary-gonadal axis, which dictates puberty and sexual maturation, including bone growth, is sensitive to metabolic changes such as those in eating disorders. Consequences may include pubertal delay/arrest, stunted growth, weakened bones, menstrual changes, impotence, sexual dysfunction, infertility, or adverse pregnancy outcomes. The physical and psychological impacts of eating disorders can also affect intimate relationships and sexual sat

Growth Hormone, Not Simply Just a Hormone for Growth.

Since its discovery almost 100 years ago, growth hormone (GH) has been extensively studied to elucidate its structural characteristics, receptor interactions and its physiologic and non-physiologic effects. These actions include but are not limited to its effects on somatic growth, substrate metabolism, body composition, bone mineral density, cardiovascular system, and cognitive function. Contextually, recombinant human GH was approved for growth promotion in children and to enhance metabolic health in adult patients with GH deficiency (GHD), along with other clinical indications. Studies invo

Growth failure and nutrition considerations in chronic childhood wasting diseases.

Growth failure is a common problem in many children with chronic diseases. This article is an overview of the most common causes of growth failure/growth retardation that affect children with a number of chronic diseases. We also briefly review the nutrition considerations and treatment goals. Growth failure is multifactorial in children with chronic conditions, including patients with cystic fibrosis, chronic kidney disease, chronic liver disease, congenital heart disease, human immunodeficiency virus, inflammatory bowel disease, short bowel syndrome, and muscular dystrophies. Important contr

Statural growth of children with renal disease.

Growth retardation occurs in 35 to 65% of children with kidney disease. It is especially common in children with congenital diseases of the kidney, anomalies, and inherited disorders. Acquired disease, however, also may impair growth, particularly where renal function (GFR) is below 25 ml/min/1.73 m2. Therapy used in renal disease, notably prednisone, also impairs growth. Chronic dialysis therapy, both hemodialysis and peritoneal, are associated with poor growth. Several specific changes in renal disease are associated with growth failure. These include, in addition to azotemia, acidosis, hypo

The genetics of human hair growth.

The regulation of human hair growth is one of the most mysterious of biologic events. Genetic factors are responsible for the density, length, color and texture of hair. Heredity disorders can affect either hair alone, hair and other ectodermal structures, or can be pansystemic. This paper considers all heredity disorders which affect human hair growth. These disorders are divided into three categories: 1) the dysmorphic syndromes; 2) the modifier gene abnormalities and 3) the single major gene abnormalities. The disorders in these categories are listed, along with the mode of inheritance, cau

Developmental syndromes: growth hormone deficiency and treatment.

Developmental syndromes are characterized by numerous phenotypical signs and malformations. In most of them such as Turner, Noonan, Prader-Willi, Silver-Russel, Williams, Kabuki, Leri-Weill syndrome and skeletal dysplasias, short stature is a common feature. Growth defect is very often related to a defect in cellular growth, but some unknown abnormality in GH action is possible. Recently, the greater availability of recombinant GH has expanded the interest towards GH secretion and therapy also in developmental syndromes. We recognize syndromes associated with GH deficiency (GHD), showing a dev

Safety and efficacy of growth hormone therapy in childhood.

Growth hormone (GH) has been used for more than 40 years. GH improves height velocity in many conditions associated with impaired growth and corrects metabolic deficits attributable to GH deficiency (GHD). Many studies and surveillance programs exist to collect efficacy and safety data. GH has been demonstrated to have a relatively wide safety margin. Reported side effects, including pseudotumor cerebri, edema, slipped capital femoral epiphysis (SCFE), worsening of scoliosis, gynecomastia, and hyperglycemia require careful monitoring. Currently, there are no data suggesting that GH therapy inc

Chronic renal disease and growth in childhood.

Growth failure, sometimes of a severe nature, has been recognized for many years as a consequence of chronic renal insufficiency (CRI) in childhood. The factors associated with growth failure, including renal osteodystrophy, anorexia and malnutrition, acidosis, salt wasting, and anemia, have also been recognized for many years. Despite vigorous treatment of these factors, patients with CRI continue to grow poorly. It was hoped that, with modern dialysis and transplantation, these patients would have normal growth or even catch-up growth and therefore overcome the height deficit that accrued du

Recombinant GH Therapy: Expected Results, Modalities and Monitoring in Children.

The past century witnessed the evolution of growth hormone (GH) treatment from pituitary-derived GH, available for treatment of selected cases of GH deficiency (GHD), to recombinant human GH (rhGH), currently approved for several clinical disorders besides GHD. Treatment with rhGH has a proven efficacy in improving short stature in patients affected by GHD, alongside beneficial effects on metabolic and bone health. Long-term safety of rhGH treatment has been intensively investigated, providing reassuring results, especially in patients treated for isolated GHD. The use of rhGH in supraphysiolo

[Discussion on the genetic mechanisms and efficacy of growth hormone therapy in children with idiopathic short stature].

Idiopathic short stature (ISS) is a term that encompasses a group of short stature disorders with unknown etiology. The genetic factors associated with ISS are complex, and the known genetic mechanisms include alterations in hormones, hormone receptors, or related pathways, defects in fundamental cellular processes (such as intracellular signaling pathways and transcriptional regulation), issues with extracellular matrix or paracrine signaling, as well as genetic variations in the genes encoding these proteins. Recombinant human growth hormone (rhGH) therapy is currently an effective clinical
