Video laryngoscopy as a teaching tool in direct intubation in undergraduate medical education - a randomized controlled trial.

BACKGROUND: Direct laryngoscopy (DL) is a crucial skill and continues to be indispensable. While the superiority of video laryngoscopy (VL) has been demonstrated regarding the rate of first attempt successful intubations, its application as a teaching tool for DL remains largely unexplored. We hypothesised a higher first-pass success and shorter durations of conventional intubation after video laryngoscopy supported training (VLF = video-laryngoscopy based feedback) compared to students trained without VL (DLF = direct-laryngoscopy based feedback). METHODS: All 211 medical students (DLF: 92/ V

High-fidelity trainer for fetoscopic spina bifida repair.

BACKGROUND: Fetoscopic spina bifida repair requires a well-trained, dedicated team to perform the surgery. However, the paucity of cases and limited training resources limit the approach's widespread adoption. Therefore, high-fidelity simulators are essential to fill these gaps. OBJECTIVE: We sought to create a synthetic, low-cost, high-fidelity simulator for appropriate training in fetoscopic spina bifida repair. STUDY DESIGN: We designed a disposable, single-use, multi-layer silicone pad resembling a myelomeningocele attached to a 3D-printed fetal mannequin mounted on a positioning base. T

Acute Upper Gastrointestinal Bleeding: A Hands-On Simulation Case for Internal Medicine Residents Improves Knowledge and Confidence.

INTRODUCTION: Upper gastrointestinal (GI) bleeding leads to approximately 350,000 hospital admissions annually. Simulation-based training enhances medical education by improving quality care, patient safety, and clinical competency. To increase internal medicine (IM) residents' exposure to critical GI concepts, we developed a GI bleed simulation curriculum. METHODS: A total of 129 IM residents participated in a hands-on simulation using a high-fidelity manikin. Pre- and postsimulation surveys assessed demographics, confidence, and knowledge. The case involved a 45-year-old male with alcohol u

Pediatric occupant-to-occupant interaction responses in side impact conditions using Q-dummy and child human body models.

OBJECTIVE: Road safety of children has improved considerably over the past decade; however, there is still much scope to better protect these occupants. Ensuring adequate protection of children seated in the rear seats requires a greater understanding of how children are injured in side crashes. The purpose of this study was to enhance the knowledge of children's potential injuries in side impact collisions by the investigation of two different child sizes seated in different positions in the rear seats. The injury metrics associated with the occupant-to-occupant or occupant-to-child restraint

Large-scale individual monitoring of internal contamination by gamma-emitting radionuclides in nuclear accident scenarios.

The results obtained in a measurement campaign concerning internal contamination by the gamma-emitting radionuclides of a large number of individuals are presented in this work. The aim is to assess the effectiveness of the spectrometric method in an emergency response following a nuclear power plant accident or a spread of radionuclides in the atmosphere due to an act of terrorism. An HPGe portable spectrometer, deployed in a collective protection apparatus, was used for both whole-body and thyroid measurements. An adult bottle mannequin absorption (BOMAB) and thyroid phantoms were used to ev

Comparison of the McGrath® MAC video laryngoscope with direct Macintosh laryngoscopy for novice laryngoscopists in children without difficult intubation: A randomised controlled trial.

INTRODUCTION: Airway management and tracheal intubation are routinely performed in paediatric anaesthesia and yet are associated with preventable adverse events. The McGrath® MAC is a video laryngoscope with a curved blade similar to the Macintosh blade, which has not been previously described in first intention paediatric tracheal intubation. We aimed to compare direct laryngoscopy to the McGrath® MAC video laryngoscope among novice laryngoscopists. METHODS: Six paediatric anaesthesia residents with novice skills in paediatric laryngoscopy were randomly assigned to perform tracheal intubatio

A multi-institutional study using simulation to teach cardiopulmonary physical examination and diagnosis skills to physician assistant students.

PURPOSE: Research demonstrates limitations in the ability of health care trainees/practitioners, including physician assistants (PAs), to identify important cardiopulmonary examination findings and diagnose corresponding conditions. Studies also show that simulation-based training leads to improved performance and that these skills can transfer to real patients. This study evaluated the effectiveness of a newly developed curriculum incorporating simulation with deliberate practice for teaching cardiopulmonary physical examination/bedside diagnosis skills in the PA population. METHODS: This mu

Comparing mechanical fetal response during descent, crowning, and restitution among deliveries with and without shoulder dystocia.

OBJECTIVE: Previous computer simulations of shoulder dystocia (SD) explored the effect of SD itself on the mechanical response of the fetus. Our objective was to perform a mechanical simulation study to explore the variations in fetal response during routine, unilateral SD (USD), and bilateral SD (BSD) deliveries. STUDY DESIGN: Using a biofidelic birthing simulator, we performed 30 experiments mimicking passage of the fetus through the pelvis. For routine deliveries, we engaged the fetal head and allowed it to progress through cardinal movements using typical uterine contraction forces. Deliv

Formal procedural skills training using a fresh frozen cadaver model: a pilot study.

Graduating medical students are expected to be proficient in a number of procedural skills. A structured curriculum is infrequently available. In addition, the use of a mannequin tends to be unrealistic and students continue to have some anxiety when performing procedures on patients. This pilot study was designed to demonstrate the utility of a fresh frozen cadaver model in practicing procedural skills as compared to mannequins. Seven third-year students carried out a number of basic procedural skills using a mannequin model and a fresh frozen cadaver. We surveyed the students after they had

Feature preserving refinement of surfaces for web-based surgical simulation.

In plastic surgery, 3D models of the affected part are often used for the purpose of visualizations and surgical simulations. The optimal models for web-based surgical simulations keep high accuracy in affected parts and keep low accuracy in other parts. Consequently, the data size becomes small. In this research, we propose a method to generate free-form surfaces based on Lattice Structure from polygonal meshes. The polygonal meshes are generated automatically from CT and MRI data using Marching Cubes. By changing the resolution of input images, the accuracy of output meshes is controlled. Fr

Optimization of glottic exposure during intubation of a patient lying supine on the ground.

Two methods of endotracheal intubation of patients lying on the ground were compared for ease and speed of intubation and minimization of complications in a crossover study of prehospital-oriented emergency physicians. Intubation of a mannequin was attempted by the physicians in either a left lateral decubitus (LLD) position or a kneeling (K) position, followed by the alternate position. The LLD position afforded more rapid intubation, better glottic visualization, and less dental trauma. Eighty-seven percent of physicians completely visualized the glottis in the LLD position, versus 33% of th

Psyche and pixels: AI-generated images and their impact on stigma of mental illnesses in Germany.

Artificial intelligence (AI) has rapidly evolved in recent years. Image generating chatbots enjoy great popularity. But they bear the risk of maintaining or enhancing preexisting stereotypes and stigma against severe mental illnesses. The objective of the study is to investigate how AI-generated imagery on psychiatric diseases and institutions are perceived compared to (non-psychiatric) medical ones. Using the chatbots Designer, DALL-E 3 and Midjourney V6 we created images associated with psychiatric and medical contexts (disease, institution, incident). Participants (N = 239, 75.31% women) ra

Can I Phone a Friend? Exploring the Use of Digital Devices in Clinical Exams.

BACKGROUND: Objective structured clinical examinations (OSCEs) are used globally to assess health professional learners' clinical skills and applied knowledge. Despite innovations with simulated participants, manikin technology and real patient involvement, there remains a gap between 'real-life' practice and 'OSCE experience'. For example, although mobile phone use is increasingly common in clinical practice; however, it would represent a significant disruption to established assessment practices in OSCEs. We aimed to explore student use of mobile phones during OSCE assessment, with a focus o
