The role of mesenchyme-like tissue in the pathogenesis of thanatophoric dysplasia.

We have studied the light microscopic, transmission, and scanning electron microscopic (SEM) findings in 13 cases of thanathophoric dysplasia (TD) and 4 control infants. In the TD growth plate, areas with less abnormal cartilage and bone alternated with areas of severely abnormal cartilage and bone. These latter abnormal areas were always found around tongues of apparent mesenchymal tissue that appeared to penetrate from the investing perichondrium and periosteum. The ultrastructure of the less abnormal areas was similar to that of the control infants, including cell and matrix structure as we…

Clinical Outcomes of Classical, V-Y, and Z-plasty Frenectomies: A Triad of Techniques.

The frenum is a fold of mucous membrane that connects the gingiva, underlying periosteum, and alveolar mucosa to the lip and cheek. It can negatively impact periodontal health by impeding plaque control or causing muscle pull if it is placed too near the gingival margin. An aberrant frenum in the maxillary region can also cause aesthetic issues or impede the results of orthodontic treatment, especially when midline diastema is present. This frequently leads to relapse following treatment. Frenectomy is the method used to manage such abnormal frenal attachments. This case series demonstrates va…

Cephalohematoma Infected With Staphylococcus epidermidis and Skull Abscess: A Case Report.

Cephalohematoma is a self-limiting lesion in 1%-2% of neonates. Cephalohematoma bleeds between the bone and periosteum and is limited to cranial sutures. Complications of the cephalohematoma include hyperbilirubinemia and secondary infection. We describe a neonate with cephalohematoma cultures positive for S. epidermidis as well as progressive deepening of the skull. There was no evidence of systemic infection, such as sepsis, meningitis, or osteomyelitis. In the ultrasound and brain MRI, the abscess was restricted to the scalp without involvement of the brain parenchyma and osteomyelitis of t…

Conventional Frenectomy With Topical Ozonated Oil Application.

The frenum is a mucous membrane fold that connects the lip and cheek to the gingiva, periosteum, and alveolar mucosa. When the frena are linked too closely to the gingival border, there may be issues with plaque removal, and the overall gingiva may be affected. In addition, the maxillary frenum may provide aesthetic difficulties or interfere with the aesthetic outcome in cases of midline diastema, which may result in a recurrence after treatment. A labial frenectomy, a frequently performed surgical operation in the specialty of dentistry, is used to address such an abnormal frenum. This articl…

Clavipectoral plane block as a sole anesthetic technique for clavicle surgery - A case report.

BACKGROUND: The clavipectoral fascial plane block was introduced by Dr. Luis Valdes in a symposium at the 2017 European Society of Regional Anesthesia and Pain Therapy Congress. CASE: Clavipectoral plane block (CPB) with intravenous sedation provided surgical anesthesia and analgesia in a 39-year-old male patient with a right midshaft clavicle fracture. This in-plane technique was used to deposit 30 ml of a local anesthesia mixture between the clavipectoral fascia and periosteum on both the medial and lateral sides of the fracture line. CONCLUSIONS: Excellent anesthesia and analgesia for up …

Digital design and 3D printing of an implant-supported prosthetic stent for protecting complete arch soft tissue grafts around dental implants: A dental technique.

A method for digital fabrication of an implant-supported soft tissue graft stent to protect, shape, and ensure intimate adaptation of the complete arch graft to the periosteum surrounding dental implants is described. To fabricate the stent, an extraoral scanner was used to convert the implant cast into digital data. Dental design software was then used to fabricate the stent, which is produced by 3-dimensional (3D) printing. Due to the lack of long-term biocompatible 3D printing material, the stent was duplicated in bisacryl resin. The patient reported more comfort and stability with the impl…

Dehiscences and fenestrations: methodological care necessary to avoid errors in diagnosis and measurement.

The low prevalence of gingival recessions observed in orthodontic clinical practice may be assigned to the fact that in studies in which dehiscences and bone fenestrations are described as frequent, they were diagnosed based on: 1) dry skull studies; 2) areas with periosteal reflection together with flap; and 3) imaging techniques with low sensitivity to detect these defects, which have a delicate structure and function. In areas of pseudo-dehiscences and fenestrations, the periosteum and the alveolar cortical bone are very thin; also, they either have been removed during preparation of the dr…

MACRODYSTROPHIA LIPOMATOSA WITH ULNAR DISTRIBUTION IN HAND: MR EVALUATION OF A RARE DISORDER.

Macrodystrophia lipomatosa is a rare, non-hereditary congenital anomaly characterized by localised gigantism in the form of macrodactyly affecting a part of limb and rarely entire limb due to overgrowth of all mesenchymal elements. Radiological and pathological hallmark is the disproportionate fibroadipose tissue proliferation in subcutaneous tissue, nerve sheaths, and periosteum that lead to soft tissue and bony enlargement. We present the case of a twenty years old female who presented with history of gradual enlargement of the medial aspect of right hand along ulnar nerve distribution since…

Minimal invasive transcaruncular optic canal decompression for traumatic optic neuropathy.

Traumatic optic neuropathy is a cause of loss of vision associated with head injuries. Treatment options include observation, steroids and decompression of the optic canal. We report a case where the optic canal decompression was performed using a transcaruncular approach under a regional block. The incision was made through the caruncle and the dissection was carried down to the periosteum down to the orbital apex where the optic nerve was seen exiting through the optic canal posterior to the posterior ethmoidal artery. The optic nerve was decompressed with good visualization. Hemostasis and …

PERIOSTIN: role in formation and maintenance of dental tissues.

The matricellular protein periostin is strongly expressed in collagen-rich connective tissues such as periodontal ligaments (PDLs), skeletal muscle, adipose tissue, tendons, skin, and bone. It is prominent in tumorigenesis, angiogenesis, and cardiac repair. It is localized in the periosteum and PDL, where it is seen in the cytoplasmic extensions of the PDL fibroblasts. It plays a key role in morphogenesis, postnatal development, and maintenance of the tooth, and related structures. It mediates and augments collagen fibrillogenesis, cell migration, adhesion, response to mechanical stress, and w…

Immediate coverage of exposed, denuded cranial bone with split-thickness skin grafts.

Avulsion injuries of the scalp are challenging to treat. If the periosteum is not intact and the status of local tissues does not allow local flap transposition, coverage of the exposed, denuded skull becomes a problem. Most authors have obtained a bed of granulation tissue at the site of exposed bone before skin grafting, which requires a prolonged waiting period. In 5 consecutive patients, the authors obtained coverage in a single operation by removing the outer table of the skull with a high-speed burr, followed by immediate application of a skin graft over the bleeding, cancellous bone. Th…

The role of the tibial nutrient artery. Microsphere estimation of blood flow in the osteotomised canine tibia.

There has been a long-standing debate as to whether medullary or periosteal flow is the dominant vascular supply during the healing of diaphyseal fractures. We used radioactive microspheres to quantify blood flow to the canine tibia two weeks after an osteotomy. There was a significant contribution from the periosteum to the blood supply of healing cortical bone after nutrient artery ligation, with a reversal of flow from a centrifugal to a centripetal direction. Our study has confirmed the qualitative observations of Trueta (1974) regarding the significant recruitment of vessels from surround…

The treatment of fractures of the proximal humerus.

Fractures of the proximal humerus, which can not be kept in place after reposition--and therefore not acceptable for conservative treatment--can be sufficiently fixed by a thin and long Rush-pin percutaneously introduced. So the damaged arm can be moved early. No inflammation and no pseudarthrosis was observed in 124 cases. The follow up examination of 98 patients after 6 to 12 months revealed good results. In 4 children the fractured neck of the humerus could not be reposed into a proper position (Interposition of periosteum and joint capsule). After open reduction fixation was performed by K…

'In-Out-In' K-wires sliding in severe tibial deformities of osteogenesis imperfecta: a technical note.

Severe infant osteogenesis imperfecta requires osteosynthesis. Intramedullary tibia's osteosynthesis is a technical challenge given the deformity and the medullar canal's narrowness. We describe an extramedullary technique: 'In-Out-In' K-wires sliding. We performed an anteromedial diaphysis approach. The periosteum was released while preserving its posterior vascular attachments. To obtain a straight leg, we did numerous osteotomies as many times as necessary. K-wires ('In') were introduced into the proximal epiphysis, and the medial malleolus ('Out') bordered the cortical and ('In') reach the…

Long-term results of Galeazzi-equivalent injuries in adolescents--open reduction and internal fixation of the ulna.

We diagnosed 10 Galeazzi-equivalent injuries. We report the radiological and clinical results at the end of growth in adolescents, including the results of ulnar lengthening. This study included 10 Galeazzi-equivalent injuries (seven patients requiring open reduction and three requiring closed reduction for ulnar lesions) seen since 2004. The periosteum was entrapped around the fractured physis in five patients and the extensor carpi ulnaris to the periosteum was interposed in two patients. Among the seven patients, ulnar lengthening was performed in only three patients. Ulnar variances at the…

Cadaveric analysis of periosteal temperature response across skin types in facial tissues using a multi-wavelength diode laser.

Energy-based devices are often applied to the midface using uniform treatment parameters despite interindividual variability in facial anatomy and skin pigmentation. For bulk-heating technologies, clinical relevance depends not only on superficial heating but also on the timing and magnitude of temperature rise at deeper tissue planes, as periosteal thermal exposure in the malar cheek may influence procedural safety and discomfort. This study aimed to quantify time to periosteal temperature rise in the malar cheek and to compare this parameter between Fitzpatrick skin types I-II and III-IV. Th

Vascularized Periosteal Grafts for Bone Union in Children: A Systematic Review.

BACKGROUND: The periosteum is the main organ responsible for bone regeneration. Vascularized Periosteal Grafts (VPG) have demonstrated exceptional efficacy and speed in facilitating bone union among children with challenging bone healing conditions. Despite their promising results, the overall impact of these interventions has yet to be comprehensively evaluated through systematic review. This systematic review aimed to provide comprehensive insights into bone union outcomes and complications related to the use of VPG in children. MATERIALS AND METHODS: MEDLINE/PubMed, Embase, Web of Science,…
