Pulmonary hypertension (PH) during pregnancy remains a high-risk clinical condition because normal gestational cardiovascular adaptation may exceed the reserve of the right ventricular (RV)-pulmonary circulation unit. Pregnancy is characterized by progressive increases in plasma volume, heart rate, stroke volume, and cardiac output, together with a reduction in systemic vascular resistance. These changes become particularly hazardous during labor, delivery, and the early postpartum period, when abrupt shifts in preload, venous return, and filling pressures may precipitate RV failure, hypoxemia

Rheumatoid arthritis-associated interstitial lung disease (RA-ILD) is one of the most clinically significant extra-articular manifestations of rheumatoid arthritis and an important cause of morbidity and premature mortality. Its clinical spectrum ranges from asymptomatic radiographic abnormalities to rapidly progressive fibrosing lung disease with respiratory failure. Current evidence indicates that RA-ILD is biologically heterogeneous and arises from the interplay of mucosal autoimmunity, genetic susceptibility, epithelial injury, dysregulated innate and adaptive immune responses, and aberran

Primary glaucoma is a leading global cause of irreversible visual impairment, necessitating management strategies to preserve visual function over a lifetime. This narrative review synthesizes contemporary evidence on risk stratification, target intraocular pressure (IOP), and interventional decision-making for primary open-angle glaucoma (POAG) and the primary angle-closure disease (PACD) spectrum. A risk-stratified approach integrates baseline risk factors, gonioscopic anatomy, structural optical coherence tomography (OCT), standard automated perimetry, rate of progression, life expectancy,

The 2024 hypertension guidelines from the European Society of Cardiology (ESC) introduce the category of 'elevated blood pressure' (120-139/70-89 mm Hg) and emphasise the importance of early prevention in individuals with such blood pressure levels and comorbid conditions. As blood pressure is recognised as a continuous risk factor, early initiation of antihypertensive treatment may be recommended even below the hypertensive range, depending on overall cardiovascular risk. All affected individuals should adopt a healthy lifestyle: regular exercise, balanced diet, reduced alcohol and salt intak

BACKGROUND: In 2020, an estimated 14.9 million herpes zoster (HZ) cases occurred worldwide among adults aged ≥ 50 years; this number is projected to rise to 19.1 million by 2030. Postherpetic neuralgia (PHN), the most common complication of HZ, develops in 5% to > 30% of patients with HZ. Poorly managed PHN may cause persistent pain, functional impairment, psychological distress, and, in severe cases, suicide. This review summarizes the current evidence on PHN and proposes a tiered management framework.
METHODS: PubMed and Web of Science were searched for PHN-related articles published from J

Fetal premature atrial contractions (PACs) are the most common fetal arrhythmia, accounting for up to 90% of cases. Although predominantly benign and self-limiting, PACs carry a small but significant risk of adverse outcomes, including progression to supraventricular tachycardia (SVT), and heart failure. This review synthesizes current evidence on the epidemiology, etiology, and risk stratification of fetal PACs. We highlight the importance of M-mode and Doppler echocardiography in diagnosis and in distinguishing PACs from atrioventricular block (AVB). A risk-stratified management pathway is p

Autoimmune gastritis (AIG) is a chronic, organ-specific autoimmune disease characterized by the immune-mediated destruction of gastric parietal cells, leading to impaired acid secretion, vitamin B12 deficiency, and an increased risk of gastric malignancies. The diagnosis of AIG relies on endoscopic findings combined with serological markers and histopathological confirmation. This review synthesizes current knowledge on the pathophysiology, diagnosis, and management of AIG, with a special focus on familial aggregation, polyglandular autoimmunity, and emerging therapeutic strategies. We discuss

Lumen-apposing metal stents (LAMS) have been used for drainage of peripancreatic collections, although they may be associated with complications, including bleeding. We report a case of a 49-year-old male patient who developed hematemesis and syncope three and a half weeks after LAMS placement for peripancreatic collection drainage. A computed tomography scan revealed splenic artery erosion by the distal flange of the stent. Splenic artery embolization was performed before LAMS removal, and splenectomy was required 3 days later. The patient remains asymptomatic after two years of follow-up. Th

Traumatic brain injury (TBI) is one of the main mechanisms underlying health issues associated with functional and structural brain changes. While direct effects such as cognitive and physical impairments are well documented, recent research has linked TBI to neurodegenerative changes similar to dementia. TBI-related neurodegeneration includes progressive brain-tissue degeneration that leads to behavioral changes, cognitive decline, and dementia-like symptoms. The exact underlying mechanisms are complex, and include neuroinflammation, oxidative stress, excitotoxicity, and disruption to protein

Targeted therapies, consisting of Bruton tyrosine kinase inhibitors (BTKis) or BCL-2 inhibitors, are the mainstay of contemporary treatments for chronic lymphocytic leukemia (CLL). The most common adverse effects (AEs) of BTKis are fatigue, bruising, infection, hematological and cardiovascular AEs. While AEs during treatment are usually mild (grades 1 and 2), grade 3 and 4 AEs have been detected in some patients, necessitating additional medical care and temporary or permanent drug discontinuation. In this review, we analyzed the cardiovascular effects associated with BTKi therapy for CLL and

INTRODUCTION: Older patients with cancer are at increased risk for adverse drug events due to chronic conditions and complex medication regimens often leading to polymedication. A pharmacist-led medication review has the potential to reduce drug-related problems and enhance medication safety. Oncogeriatric scores are effective in predicting chemotherapy-associated toxicity. However, risk scores are only of benefit for the patient if the results lead to therapeutic consequences. This study aimed to develop and evaluate an interprofessional risk-adapted medication management intervention (IrMa)

The increasing emphasis on risk management and quality assessment in hospital management necessitates strict adherence to government and international standards. Those standards are particularly important, and certification requirements further underscore this need. The objective of this paper is to outline the application of the INTegrated Audit Model in a Comprehensive Cancer Center. This model represents a revised version of Tracer Methodology, for audit surveys, aimed at aligning audit activities with UNI EN ISO 9001:2015 and Joint Commission Standards for quality, patient safety and clini…

The Kidney Failure Risk Equation (KFRE) is a predictive tool that estimates the risk of progression to end-stage renal disease (ESKD) in patients with chronic kidney disease (CKD). This study evaluated the systematic implementation of KFRE in our nephrology center to improve the management of pre-dialysis patients. Through the analysis of 100 patients followed in the pre-dialysis pathway in the last 36 months, we observed a significant reduction in the initiation of dialysis in late referral mode, a lower use of temporary central venous catheters and an increase in timely preparation of vascul

Stent thrombosis remains a major complication following percutaneous coronary intervention, with significant morbidity and mortality implications. Despite advancements in drug-eluting stents and optimized pharmacotherapy, real-world registry data indicate that definite or probable stent thrombosis occurs in approximately 0.5% of percutaneous coronary intervention cases, with a 30-day mortality rate approaching 25% and a long-term risk exceeding 30% at 10 years. Stent thrombosis is classified based on timing into acute, subacute, late, and very late thrombosis, with subacute and very late stent

Pulmonary hemorrhage (PH) is a life-threatening complication predominantly affecting preterm infants, particularly those with very low birth weight (VLBW) and fetal growth restriction (FGR). Typically occurring within the first 72 h of life, PH is characterized by acute respiratory deterioration and significant morbidity and mortality. This review synthesizes current evidence on the multifactorial pathogenesis of PH, highlighting the roles of immature pulmonary vasculature, surfactant-induced hemodynamic shifts, and left ventricular diastolic dysfunction. Key risk factors include respiratory d
