Implantation of cardiac rhythm devices without interruption of oral anticoagulation compared with perioperative bridging with low-molecular weight heparin.

BACKGROUND: Increasing numbers of patients requiring arrhythmia device implantation are taking warfarin. The common practice of warfarin interruption and perioperative bridging with heparin is associated with a high rate of postoperative hemorrhagic complications. We assessed the safety of device implantation without interruption of warfarin therapy. METHODS: Three patient groups were studied: Group 1 consisted of 117 consecutive patients on long-term warfarin therapy with significant risk of thromboembolism (atrial fibrillation with CHADS(2) score > or =2, mechanical heart valve, recent veno

Fixation of proximal humeral fractures with the Polarus nail.

Thirty patients with fractures of the proximal humerus were treated by internal fixation with an intramedullary locked nail. Fourteen of these patients were more than 60 years of age; 23 patients had 2-part fractures (surgical neck fractures) and were operated on within an average of 3 days after injury. At follow-up, 20 (80%) of 25 patients showed satisfactory to excellent functional outcomes, as measured by Constant score. All but 1 fracture progressed to clinical and radiologic union. The good functional results indicate that the Polarus nail is a satisfactory mechanical device that can be

Development of a Rat Caudal Interbody Fusion Model: A Pilot Feasibility Study.

Rat caudal vertebrae are widely used in bone regeneration and spinal fusion research as convenient sites for testing bone graft materials and implants. Prior rat tail fusion models have achieved intervertebral fusion through disc removal and bone grafting, or with external fixation devices. Nonetheless, no such small-animal model has been developed that uses an interbody fusion cage analogous to that used clinically. It was hypothesized that an evaluable bone fusion environment could be established without neurological complications by performing a posterior cage insertion mimicking clinical p

Fixation failure and early loss of reduction with the use of suture anchors for surgical repair of acromioclavicular joint dislocation: a case series.

BACKGROUND: Suture anchors have been used in surgical repair of acromioclavicular joint (ACJ) dislocation. While some reports indicate favorable results, others emphasize less promising outcomes. This case series reports our experience with suture anchors for surgical treatment of ACJ dislocation. METHODS: Clinical and radiographic outcomes in three patients treated operatively for ACJ dislocations were reviewed. In all patients, two suture anchors were inserted in the coracoid (unicortical) and #5 nonabsorbable suture from the suture anchor was shuttled through drill holes in the clavicle an

Management of infected fractures of the tibia with associated soft tissue loss: experience with external fixation, bone grafting and soft tissue reconstruction using pedicle muscle flaps or microvascular composite tissue grafts.

The aim of this study was to assess the results of treatment given by a team of orthopaedic and plastic surgeons in a series of infected unhealed fractures of the tibia associated with loss of adjacent soft tissues. Twenty-five lower leg fractures, treated during a 10 year period, entered the study and were grouped according to the principles of treatment followed. In the earlier Group A (nine patients), the osteosynthesis implants were retained or changed to more stable internal fixation devices, the soft tissue defects were closed by conventional muscle or musculocutaneous flaps and bone gra

Factors associated with adverse events after distal tibiofibular syndesmosis fixation.

PURPOSE AND HYPOTHESIS: Factors associated with adverse outcomes following surgery for syndesmotic instability with associated closed fibula fracture are incompletely understood. The purpose of this study was to determine the pathoetiology and incidence of adverse events after stabilization of syndesmotic instability. In addition, we aimed to identify any patient or surgeon related factors that might be associated with unanticipated outcomes. METHODS: Between January 2000 and May 2015, a total of 849 adult patients who were surgically treated with either screw or suture button fixation for sy

Two-stage maxillary distraction osteogenesis using a modified external device: clinical outcome and complications.

Rigid external distraction is currently used to correct severe maxillary hypoplasia. The purpose of this retrospective study was to present the clinical results and complications of a two-stage surgical approach using a modified external distraction system that consists of maxillary distraction and then maxillary fixation. We treated eight patients with cleft lip and palate in this way from 2016 to 2018. Lateral cephalograms taken before the first operation, after distraction, two weeks after the second operation, and one year after treatment were used to examine maxillofacial morphology. Velo

The Role of Intramedullary Nailing for Proximal Humerus Fractures: What Works and What Does Not.

PURPOSE OF REVIEW: The evaluation of proximal humerus fractures (PHFs) should be aimed to answer the following four questions: (1) does the fracture need surgery in each particular patient? (2) if surgery is recommended, is it better to proceed with internal fixation or shoulder arthroplasty, (3) if internal fixation is recommended, what is the ideal fixation device strategy, and (4) how can outcomes be optimized? This review article tries to answer these questions and provides some clarity regarding what works and what does not in PHFs treated with intramedullary nailing. RECENT FINDINGS: Ac

The timing of spinal stabilization in polytrauma and in patients with spinal cord injury.

PURPOSE OF REVIEW: Despite improved care of multiple injured patients, controversial data about the best time point for decompression and stabilization of spine fractures and spinal cord injuries are published. Some studies even question the value of surgical management. RECENT FINDINGS: In several mostly retrospective studies and a meta-analysis, there is growing evidence that early decompression and stabilization of spine injuries seems to be beneficial, especially in more severely injured patients. Early stabilization seems to reduce length of stay and pulmonary complications. Some studies

Correction of syndesmotic malreduction following fixation flexibilization.

Prior studies showed the potential of a rigid fixation, such as a trans-syndesmotic screw, to produce tibiofibular malreduction. Flexible implants, although capable of allowing forgiveness, may not provide sufficient stability for all injury patterns. To assess the malreduction forgiveness of a two-phase syndesmotic device that is designed to transition from a rigid screw construct to a flexible suture-type. Cadaveric specimens were loaded in a frame under four conditions: native (control), syndesmotic instability (injured), malreduced with engaged syndesmotic device (malreduced), and post-dis

The Pelvic INFIX Technique for Unstable Anterior Pelvic Ring Fractures: Clinical Outcomes, Radiographic Results, and Complications.

Background: Unstable pelvic ring injuries often require surgical stabilization to restore pelvic ring integrity. The anterior subcutaneous internal fixator, or pelvic INFIX, has emerged as an alternative to external fixation and open anterior fixation. This study evaluated the functional, radiographic, and complication-related outcomes of INFIX fixation for unstable anterior pelvic ring injuries. Methods: We retrospectively reviewed 21 adult patients treated with anterior pelvic INFIX for unstable anterior pelvic ring fractures, with or without posterior fixation, at a Level 1 Trauma Center be

Transmaxillary osteogenic distraction with intraoral tooth-borne distractors.

Midfacial retrusion and class III malocclusion in maxillary hypoplasia are frequent sequelae in patients with cleft lip and palate. Similar deformities are seen in craniofacial dysostosis surgically treated in infancy. Recurrences after orthognathic surgery are unpredictable even using rigid fixations. Distraction osteogenesis, using Le Fort I osteotomies with external distractors, is uncomfortable for the patient, and internal distractors require a second procedure for removing the retractor. A new model of distraction osteogenesis is presented. Transmaxillary osteogenic distraction, using to

Current therapy: complications associated with rigid internal fixation of facial fractures.

Rigid internal fixation (RIF) is commonly used for the anatomical reduction and fixation of facial fractures. This technique has many advantages over more traditional methods, such as wire osteosynthesis and maxillomandibular fixation. Properly placed RIF ensures a stable anatomical reduction and allows for immediate or early restoration of function. RIF has a number of complications associated with its use in facial trauma surgery including metal sensitivity, infection, neurologic injury, dental trauma, stress shielding, and malocclusion. Complication rates appear to be inversely proportional

A new way to anchor the external device in mandibular distraction: three case reports with a Pierre Robin sequence.

Pierre Robin sequence is a pathology derived from alteration in the first and second branchial arch. Patients have breathing problems due to micrognathia and glossoptosis, causing severe upper airway obstruction. One surgical treatment is distraction osteogenesis. Three patients with Pierre Robin sequence (case 1, 3 months old; cases 2 and 3, 1 month old) with severe upper airway obstruction requiring mechanical ventilator assistance, underwent mandibular distraction osteogenesis prematurely with a new anchoring system, thus avoiding tracheostomy and its consequences. An intraoral approach was

Development of the Surgical Implantation and Fixation of an Implanted Middle Ear Microphone, the "UmboMic," in Cadaveric Sheep.

PURPOSE: The UmboMic is a recently developed middle ear microphone that functions by detecting the sound-induced motion of the umbo [1]. To prepare for a live animal study of the in vivo performance of the UmboMic, we developed the surgical implantation and fixation system in cadaveric sheep ears. METHODS: Temporal bones from seven female sheep were prepared for implantation of the UmboMic system. Fixation hardware for sheep was designed to secure the UmboMic sensor in position so that the sensing tip contacted the umbo in the middle-ear cavity and the non-sensing tail end was secured to the
