Long-term effects of transcatheter closure of atrial septal defect on cardiac remodeling and exercise capacity in patients older than 40 years with a reduction in cardiopulmonary function.

BACKGROUND: Although it has been demonstrated that cardiac remodeling and exercise capacity improve after transcatheter closure of atrial septal defect (ASD), little is known about long-term benefits in middle-aged and elderly patients with a reduction in cardiopulmonary function. OBJECTIVES: To evaluate long-term extent and time course of improvements in cardiac remodeling and exercise capacity in those patients. METHODS: Twenty ASD patients ≥ 40 years of age with a reduction in cardiopulmonary function (predicted peak oxygen uptake [VO(2)] < 65%) were enrolled. Transthoracic echocardiograp

Ultrafast computed tomography in the diagnosis of cardiac disease.

Ultrafast computer tomography (CT) is one of the major new imaging modalities that has become available for the evaluation of patients with heart disease. With cine CT it is possible to obtain high resolution images of the heart at 58-ms intervals. The inplane resolution is 0.7 to 1.5 mm, and the slice thickness is 3 to 9 mm. Images require the intravenous injection of nonionic contrast agents to obtain quantitative information about cardiac function. Cine CT has thus far been used in 3 general areas that relate to the evaluation of patients with heart disease: evaluation of cardiac structure

Early gene expression profiles during intraoperative myocardial ischemia-reperfusion in cardiac surgery.

OBJECTIVE: The effects of cold cardioplegic arrest and reperfusion on human ventricular gene expression are unknown. We tested the hypothesis that intraoperative ischemia-reperfusion under conditions of blood cardioplegic arrest would induce a unique myocardial genomic profile indicative of a cardioprotective response. METHODS: Right ventricular samples were serially acquired during surgical repair of ventricular septal defect. RESULTS: Expression profiling revealed 3 patterns of gene expression: (1) increased expression above control levels within 1 hour of cardioplegic arrest, with further

[The pulmonary artery catheter-Not a niche tool but often a must].

The pulmonary artery catheter (PAC) is the only hemodynamic monitoring tool that continuously integrates systemic blood flow, global oxygen balance, cardiopulmonary interactions and right ventricular function. Meta-analyses show that PAC-guided treatment reduces morbidity and mortality in high-risk surgical patients. Additionally, multiple observational and register data suggest a survival benefit in cardiogenic shock. Despite this evidence the use of PAC has declined in Germany in contrast to the rest of the world, with a concomitant loss of clinical expertise in the use of this monitoring mo

Brugada Syndrome: 30 Years of Scientific Adventure.

Thirty years ago, a distinct new clinical-electrocardiographic syndrome was described, now known as Brugada syndrome (BrS). Typical for this syndrome is EKG with ST elevation in the right precordial leads. The clinical presentation of the disease is highly variable: Patients may remain completely asymptomatic, but may also develop episodes of syncope, atrial fibrillation (AF), sick sinus syndrome (SSS), conduction disturbances, asystole, and ventricular fibrillation (VF). The disease is caused by mutations in the genes responsible for the action potential of the heart cells. The most frequentl

High-Resolution, Contrast-Enhanced Cardiac Magnetic Resonance Detects an Intracardiac Thrombus in a Fontan Patient.

A 28-year-old male with Fontan physiology presented with chest pain, elevated troponin, regional dyskinesis, and an apical echogenicity in the right-sided morphologically left ventricle (LV) on echocardiogram. Cardiac magnetic resonance (CMR) imaging was performed with a three-dimensional respiratory-navigated inversion recovery FLASH (low flip angle, spoiled gradient echo technique) sequence during slow gadolinium infusion. A hypointense lesion in the LV apex was consistent with thrombus. Gadolinium-delayed enhancement imaging demonstrated increased signal in the mid-inferior LV free wall. In

[Unsuccessful anesthetic management for cesarean section in a patient with primary pulmonary hypertension].

A 31-year-old woman with primary pulmonary hypertension presented for an elective cesarean section at the 34-week gestation. After monitoring pulmonary artery, systemic artery blood pressures and an electrocardiogram, continuous lumbar epidural anesthesia was performed. Uneventful delivery was followed by a sudden decrease in systemic pressure and loss of consciousness. Her trachea was intubated and administration of epinephrine was started. Nitroprusside and milrinone were infused to decrease pulmonary artery pressure and to maintain systemic arterial pressure. However, she died after 16 hour

Pulmonary hypertension and cardiomyopathy: anaesthetic management for caesarean section.

Pulmonary hypertension in pregnant women is uncommon but is associated with a high mortality. We present the case of a 14-yr-old parturient with pulmonary hypertension and cardiomyopathy who required a Caesarean section. Management goals included: (1) maintaining right ventricular function, (2) avoiding the haemodynamic effects of general endotracheal anaesthesia, and (3) minimizing narcotic-related neonatal respiratory depression. While most authors agree on invasive pulmonary and systemic monitoring, opinions differ as to the optimal method of providing anaesthesia for these patients. The su

Cerebral perfusion and oxygenation after the Norwood procedure: comparison of right ventricle-pulmonary artery conduit with modified Blalock-Taussig shunt.

OBJECTIVE: The proposed physiologic advantage of the modified Norwood procedure using a right ventricle-pulmonary artery conduit to supply pulmonary blood flow, compared with a modified Blalock-Taussig shunt, is reduced runoff from the systemic-to-pulmonary circulation during diastole, resulting in a higher diastolic blood pressure and improved systemic perfusion. We hypothesized that the modified Norwood procedure is associated with improved cerebral perfusion and oxygenation. METHODS: Transcranial Doppler sonography and near-infrared spectroscopy were performed in neonates undergoing the No

Effect of Preoperative Tricuspid and/or Mitral Regurgitation on Development of Late Right-Sided Heart Failure After Insertion of the HeartWare Left Ventricular Assist Device.

Right-sided heart failure (RHF) after left ventricular assist device implantation is a significant cause of morbidity and mortality. Although multiple predictors of early RHF have been described, information on late RHF is scarce. The aim of this study was to identify predictors of late RHF in left ventricular assist device patients. A retrospective analysis of all adult patients who underwent HeartWare-ventricular assist device implantation as a bridge to transplantation in a single-centre was performed. Late RHF was defined as RHF requiring rehospitalization after 30 days of implantation. A

Prognostic Value of a Simple Echocardiographic Parameter, the Right Ventricular Systolic to Diastolic Duration Ratio, in Patients with Advanced Heart Failure with Non-Ischemic Dilated Cardiomyopathy.

The prognostic value of the right ventricular (RV) systolic to diastolic duration ratio (S/D ratio) in patients with advanced heart failure is not clear. We enrolled 45 patients with DCM (40 ± 13 years, 33 male) who were admitted to our hospital for evaluation or treatment of heart failure. The RV systolic and diastolic durations were measured using continuous Doppler imaging of tricuspid regurgitation, and the RV S/D ratio was calculated. Cardiac events were defined as cardiac death or left ventricular assist device implantation within the first year. Twenty-eight cardiac events occurred. The

Association between myocardial right ventricular relaxation time and pulmonary arterial pressure in chronic obstructive lung disease: analysis by pulsed Doppler tissue imaging.

This study assessed right ventricular function in chronic obstructive lung disease and pulmonary hypertension by Doppler tissue imaging. Doppler echocardiography of the right ventricle and Doppler tissue imaging of the tricuspid annulus were performed in 63 subjects: 20 healthy controls, 20 with lung disease, and 23 with both lung disease and pulmonary hypertension. Two-dimensional tricuspid systolic plane excursion was lower in patients with pulmonary hypertension than in the other 2 groups. Doppler tricuspid inflow measurements distinguished patients in both of the diseased groups from the c

Tricuspid regurgitation secondary to mitral valve disease: when and how to repair.

The etiology of tricuspid regurgitation (TR) in North American patients with mitral valve disease is almost entirely nonrheumatic functional dilatation of the tricuspid annulus. In mild TR, no operative therapy is necessary since relief of the left sided valve lesion will suffice to bring the pulmonary pressure down and, consequently, relief of TR for moderate (2+ to 3+) TR. We have used a technique of posterior annuloplasty using a DeVega type double running braided suture beginning at the commissure of the anterior and posterior leaflets and running to the commissure between the septal and p

Right Atrial Dysfunction Is Prevalent in Pediatric Acute Respiratory Distress Syndrome and Reflects Pulmonary Hypertension and Right Ventricular Dysfunction.

IMPORTANCE: Right atrial (RA) dysfunction is associated with worse outcomes in some populations with pulmonary hypertension or respiratory failure but the prevalence and correlates of RA dysfunction in pediatric acute respiratory distress syndrome (PARDS) are unknown. OBJECTIVES: The aim of this study was to evaluate RA function by characterizing the prevalence and pattern of RA dysfunction within the first 24 hours of PARDS onset. We hypothesized that RA dysfunction would be common and correlate with the presence of pulmonary hypertension and right ventricular (RV) systolic dysfunction. DES

Pulmonary Valve Replacement for Pulmonary Regurgitation in Adults With Tetralogy of Fallot: A Meta-analysis-A Report for the Writing Committee of the 2019 Update of the Canadian Cardiovascular Society Guidelines for the Management of Adults With Congenital Heart Disease.

BACKGROUND: There is no systematic evidence review of the long-term results of surgical pulmonary valve replacement (PVR) dedicated to adults with repaired tetralogy of Fallot (rTOF) and pulmonary regurgitation. METHODS: Our primary objective was to determine whether PVR reduced long-term mortality in adults with rTOF compared with conservative therapy. Secondary objectives were to determine the postoperative incidence rate of death, the changes in functional capacity and in right ventricular (RV) volumes and ejection fraction after PVR, and the postoperative incidence rate of sustained ventr

Doppler flow patterns in the right ventricle-to-pulmonary artery shunt and neo-aorta in infants with single right ventricle anomalies: impact on outcome after initial staged palliations.

BACKGROUND: A Pediatric Heart Network trial compared outcomes in infants with single right ventricle anomalies undergoing Norwood procedures randomized to modified Blalock-Taussig shunt (MBTS) or right ventricle-to-pulmonary artery shunt (RVPAS). Doppler patterns in the neo-aorta and RVPAS may characterize physiologic changes after staged palliations that affect outcomes and right ventricular (RV) function. METHODS: Neo-aortic cardiac index (CI), retrograde fraction (RF) in the descending aorta and RVPAS conduit, RVPAS/neo-aortic systolic ejection time ratio, and systolic/diastolic (S/D) rati
