We report a case in which one patient had Dowling-Degos disease (reticulate pigmented anomaly of the flexures), hidradenitis suppurativa, and multiple keratoacanthomas. Abnormal epithelial proliferation involving mainly the pilosebaceous apparatus has been recognized in all three conditions. We speculate that a single underlying defect in follicular epithelial proliferation, characterized by variable expressivity, accounts for the coexistence of these clinically distinct disorders of follicular derivation.

Spreading pigmented actinic keratosis is a rarely reported premalignant, pigmented, epidermal tumor that can be difficult to distinguish clinically from other benign and malignant pigmented lesions. In an attempt to determine the cause of clinical hyperpigmentation, seven lesions from five patients were examined by light microscopy. Three of these lesions were also examined using electron microscopy. Findings from these studies indicate that hyperpigmentation is due to enhanced melanosome formation and distribution and not to a melanocyte-keratinocyte melanosome transfer block.

A 63-year-old male school teacher with itchy depigmented macules on his left dorsum manus, left shoulder and abdomen presented at our clinic on 8 July 1986. He had practiced the incense ceremony for about 15 years, and had burnt several incenses and sandalwood. 48 h closed patch testing revealed perfume in the incenses was the cause. We assumed that perfume in the incense was volatized in air when incense was burnt; skin surface contact occurred with airborne particle, which dissolved in sebum; thus allergic contact dermatitis accompanied by depigmentation might arise.

Incontinentia pigmenti (IP) has been associated with CNS involvement, including seizures, retardation, motor abnormalities, and malformations in greater than 30% of patients. Two patients with premycotic poikiloderma developed one squamous cell carcinoma each. A review of the literature reveals 28 reported cases of this association, but in 26 of these the patient had been exposed to carcinogens. Our patients had no history of exposure to carcinogenic agents. In both our cases, the squamous carcinomas behaved aggressively. The possible relationship with the poikiloderma is discussed.

We experienced a case of blue rubber bleb nevus syndrome with familial onset. The patient was a 32-year-old male with a gallstone and many bluish rubber bleb-like hemangiomas on the skin. He suffered from repeated rectal bleeding and underwent a sigmoidectomy at age 17. Gastrointestinal hemangiomas were recognized in the esophagus, stomach, ileum and colon. The fluorescence of routine formalin-fixed sections of a variety of melanocytic lesions has been studied. The technique has value in distinguishing benign and atypical nevi from malignant melanomas. There are several recognizable melanocytic precursors of cutaneous melanoma. These precursors include lentigo maligna, dysplastic melanocytic nevi, congenital nevi (of any size), and darkly pigmented lesions of acral surfaces and mucous membranes. Incontinentia Pigmenti Achromians (IPA) or, more aptly, Hypomelanosis of Ito is now largely accepted as a Neurocutaneous Disorder. Studies, so far, have focussed on the ultrastructural abnormalities and to our knowledge, there have been no attempts to ascertain if there is a particular HLA pattern. We describe an interesting case where IPA occurred in association with Insulin Dependent Diabetes as well as a goitre, suggesting that HLA studies may well be worthwhile. The author describes a case of "Woolly Hair Nevus" in a four years old white boy. The patient presents "Woolly Hair" in three circunscribed areas of the scalp since the age of ten months, and "nevus unius lateris" on the left arm and back of the neck since the age of eight months. The pathology of one of the localized areas of "Woolly Hair" is normal in what respects the hair follicle. Optical microscopy of the hair shaft of the involved areas shows reduced diameter and in two of them Trichonodosis. In some, the hair shaft is twisted in its own axis.
