Emerging pathogens of ocular infections due to environmental changes-what lies ahead?

Due to the global effects of climate change numerous infectious diseases are increasingly spreading to regions that were previously hardly or only slightly affected. As ocular involvement is possible in many of these infectious diseases, we must also adapt to new pathogens and clinical pictures in Germany in the medium to long term. Using selected bacterial, viral and mycotic pathogens and diseases as examples, the causes of the increase in dissemination and the consequences for ophthalmology are presented.

Meeting Report on "The International Congress on Autoimmune Pre-disease (2024)".

The International Congress on Autoimmune Pre-Disease was organized by the German Research Foundation-founded Research Training Group "Autoimmune Pre-Disease" and took place at the University of Lübeck, Germany, on September 16-17, 2024. The event featured various talks and posters from young researchers and international experts and emphasized early interventions and prevention in autoimmune diseases with a focus on systemic rheumatic diseases, pemphigus, and pemphigoid diseases.

Community-acquired and nosocomial bacterial pneumonia are among the most common infectious diseases in Germany. [Acute treatment and secondary prophylaxis of ischemic stroke : An excellent example for personalized medicine].

About a quarter of a million people in Germany suffer a stroke every year. Stroke is the most dreaded cardiovascular disease, even before myocardial infarction and heart failure. In the last two to three years, significant progress has been made in acute treatment, secondary prophylaxis in patients with patent foramen ovale, and the interdisciplinary evaluation of atrial fibrillation as the cause of the stroke. These new findings allow for more precise treatment.

We report on a 55-year-old Thai male who presented during a visit to Germany with acute abdominal pain in the right upper quadrant and signs of cholestasis. The diagnosis of liver fluke infection (opisthorchiasis) was confirmed by the detection of the characteristic eggs of Opisthorchis viverrini in stool microscopy. The patient was treated with praziquantel. Opisthorchiasis is a common parasitic disease and major public health problem in Southeast Asia and in Eastern Europe.

[Epidemiology of sexually transmitted diseases in the catchment area of the Rostock University Dermatology Clinic after 1945].

The decline of STD in the region of Rostock, Germany, since 1945 is explained by a decreasing incidence of gonorrhoea and trichomoniasis. On the other hand chlamydial and candidal infections have increased. The rate of chlamydia infections was about 45% in our venerological patients, as common as gonorrhea used to be. Genital candidal infections were to be found in 23%. Mycoplasmas were identified in 15%, usually combined with other pathogens except in some cases of vaginitis.

Pathological gambling and criminal culpability: An analysis of forensic evaluations presented to German penal courts.

In Germany penal courts and expert witnesses are being confronted with an increasing number of offenders claiming that their criminal behavior had been caused by an irresistible urge to gamble. In 48 of 56 evaluated cases the defendants were diagnosed as pathological gamblers. However, diminished culpability (§ 21 German Criminal Code) was granted in only 32 cases. In these cases the illegal activity to get money could be classified as primarily gambling related.

[Myiasis of the scalp--an incidental finding in ambulatory surgery].

Because of the increase of travels to tropical countries the physician in Germany is more often confronted with parasitic tropical diseases. In an apparently routine case a 28 year old patient suffered from infected stitches of the scalp, caused by an unknown insect, after returning from Sri Lanka. Four larvae of Dermatobia hominis, which causes the cutaneous myiasis, were found into the pyogenic furuncles and were removed by surgical approach.

Psychiatric and legal aspects of persistent litigation.

There is a paucity of literature in English about those people who persistently complain. In Germany and in Scandinavia, a diagnosis of querulent paranoia may be made, although this interesting and uncommon syndrome is rarely recognised in the UK. Five cases of litigiousness or persistent complaining are reported, to illustrate the typical psychopathology and the types of diagnosis that occur. Four of these people are barred from further litigation by the courts (vexatious litigants) and were contacted directly for the purpose of this study.

The effect of various interventions during a typhoid epidemic. Results of a simulation study.

This paper presents an epidemiological model for typhoid fever epidemics or paratyphoid diseases with an enteric course, as well as a deterministic approach for the quantitative representation of this model. The model has been tested against one typhoid and two paratyphoid epidemics which occurred in the Federal Republic of Germany. It was possible to simulate the course of these epidemics with sufficient precision, and to obtain information on the effects of various interventions (e.g. oral vaccination or hygiene measures).
