"The mind is its own place": amelioration of claustrophobia in semantic dementia.

Phobias are among the few intensely fearful experiences we regularly have in our everyday lives, yet the brain basis of phobic responses remains incompletely understood. Here we describe the case of a 71-year-old patient with a typical clinicoanatomical syndrome of semantic dementia led by selective (predominantly right-sided) temporal lobe atrophy, who showed striking amelioration of previously disabling claustrophobia following onset of her cognitive syndrome. We interpret our patient's newfound fearlessness as an interaction of damaged limbic and autonomic responsivity with loss of the cogn…

[Cognitive and behavioral consequences of epilepsy during the course of brain development].

The notion that cognitive and behavioural dysfunctions of epileptic origin can be prolonged and with an insidious onset, has meant a radical conceptual change about epilepsy considered so far uniquely as a paroxysmal disorder. Also, these dysfunctions can be the only observable manifestations of the disorder. It is particularly true in some epileptic syndromes with onset in childhood. Epileptic dysfunctions are characterized by a discontinuity in time (intermittent, recurrent), in space (cerebral localization of the dysfunction and its spread). They often have a significant relationship with s

Lateral supracerebellar transtentorial approach to a middle fossa epidermoid tumor.

A 40-year-old patient was admitted with a 9-month history of headaches and multiple episodes of generalized seizures. Investigations revealed a medial temporal epidermoid tumor that extended into the suprasellar region. The tumor was surgically resected using a lateral supracerebellar-transtentorial approach. The rationale for the surgical approach and its validity in this clinical situation is discussed.

Febrile seizures and mechanisms of epileptogenesis: insights from an animal model.

Temporal lobe epilepsy (TLE) is the most prevalent type of human epilepsy, yet the causes for its development, and the processes involved, are not known. Most individuals with TLE do not have a family history, suggesting that this limbic epilepsy is a consequence of acquired rather than genetic causes. Among suspected etiologies, febrile seizures have frequently been cited. This is due to the fact that retrospective analyses of adults with TLE have demonstrated a high prevalence (20-->60%) of a history of prolonged febrile seizures during early childhood, suggesting an etiological role for the…

A historical case of disseminated chronic tuberculosis.

A British author has put forward a new explanation for Joan of Arc's behaviour. This author suggests that the voices Joan of Arc heard were due to the presence of a temporal lobe tuberculoma in the context of widespread chronic tuberculosis (exposure to bovine tuberculosis, amenorrhoea, heart and intestines incombustible). We consider that some elements are incompatible with widespread tuberculosis. It is difficult to draw final conclusions, but it would seem unlikely that widespread tuberculosis, a serious disease, was present in this 'patient' whose life-style and activities would surely hav…

[Study of the diagnostic specificity by Schneider's first rank symptoms].

This study rediscussed the prevalence of Schneider's first rank symptoms (FRS) in 132 patients with mental diseases and the specificity and the sensitivity of FRS for schizophrenia. Results showed: In the absence of organic or toxic etiology, the specificity of FRS for schizophrenia as opposed to manicdepressive psychosis, was 93%. The sensitivity of FRS for Schizophrenia was 72.9%.

Acute postictal psychosis: a stereo EEG study.

An acute psychosis characterized by auditory hallucinations and paranoid delusions developed in a 19-year-old man with temporal lobe epilepsy after he had a cluster of seizures when antiepileptic drugs (AEDs) had been gradually discontinued. Continuous sterotactic depth and epidural EEG recordings confirmed that this was a postictal rather than an ictal event. Acute postictal psychosis is a self-limited condition phenomenologically distinct from ictal or postictal confusion.

[Duplication of the middle cerebral artery and the accessory middle cerebral artery].

A double middle cerebral artery and their branches are described. Accessory middle cerebral arteries are interpreted as thinner twigs of the anterior cerebral or the anterior choroidal arteries.

Formed visual hallucinations after metrizamide myelography.

About 12 hours after 77-year-old woman underwent metrizamide myelography of the posterior fossa to elucidate the cause of downbeat nystagmus … 105 units/liter. The hallucinations were probably caused by penetration of the metrizamide into the temporal lobe and visual association cortex.

[The neurosurgical treatment of the cerebral localisations of the alveolaris (multilocularis) echinococcosis (author's transl)].

A case of adult woman surgically treated for hepatic alveolaris ecchinococcosis on 1975 is presented. On 1976, a hypertensive intracranial syndrome is treated by removal of a right temporal kystic localisation. On 1977, a new localisation is successfully raised. The authors, in a short review of the literature note that these cerebral localisations are not always terminal. The alveolaris ecchinococcosis is now autochthonous in France.
