OBJECTIVE: To evaluate the feasibility and surgical procedure of bone grafting with elastic trephine.
METHODS: A total of 42 cases of thoracolumbar fracture received pedicle screw fixations by combining bone grafting with elastic trephine through pedicle of fractured vertebra. The ratios of the altitude of traumatic vertebra to normal altitude, kyphosis Cobb angle and sagittal index were measured by preoperative and postoperative X-ray images.
RESULTS: A follow-up visit of 12-26 months (average: 14.8) showed that the dislocations of vertebrae were corrected and fusions completed. Proportion

BACKGROUND: Reports of nonunion of proximal fifth metatarsal fractures treated by internal fixation indicate that current fixation methods do not always adequately address the stresses to which the bone is subjected during ambulation. In particular, the insertion sites of the peroneus brevis and peroneus tertius tendons on the fifth metatarsal suggest that their actions can impose torsional stresses on the areas of the bone in which Jones fractures and stress fractures occur. Intramedullary screw fixation, however, offers little resistance to rotation of the proximal and distal fragments relat

PURPOSE OF THE STUDY: The authors describe a new surgical technique of tendo achilles reconstruction using bone tendon plasty from the patellar tendon, for rare chronic ruptures of the Achilles tendon flush within its calcaneal insertion, with distal loss of substance, requiring bone fixation.
METHODS: The two step operation was carried out either in lateral or ventral decubitus (the latter two involving repositioning the patient): 1) Plasty harvesting. By a short anterior approach, we harvested either a strip of patellar tendon (10 mm wide) with a piece of tibia tuberosity or a strip of rect

Background and Objectives: Adequate soft tissue thickness and keratinized mucosa are essential for the long-term health and esthetics of the peri-implant area. A porcine acellular dermal matrix (PADM) has shown promise in augmenting soft tissue, but reliable fixation remains a challenge. Materials and Methods: This case series describes the use of a PADM fixed with resorbable magnesium screws (NOVAMag®) in three patients requiring peri-implant soft tissue augmentation. The grafts were stabilized with magnesium screws on the buccal side. The clinical outcomes were evaluated over a period of 3-6

PURPOSE: To investigate the biomechanical properties of bilateral SAI annular internal fixation in the treatment of unilateral Denis type Ⅱ sacral vertical fracture, so as to provide a reference for clinical practice.
METHODS: The finite element approach was utilized to simulate the Denis type Ⅱ fracture of the right sacral (the entire fracture through the sacral foraminarum) as well as the fracture of the upper and lower ramus of the right pubic bone, which represents unilateral vertical instability of the pelvic ring. The posterior pelvic ring was fixed using three different methods: lumbop

Sliding hip screws (SHSs) are commonly used for the operative treatment of extracapsular neck of femur fractures. We report 2 rare cases of intrapelvic migration of an SHS, resultant of non-unions, threatening regional major vessels. Only a handful of such cases have previously been reported in the literature. We provide an up-to-date review of the literature, propose rationale for how a lag screw may migrate, and suggest how this complication can be prevented.

Traditional treatment methods have certain limitations. In recent years, the technique of internal fixation with double-plane double-supported screws based on X-ray images has been proposed to improve the therapeutic effect. The main objective of this research was to examine the effectiveness of the X-ray image-based bi-planar double-braced screw internal fixation technique . During surgery, the procedure was determined based on X-ray images, followed by an open reduction procedure at the fracture site, and finally internal fixation using bi-planar double-support screws. All patients were succ

Weber Type B fractures often arise from external rotation with the foot supinated or pronated. Altered tibiofibular joint kinematics in Weber B fractures are responsible for syndesmotic damage seen in Weber B fractures. Weber B fractures are managed using open reduction and internal fixation if displaced. The syndesmosis is injured in up to 40% of cases resulting in an unstable injury with a syndesmotic diastasis. This systematic review aimed to evaluate the current literature on syndesmotic fixation in Weber B fractures, assess the outcomes and complications of syndesmotic fixation and assess

BACKGROUND: Severe adolescent idiopathic scoliosis (AIS) can be treated with instrumented fusion, but the number of anchors needed for optimal correction is controversial.
METHODS: We conducted a multicenter, randomized study that included patients undergoing spinal fusion for single thoracic curves between 45° and 65°, the most common form of operatively treated AIS. Of the 211 patients randomized, 108 were assigned to a high-density screw pattern and 103, to a low-density screw pattern. Surgeons were instructed to use ≥1.8 implants per spinal level fused for patients in the high-implant-den

Ankle fractures in patients with diabetes can be difficult to manage, especially when patients present with hyperglycemia. Treatment often requires a combination of both medical and surgical care, especially in patients with poorly controlled diabetes. The goal of any treatment is to obtain a well-aligned ankle fracture that heals without any further displacement and to avoid the development of a Charcot joint. Nonsurgical treatment is usually reserved for nondisplaced fractures. Displaced fractures often require surgical treatment, and there are different options available, including standard

BACKGROUND: The purpose of this study is to explore the feasibility and accuracy of a robot-assisted technique in four-corner fusion compared with traditional freehand operation.
METHODS: Twenty cadaver specimens were randomly assigned to the robot-assisted group and freehand groups. Three screws were placed percutaneously to fix the capitate-lunate joint, lunate-triquetrum joint, and triquetrum-hamate-capitate joint in each specimen by robot-assisted or freehand technique. The offset between the actual and planned screw positions was determined by merging the images of intraoperative and pos

This case report describes a complex class II case with important maxillo-mandibular asymmetry in an adult patient treated with lingual straight-wire appliance. With the twofold aim of obtaining dramatic aesthetic improvement, since the patient refused surgical option, and ideal occlusal relationship, accurate set-up planification and biomechanical strategies with TADS for occlusal plane cant correction are needed to achieve the planned results. This case report demonstrates the possibility of solving successfully class II malocclusion with significant asymmetry in adult patient without surger

PURPOSE: There is no specific literature on the best implantation position of the Femoral Neck System (FNS) for treating Pauwels type III femoral neck fracture in young adults.
METHODS: Use finite-element analysis to compare the mechanical properties of implantation positions: FNS in the central position, FNS in the low position, and FNS in the low position combined with cannulated screw (CS). The CT data of the femur were imported into the mimics20.0 to obtain the three-dimensional model of the femur; imported into geomagic2017 and SolidWorks 2017 for optimizations; models of FNS and CS are

Fractures of the upper end of the femur (FUEF) are a serious complication for the elderly, affecting 1,3 million patients a year worldwide (1). Studies dedicated to the post-operative follow-up of patients operated on for hip fracture in the long term are rare. The aim was to determine the epidemiological and evolutionary profile focusing on morbidity and mortality (survival and functional and cognitive decline) in the year following surgery for FUEF in patients aged over 65. Methods: Prospective observational study. Geriatric data were collected at the time of surgical management of FUEF with

Pediatric spondylolisthesis is a common cause of back pain in children, typically managed conservatively with bracing and non-steroidal anti-inflammatory drugs. When posterolateral fusion is performed for refractory pain, pseudarthrosis and implant failure may occur, necessitating reoperation. To improve patient outcomes, there is a need for alternative surgical techniques to effectively manage high-grade isthmic slips. Here, the authors report the case of a child with Meyerding grade III anterolisthesis of L5 on S1 who was treated with a single-level, instrumented fusion using bilateral S1-L5

BACKGROUND: Intracapsular femoral neck fractures are challenging to treat, with outcomes depending on the quality of reduction, and the stability of fixation. Cannulated cancellous screws (CCS) are the most commonly used implants to fix these fractures, but failure rates are significant. The recently introduced femoral neck system (FNS) may be a better option than CCS fixation and this review attempts to compare the results.
METHODS: Four electronic databases were searched for eligible articles that had comparative data on the outcomes of fixation of adult femoral neck fractures with FNS and
