From theory to practice: a Belgian consensus on minimal monitoring strategies for patients with IBD treated with advanced therapies.

BACKGROUND AND STUDY AIMS: Inflammatory bowel diseases (IBD) are chronic conditions requiring lifelong management, often involving advanced therapies. To harmonize the current heterogeneity in monitoring practices we developed a Belgian Standard of Care (SOC) guidance for patients with IBD treated with these advanced therapies. METHODS: A multistep approach was adopted. First, a core team of IBD clinicians conducted a national survey to evaluate current practices and identify gaps. Next, a SOC guidance document was drafted and reviewed by multiple stakeholders, including physicians and IBD nu

The Impact of Family Strengths Oriented Therapeutic Conversations on Parents of Children with a New Chronic Illness Diagnosis.

A growing number of families with children are dealing with a new diagnosis of chronic illnesses or health problems that are demanding. Nurses are in a prime position to provide support and empowerment to these families. The aim of the study was to evaluate the benefits of two sessions of a Family Strengths Oriented Therapeutic Conversation (FAM-SOTC) intervention, offered by advanced practice nurses (APNs) to mothers (N = 31) of children and adolescents in Iceland with newly diagnosed chronic illnesses/disorders. Families of children with Juvenile Idiopathic Arthritis (JIA), epilepsy, Type 1

Polymedicine and aging. Enhancing older adult care through advanced practitioners. GNPs and elder care pharmacists can help provide optimal pharmaceutical care.

Approximately one third of all drugs prescribed in the United States are considered unnecessary. Polymedicine, polypharmacy, medication nonadherence, and adverse drug reactions are among the top five causes of acute care hospitalizations in persons older than age 65. These conditions significantly increase health care costs and will continue to do so at an alarming rate as America's population ages. Although medications are an important factor in improving and maintaining the quality of life for older adults, polymedicine places them at risk of an acute care admission for medication-related pr

Structure and processes of interdisciplinary geriatric consultation teams in acute care hospitals: A scoping review.

BACKGROUND AND OBJECTIVES: Interdisciplinary geriatric consultation teams are implemented in the acute hospital setting in several high-income countries to provide comprehensive geriatric assessment for the increasing numbers of older patients with a geriatric profile hospitalized on non-geriatric units. Given the inconclusive evidence on this care model's effectiveness to improve patient outcomes, health care policy and practice oriented recommendations to redesign the structure and process of care provided by interdisciplinary geriatric consultation teams are needed. A scoping review was con

Opioid Use Disorder Practice by Licensure Category in North Carolina.

BACKGROUND: Nurse practitioners, physician assistants, and certified nurse midwives, collectively known as advanced practice providers (APPs), are critical members of the opioid use disorder (OUD) treatment workforce. Few studies compare the OUD treatment practice patterns of APPs relative to physicians. METHODS: In this cross-sectional study, we distributed a survey examining general substance use disorder (SUD) and OUD-specific treat-ment practices to all licensed physicians and APPs in North Carolina, resulting in a sample of 332 respondents. Respondents were asked about screening, referra

Enhancing Pediatric Patient Safety by Evaluating the Effect of Targeted Education on Nurses' Knowledge, Attitudes, and Practices of Fall Prevention.

Background Patient safety in pediatric care is a critical concern, as hospital environments pose unique risks for children, particularly falls. Despite advancements in healthcare, falls remain a common safety issue, emphasizing the need for effective preventive measures. Nurses play a vital role in reducing fall risks, but gaps in their knowledge, attitudes, and practices (KAP) can hinder the success of these efforts. This study evaluates the impact of targeted education on enhancing nursing officers' KAP of fall prevention. Materials and methods A quasi-experimental pre-test and post-test des

Utility of skinfold thickness measurement in non-ambulatory patients with Duchenne muscular dystrophy.

Nutritional disorders in Duchenne muscular dystrophy (DMD) worsen the medical condition. In particular, obesity is a serious problem that increases the risk of cardiomyopathy and affects nursing care. However, it is often difficult to evaluate body fatness in the advanced stages of DMD. Skinfold thickness measurement is a classical method to evaluate body fatness and is easily performed, even for bed-bound patients at home. We aimed to investigate the utility of skinfold thickness measurement in non-ambulatory DMD patients. Twenty-two patients with non-ambulatory, steroid-naive DMD ranging in

Advancing paternal perinatal mental health: Integrating qualitative evidence, theory, and measurement science.

AIM: To advance understanding of paternal perinatal mental health through the integration of qualitative evidence, mid-range theory, and conceptual framework development. BACKGROUND: Existing scholarship largely emphasizes prevalence and symptom identification of paternal postpartum depression, with limited integration of fathers lived experiences into theory development and assessment frameworks, and mental health nursing practice. Greater integration of qualitative evidence, theory, and measurement science may strengthen the recognition and assessment of paternal distress. METHODS & RESULT

Reimagining Supportive Approaches at the Intersection of Mandatory Reporting Policies for the Mother-Infant Dyad Affected by Substance Use.

BACKGROUND: As rates of substance use during pregnancy persist, the health and optimal development of infants with prenatal substance exposure remain a key priority. Nurses are tasked with identifying and reporting suspected cases of child maltreatment, including abuse and neglect, which is often assumed to be synonymous with substance use during pregnancy. While policies aimed at protecting infants from child abuse and neglect are well intentioned, literature regarding the short- and long-term social and legal implications of mandatory reporting policies is emerging. PURPOSE: In this article

Improving care for patients who experience miscarriage in emergency departments: a practice innovation.

Vaginal bleeding during pregnancy is a common patient presentation in emergency departments (EDs), and in some cases this will occur due to miscarriage. However, there are several barriers to effective and sensitive communication with patients experiencing a miscarriage. Women presenting to EDs who are experiencing a miscarriage are more likely to be psychosocially vulnerable and less satisfied with their care compared with those seeking care in the outpatient setting. There is a gap in nursing and advanced practice provider preparation regarding techniques for breaking bad news to patients in

Improving the quality of life of people with advanced respiratory disease and severe breathlessness.

UNLABELLED: Advanced respiratory disease imposes a greater symptom burden than many cancers but not does have comparable recognition of the need for supportive and palliative care or the infrastructure for its systematic delivery. Consequently, many people with advanced respiratory disease (and those closest to them) have a poor quality of life, disabled by chronic breathlessness, fatigue and other symptoms. They are socially isolated by the consequences of long-term illness and are often financially impoverished. The past decade has seen an increasing realisation that care for this group must

Calm Down With Kava: What Clinicians Need to Know.

Kava is a neuroactive medicinal herb that can induce pharmacological effects when ingested. As an herbal remedy, kava exhibits sedative, anesthetic, euphoriant, and entheogenic effects. Kava is used as a relaxant, pain reliever, and remedy for anxiety and insomnia. In the United States, kava is marketed as a safe dietary supplement. Kava's popularity is on the rise due to heightened awareness and interest in natural plant-based health alternatives. Although meta-analyses and systematic reviews of kava use in treating anxiety are favorable, results remain inconsistent. Due to poor quality contr

School-Based Psychiatric Mental Health Nurse Practitioners to Decrease Inappropriate Psychiatric Pediatric Emergency Rooms Presentations.

An alarming number of children are being referred to pediatric emergency rooms for disruptive behavior disorders (DBDs) such as attention deficit hyperactivity disorder, oppositional defiant disorder, and conduct disorders. Although presenting symptoms of an exacerbation of disruptive behaviors may be urgent, often they do not require emergency care and could have been efficiently handled through outpatient care. Inappropriate emergency room (ER) referrals have been linked to inadequate school, family, or self-referrals. This article proposes using psychiatric mental health nurse practitioners

Challenges in nursing: infants with congenital diaphragmatic hernia.

Despite advances in the care of infants born with congenital diaphragmatic hernia, mortality rate continues to be high. Immediate survival is directly related to the degree of pulmonary hypoplasia present. The spectrum is wide, from minimal pulmonary hypoplasia, in which neonates do well, to severe pulmonary hypoplasia, which is incompatible with life. Between these two extremes lie infants with compromised pulmonary function whose long-term survival depends on the clinical strengths of their multidisciplinary team. Over the past year, 23 infants with congenital diaphragmatic hernia were cared
