Sinus node recovery time in the elderly.

Measurement of the sinus node recovery time has been proposed as a diagnostic tool for recognition of the sick sinus syndrome. The latter is most frequently encountered in elderly patients with hypertension, coronary heart disease, and atherosclerosis. In order to provide normal values for the sinus node recovery time in this particular population group, atrial pacing studies were carried out in 30 subjects over 50 years of age, all with peripheral vascular disease and some with angina pectoris (10), residua of infarction (6), or hypertension (7). On stimulation, 7 patients maintained a I:I at

Atrial Synchrony Restored With Bachmann Bundle Pacing.

CASE SUMMARY: Capturing intrinsic interatrial conduction with Bachmann bundle (BB) pacing is known as a more physiological atrial pacing strategy than conventional right atrial appendage pacing. We present a 74-year-old man with sinus node dysfunction and paroxysmal atrial fibrillation who underwent dual-chamber pacemaker implantation, incorporating BB and left bundle branch ventricular pacing. Interatrial and atrioventricular synchrony was achieved with a shorter P-wave duration and normalization of the PR interval. TAKE-HOME MESSAGE: BB pacing may restore physiologic atrial activation by en

Left Bundle Branch Block: A Reversible Pernicious Effect of Lacosamide.

A 95-year-old male with a medical history of focal epilepsy presented with transient ischemic attack (TIA)/pre-syncope like symptoms. He was on lacosamide (LCM) and levetiracetam. On evaluation, he was found to have left bundle branch block (LBBB), sinus pause of three seconds, and 1st degree atrioventricular (AV) block. After holding LCM, electrocardiogram changes were reversed to baseline (before commencing LCM). In conclusion, to the best of our knowledge, this is the first case of reversible LBBB along with sinoatrial (SA) node and AV node dysfunction in an elderly male on LCM therapy.

Bradyarrhythmias: Clinical Presentation, Diagnosis, and Management.

Bradyarrhythmias are common clinical findings consisting of physiologic and pathologic conditions (sinus node dysfunction and atrioventricular [AV] conduction disturbances). Bradyarrhythmias can be benign, requiring no treatment; however, acute unstable bradycardia can lead to cardiac arrest. In patients with confirmed or suspected bradycardia, a thorough history and physical examination should include possible causes of sinoatrial node dysfunction or AV block. Management of bradycardia is based on the severity of symptoms, the underlying causes, presence of potentially reversible causes, pres

Stellate ganglion block: a therapeutic alternative for patients with medically refractory inappropriate sinus tachycardia?

Inappropriate sinus tachycardia (IST) is a clinical syndrome characterized by presence of non-paroxysmal tachyarrhythmia manifesting with increased resting heart rate and exaggerated or persistent response to exercise or position. When IST is intolerable and medically refractory, invasive therapies such as sinus node modification or atrioventricular node ablation with placement of permanent pacemaker are tried to control symptoms. We present a 34 year old patient with symptomatic IST unresponsive to medical therapy who underwent anesthetic block of the right and left stellate ganglia. At four …

The Maze procedure: surgical therapy for refractory atrial fibrillation.

Although atrial fibrillation is well tolerated by most patients, in some patients the consequences may be severe. The Maze procedure is a new open-heart operation that creates a carefully designed maze of incisions in the atrial myocardium; this maze then acts as an electrical conduit to channel atrial impulses from the sinoatrial node to the atrioventricular node. The Maze procedure has been shown to restore sinus rhythm and atrial systole (thus reducing the risk of thromboembolism), improve hemodynamics, alleviate palpitations, and eliminate the need for antiarrhythmic and anticoagulant drug…

Sinus and atrioventricular nodal reentrant tachycardia in the same patient.

Sinus and atrioventricular (A-V) nodal reentry are shown to coexist in the same patient, and the following conclusions are drawn: (1) Reentry at one nodal site may mask reentry at the other nodal site, (2) concealed reentry at either site may become manifest reentry under the appropriate conditions, (3) manifest sinus nodal reentry may alternate with manifest A-V nodal reentry, and (4) a Wenckebach type phenomenon manifest in the A-V node and concealed in the sinus node may in some instances be the basis for coexistent sinus and A-V nodal reentry in man.

Pathologic Evidence of Granulomatosis With Polyangiitis as a Cause of Atrioventricular Block.

BACKGROUND: Atrioventricular block (AVB) is a rare manifestation of granulomatosis with polyangiitis (GPA), and the underlying pathologic relationship between GPA and AVB remains poorly understood. CASE SUMMARY: We present the case of a 71-year-old woman who developed complete AVB and had a history of microscopic polyangiitis. Despite the absence of clinical signs indicative of GPA relapse throughout her lifetime, postmortem examination revealed microgranulomas and multinucleated giant cells within the atrioventricular node. DISCUSSION: This case provides pathologic evidence that GPA can lea…

[Electrophysiological mechanisms of the effect of allapinin in patients with paroxysmal supraventricular tachycardia].

An intracardiac electrophysiological study was undertaken to examine 15 patients with paroxysmal supraventricular tachycardias. Allapinin in … f paroxysmal supraventricular tachycardias. The agent fails to virtually affect the function of the atrioventricular node in the anterograde direction in the two types of the tachycardia. Thus, allapinin has the mechanism of action that is typical of quinidine-like drugs used in supraventricular tachycardias.

Atrioventricular dissociation during paroxysmal junctional tachycardia.

We describe the rare occurrence of atrioventricular dissociation in three patients during paroxysmal junctional tachycardia. The mechanism of tachycardia was atrioventricular nodal reentry in to patients. The third patient had reentrant tachycardia that utilized the AV node for at least part of the reentrant circuit. This patient also had a nodoventricular pathway that may have participated in the reentrant circuit. In two patients, ventricular tachycardia was diagnosed prior to electrophysiologic assessment and medication to prevent tachycardia was not successful. After electrophysiological s

Electrophysiological effects soon after intravenous beta-methyldigoxin.

The digitalis derivative beta-methyldigoxin has been shown to be quickly and well absorbed from the gut and, in hemodynamic studies, to start acting rapidly after intravenous administration. However, when tested on 6 patients suffering from paroxysmal reciprocating atrioventricular tachycardia, or having an accessory pathway who might develop this disorder, there was no effect on induced tachycardias or on AV conduction during rapid atrial pacing or the extrastimulus test. One of the 6 patients showed some increase in refractoriness of conduction through the AV node within 25 min after the inj

[Cardiac electrophysiologic effects of prostacyclin in man].

The electrophysiological effects of prostacyclin (PGI2) at increasing doses (2.5, 5 and 10 ng/kg/min) were assessed in 16 patients during classical investigations of sinus node function and atrioventricular conduction and during programmed atrial and right ventricular under basal conditions and during prostacyclin perfusion. Ten patients had normal sinus node function and atrioventricular conduction under basal conditions. Stastistically significant changes were observed during PGI2 perfusion: shortening of the sinus cycle length (p 0.01), decreased intraatrial conduction time (p less than 0.0

Modern physiological approach to inappropriate ICD shocks due to atrial fibrillation with very fast ventricular response. A case report.

BACKGROUND: Fast-conducting atrial fibrillation misinterpreted as ventricular tachycardia is the leading cause for inappropriate shocks in p … e didn`t find any reversible causes for the heart rate acceleration, we opted for rate control with atrioventricular node ablation. To counteract the risk of pacing-induced cardiomyopathy in this patient who would become totally pacemaker-dependent, we successfully performed left bundle branch area pacing. A novel approach to typical atrioventricular nodal reentrant tachycardia with high-resolution mapping using the CARTO 3 cardiac mapping system.

BACKGROUND: We hypothesized that high-resolution activation mapping during sinus rhythm (SR) in Koch's triangle (KT) can be used to describe the most delayed atrial potential around the atrioventricular node and evaluated whether ablation targeting of this potential is safe and effective for the treatment of patients with typical atrioventricular nodal reentrant tachycardia (AVNRT). METHODS: We conducted a prospective, non-randomized, observational study using high-resolution activation mapping from the sinus node to KT with a PENTARAY or OCTARAY catheter using the CARTO 3 cardiac mapping sys…

Left bundle branch pacing utilizing three dimensional mapping.

INTRODUCTION: Permanent His bundle pacing is feasible and effective in patients with atrioventricular block or left bundle branch block. However, pacing thresholds to capture the distal His bundle is often higher. Recently left bundle branch area pacing (LBBP) has been shown to be feasible by advancing the lead transvenously, deep into the interventricular septum to reach the left ventricular endocardial surface. In this article we describe the utility of three dimensional (3D) mapping to achieve LBBP. METHODS: Ensite Precision (Abbott) mapping system was used to perform LBBP. A decapolar cat

Therapeutic Effect of His-Purkinje System Pacing Proportion on Persistent Atrial Fibrillation Patients With Heart Failure.

BACKGROUND: His-Purkinje system pacing (HPSP) combined with atrioventricular node ablation is an effective therapy for atrial fibrillation (AF) patients with heart failure (HF). However, atrioventricular node ablation has some limitations and disadvantages. HPSP combined with β -blockers reduces intrinsic heart rate and increases pacing proportion, which may be an alternative to HPSP combined with atrioventricular node ablation. This study was to assess the therapeutic effect of different HPSP proportion on AF patients with HF. METHODS: The study enrolled 30 consecutive persistent AF patients…
