Shoe Dermatitis.

Shoe dermatitis is a type of contact dermatitis precipitated by allergens or irritants found in shoes. Potassium dichromate, commonly used in leather processing, is one of the most prevalent agents responsible for shoe dermatitis; however, it is not the only one. Shoe dermatitis caused by an allergen or an irritant may affect a person of any age, sex, or ethnicity. Numerous treatments exist for shoe dermatitis, the most simple yet important being avoidance of causative agents. Pharmaceutical agents commonly used are emollients, humectants, and topical corticosteroids. In more severe cases, top

[Algorithm to facilitate the management of onychomycosis].

An algorithm is proposed to facilitate the management of onychomycosis and ensure a good therapeutical approach. Its aim is to simplify the diagnosis process by integrating a pertinent anamnesis and physical examination with the classic tool of mycology (direct examination, culture, eventually examination by PCR), in order to give the adequate targeted treatment.

How do we measure efficacy of therapy in onychomycosis: Patient, physician, and regulatory perspectives.

BACKGROUND: Onychomycosis is a difficult to treat fungal infection of the nail plate. Definitions of treatment outcomes measured in clinical trial settings vary widely, preventing the comparison of data across studies. OBJECTIVE: To investigate definitions of efficacy measures used in clinical trial settings for the treatment of onychomycosis and discuss the importance of consistent reporting. METHODS: We searched Pubmed and Ovid for studies in which oral and topical drugs were used to treat onychomycosis. RESULTS: The majority of clinical trials investigated used mycological and clinical c

Efinaconazole 10% topical solution for the topical treatment of onychomycosis of the toenail.

Efinaconazole 10% topical solution is a new antifungal therapy for the topical treatment of mild to moderate toenail onychomycosis. In vitro and in vivo data have shown significant antifungal activity against dermatophytes, Candida spp. and nondermatophyte molds, and its mechanism of action is through inhibition of fungal lanosterol 14α-demethylase. In two parallel, double-blind, randomized, controlled, Phase III trials, complete cure rates were 17.8 and 15.2%, respectively, and mycological cure rates were 55.2 and 53.4%, respectively, for efinaconazole 10% topical solution, which were superio

Therapies for onychomycosis a systematic review and network meta-analysis of mycological cure.

New therapies for onychomycosis continue to be developed, yet treatments are seldom directly compared in randomized controlled trials. The objective of this study was to compare the rates of mycological cure for oral and topical onychomycosis treatments using network meta-analysis. A systematic review of the literature on onychomycosis treatments published before March 25, 2013, was performed, and data were analyzed using network meta-analysis. Terbinafine, 250 mg, therapy was significantly superior to all treatments except itraconazole, 400 mg, pulse therapy; itraconazole, 200 mg, therapy was

Topical treatment of onychomycosis with efinaconazole solution 10%.

Onychomycosis, a common nail infection that often is associated with substantial patient distress, disability, and pain, is a challenge to manage successfully. This report presents the case of a 44-year-old man with moderate distallateral subungual onychomycosis (DLSO) of more than 5 years' duration and discusses effective treatment with efinaconazole solution 10%, a new topical antifungal, once daily for 48 weeks. At baseline, 50% of the great toenail was affected and laboratory test results were positive for Trichophyton rubrum. Mycologic cure was noted at week 36 with complete cure observed

Update: nail unit dermatopathology.

Nail unit dermatopathology is a growing field filled with many challenges. Many advances in this field have been made in the last 5 years. This review article provides an update on new information and studies published in that period of time. We divided these works into different sections, including clinical and pathologic challenges in diagnosis and treatment of nail disorders, nail unit biopsy and processing techniques, normal nail unit histology, nail plate structural and growth pathology, metabolic disease, inflammatory conditions, onychomycosis, benign growths, malignant growths, and dysc

Trichophyton rubrum onychomycosis in an 8-week-old infant.

An 8-week-old infant presented with 7 weeks history of nail involvement and discoloration. Lesions started over the middle fingernail of right hand at 1 week of age, spreading over to other nails within 2 weeks. Only two nails of the feet were spared. On KOH examination, fungal hyphae were seen and culture showed growth of Trichophyton rubrum. The purpose is to report the earliest case of onychomycosis having multiple nail involvement of fingers and toes (18 nails).

Acroangiodermatitis (pseudo-Kaposi sarcoma): a rarely-recognized condition. A case on the plantar aspect of the foot associated with chronic venous insufficiency.

Acroangiodermatitis, often known as pseudo-Kaposi sarcoma, is an uncommon angioproliferative entity related to chronic venous insufficiency, arteriovenous fistulae, paralysed limbs, amputation stumps, vascular syndromes and conditions associated with thrombosis. It presents most frequently as purple macules, papules or plaques in the dorsal aspects of the feet, especially the toes, and the malleoli. We report a case of acroangiodermatitis in the plantar aspect of the foot, misdiagnosed for two years, in which haematoxylin-eosin hystopathological stain and immunolabeling with CD34 histochemistr

[Red painful feet after visiting a swimming bath].

Idiopathic palmoplantar eccrine hidradenitis is characterised by painful erythematous plantar and/or palmar nodules and is triggered by exposure to wet and cold conditions. The course is benign and self-limiting with a tendency for recurrence. We describe two cases in otherwise healthy children after physical activity in a swimming bath. They were hospitalised for one and three days, respectively. Diagnostic considerations are briefly reviewed. Sufficient attention to the characteristic clinical appearance could prevent unnecessary diagnostic procedures and hospitalisation.
