Bacterial arthritis of native joints can be successfully managed with needle arthroscopy.

PURPOSE: To assess the feasibility of needle arthroscopy for management of suspected bacterial arthritis in native joints. METHODS: During a pilot period, patients presenting with symptoms suggestive of native joint bacterial arthritis were eligible for initial management with needle arthroscopy. Procedures were performed in the operating theatre or at the patient bedside in the emergency department or inpatient ward. As our primary outcome measure, it was assessed whether needle arthroscopic lavage resulted in a clear joint. In addition, the need for conversion to standard arthroscopy or art…

The Interchondral Joints of Thorax in Microtia Surgery: Classification and Fabrication Strategies.

BACKGROUND: The interchondral joint between the sixth and seventh costal cartilages, called synchondrosis, assists in harvesting and fabricating the microtia framework. However, its looseness often complicates the microtia surgery. We aimed to classify the interchondral joints based on looseness and identify predictors for each subtype. METHODS: Electronic chart and intraoperative photographs were reviewed for consecutive microtia patients who underwent costal cartilage graft for ear reconstruction from June 2001 to February 2020. The sixth and seventh costal interchondral joint was classifie…

Load Carriage During Walking Increases Dynamic Stiffness at Distal Lower Limb Joints.

The addition of a load during walking requires changes in the movement pattern. The investigation of the dynamic joint stiffness behavior may help to understand the lower limb joints' contribution to these changes. This study aimed to investigate the dynamic stiffness of lower limb joints in response to the increased load carried while walking. Thirteen participants walked in two conditions: unloaded (an empty backpack) and loaded (the same backpack plus added mass corresponding to 30% of body mass). Dynamic stiffness was calculated as the linear slope of the regression line on the moment-angl…

Seismic Performance of Steel Fiber Reinforced High-Strength Concrete Beam-Column Joints.

In high-strength concrete, the reinforcement concentration will cause some problems in the beam-column joints (BCJs) due to a large amount of transverse reinforcement. Hence, the main object of this paper is to prevent the reinforcement concentration and reduce the amount of transverse reinforcement in the BCJs through the ideal usage of steel fibers and reinforced high-strength concrete. Pseudo-static tests on seven specimens were carried out to investigate and evaluate the seismic performance of beam-column joints in steel fiber reinforced high-strength concrete (SFRHC). Test variables were …

Equal Load-Carrying Design of Lapped Joints of Al-Cu Dissimilar Materials.

In order to avoid the adverse effects of additional moment and stress concentration of traditional lap joints, a new lap joint was put forward, according to the concept of "equal load-carrying". Through static analysis and brazing characteristics consideration, the equal load-carrying design method of Al-Cu lap joint based on brazing method was established. Through three types of brazing, the relationship among two fracture modes, brazing process and static tension curve of lap joint, was analyzed. The results demonstrated that the selection of solder was required to simultaneously meet the re…

[Performance control after arthroscopic arthrolysis with capsulectomy in fresh-frozen elbow joints].

BACKGROUND AND OBJECTIVE: Posttraumatic or postoperative movement restrictions in elbow joints can often occur (including capsular contracture) and can generate everyday limitations. In persistent elbow stiffness, arthroscopic arthrolysis with removal of the dorsal and ventral capsule portions can be carried out. The purpose of this study was to assess the efficacy of arthroscopic capsulectomy by means of an in vitro anatomical study. METHODS: A standardized elbow arthroscopy with ventral and dorsal capsulectomy was performed and image-documented in five fresh-frozen elbow specimens. Subseque…

[Bony contact in arthritic and healthy elbow joints : Studies in cadaveric specimens].

BACKGROUND: Arthroscopic and open debridement arthroplasty for elbow arthrosis produce excellent results. Resection of the tip of the olecranon and coronoid, as well as debridement of the associated fossae, is frequently described as sufficient for restoring range of motion and reducing pain. OBJECTIVES: The purpose of this study was to demonstrate that medial and lateral osteophytes of the olecranon and the coronoid usually mark the first bony contact and may, therefore, restrict range of motion. MATERIAL AND METHODS: 11 fresh-frozen elbows were dissected. The fossae were coated with Optosi

Functional disorders of the temporomandibular joints: Internal derangement of the temporomandibular joint.

Temporomandibular joint (TMJ) is one of the most complex joints of the human body. Due to its unique movement, in terms of combination of rotation and translator movement, disc of the joint plays an important role to maintain its normal function. In order to sustain the normal function of the TMJ, disc must be kept in proper position as well as maintain normal shape in all circumstances. Once the disc is not any more in its normal position during function of the joint, disturbance of the joint can be occurred which will lead to subsequent distortion of the disc. Shape of the disc can be influe…

The relationship between target joints and direct resource use in severe haemophilia.

OBJECTIVES: Target joints are a common complication of severe haemophilia. While factor replacement therapy constitutes the majority of costs in haemophilia, the relationship between target joints and non drug-related direct costs (NDDCs) has not been studied. METHODS: Data on haemophilia patients without inhibitors was drawn from the 'Cost of Haemophilia across Europe - a Socioeconomic Survey' (CHESS) study, a cost assessment in severe haemophilia A and B across five European countries (France, Germany, Italy, Spain, and the United Kingdom) in which 139 haemophilia specialists provided demog…

Translational Motion Tracking of Leg Joints for Enhanced Prediction of Walking Tasks.

OBJECTIVE: Walking task prediction in powered leg prostheses is an important problem in the development of biomimetic prosthesis controllers. This paper proposes a novel method to predict upcoming walking tasks by estimating the translational motion of leg joints using an integrated inertial measurement unit. METHODS: We asked six subjects with unilateral transtibial amputations to traverse flat ground, ramps, and stairs using a powered prosthesis while inertial signals were collected. We then performed an offline analysis in which we simulated a real-time motion tracking algorithm on the ine…

Variability of the Dynamic Stiffness of Foot Joints: Effect of Gait Speed.

BACKGROUND: Comparison of dynamic stiffness of foot joints was previously proposed to investigate pathologic situations with changes in the properties of muscle and passive structures. Samples must be controlled to reduce the variability within groups being compared, which may arise from different sources, such as gait speed or Foot Posture Index (FPI). METHODS: Variability in the measurement of the dynamic stiffness of ankle, midtarsal, and metatarsophalangeal joints was studied in a controlled sample of healthy men with normal FPI, and the effect of gait speed was analyzed. In experiment 1,…

Compression-rate-dependent nonlinear mechanics of normal and impaired porcine knee joints.

BACKGROUND: The knee joint performs mechanical functions with various loading and unloading processes. Past studies have focused on the kinematics and elastic response of the joint with less understanding of the rate-dependent load response associated with viscoelastic and poromechanical behaviors. METHODS: Forty-five fresh porcine knee joints were used in the present study to determine the loading-rate-dependent force-compression relationship, creep and relaxation of normal, dehydrated and meniscectomized joints. RESULTS: The mechanical tests of all normal intact joints showed similar stron…

Evaluating the deflection of dexterous continuum manipulators with unevenly distributed compliant joints.

Dexterous continuum manipulators (DCMs) offer great potential for increasing instrument reach in minimally-invasive surgical procedures. We previously designed and fabricated a tendon driven DCM with a large instrument channel and evenly distributed compliant joints for minimally-invasive skull base surgery and the treatment of osteolysis during hip revision surgery. The evenly distributed compliant joints, in some cases, may limit the reach of the DCM during lesion removal. In this paper, we propose a finite element analysis (FEA) method for optimizing the distribution of the compliant joints…
