Altered systemic levels of acute phase proteins in tuberculous lymphadenitis and modulation after treatment.

BACKGROUND: Pulmonary tuberculosis (PTB) is characterized by elevated levels of acute phase proteins (APPs), but their association with tuberculous lymphadenitis (TBL) is poorly studied. METHODS: We examined the systemic levels of APPs (alpha-2-macroglobulin [⍺-2MG], serum amyloid A [SAA], C-reactive protein [CRP] and haptoglobin [Hp]) in TBL, PTB, latent tuberculosis (LTB) and healthy controls (HC) at baseline and in TBL after the completion of anti-tuberculosis treatment (ATT). We have also examined the association of these proteins with lymph node (LN) size, culture grade and multiple vers…

Ectopic Enterobius Vermicularis Infestation; An Extremely Rare Cause Of Mesenteric Lymphadenopathy Mimicking Tuberculous Lymphadenitis.

Enterobius vermicularis (EV) is a pinworm which commonly resides in the lumen of the intestinal tract and lays eggs on the perianal skin. However, rarely the worm can infest various other sites in the body and cases with infestation of such ectopic sites have been reported in literature. Rare cases of mesenteric lymph node involvement have also been reported. We report a case in a young male who presented with signs and symptoms of acute appendicitis. During surgery, enlarged mesenteric lymph nodes were identified. Histological examination revealed adult worm in the appendiceal lumen. Histolog

CT Findings of Axillary Tuberculosis Lymphadenitis: A Case Detected by Breast Cancer Screening Examination.

We report the first description of CT findings of axillary tuberculous lymphadenitis confirmed by the pathological specimen. The breast cancer screening examination is one of the prime methods of detection of axillary tuberculous lymphadenitis. The most common site of axillary tuberculous lymphadenitis is the deep axilla. Screening mammography often fails to cover the whole axilla. The presence on the contrast-enhanced CT of unilateral multiple circumscribed dense nodes, some of which have large and dotted calcifications, might suggest tuberculous lymphadenitis in axillary region.

Esophageal anthracosis with tuberculous lymphadenitis confirmed on transesophageal endoscopic ultrasound-guided fine-needle aspiration.

Esophageal anthracosis with tuberculous lymphadenitis is a very rare disease. Almost all reported cases are diagnosed using multiple endoscopic biopsies or thoracic esophagectomy. The present case report describes a case of esophageal anthracosis with tuberculous lymphadenitis that was diagnosed using transesophageal endoscopic ultrasound-guided fine-needle aspiration (EUS-FNA) alone. After taking antituberculosis drugs, the patient's chest pain was relieved and she recovered. The patient achieved an excellent outcome following the use of less invasive methods than mediastinoscopy. If no esoph…

Isolated lymphadenitis due to Histoplasma capsulatum diagnosed by fine-needle aspiration biopsy and immunohistochemistry.

In the disseminated form of histoplasmosis, isolation and further identification of Histoplasma capsulatum can be performed by several methods, namely, bone marrow aspiration, blood culture, and liver biopsy. Lymph node disease usually is diagnosed by excisional biopsy. Although fungal stains can identify this fungus, detection of specific antigens by immunohistochemistry shows a higher specificity and sensitivity. This approach can use the cell block method when the material is not sent to fungal cultures or fresh staining.

Immunohistochemistry using a Mycobacterium tuberculosis complex specific antibody for improved diagnosis of tuberculous lymphadenitis.

The clinical and histological criteria used to diagnose lymphadenitis caused by Mycobacterium tuberculosis complex organisms have poor specificity. Acid-fast staining and culture has low sensitivity and specificity. We report a novel method for diagnosis of tuberculosis that uses immunohistochemistry to detect the secreted mycobacterial antigen MPT64 on formalin-fixed tissue biopsies. This antigen has not been detected in non-tuberculous mycobacteria. Polymerase chain reaction (PCR) for amplification of IS6110 from DNA obtained from the biopsies was used as a gold standard. Fifty-five cases of…

Primary multi-drug resistant tuberculosis presented as lymphadenitis in a patient without HIV infection.

Primary multi-drug resistant extrapulmonary tuberculosis is an uncommon form of the disease, but it seems that by increasing the number drug resistant tuberculosis around the world, the number of cases of primary multi-drug resistant tuberculosis with extrapulmonary presentation also is going to rise. In this report, we describe a 19-year old, HIV negative man with primary multi-drug resistant TB lymphadenitis, presented with cervical lymphadenopathy and sinus discharge at the site of involved lymph nodes. The Acid Fast Bacilli (AFB) smear of sputum was negative but the AFB smear of discharged…

HIV combination therapy: immune restitution causing cryptococcal lymphadenitis dramatically improved by anti-inflammatory therapy.

Two patients with AIDS developed microscopically verified focal cryptococcal lymphadenitis while treated with highly active anti-retroviral therapy for 8 and 15 months. Both were treated with fluconazole as a secondary prophylaxis for prior cryptococcal meningitis. Cryptococcus neoformans did not grow. Amphotericin was ineffective. Anti-inflammatory drugs had a dramatic effect.

Necrotizing granulomatous cervical lymphadenitis associated with B lymphocyte abnormalities and a positive PPD reaction.

Sarcoidosis is a diagnosis often made by exclusion. We present a case of what we believe is sarcoidosis presenting as cervical lymphadenopathy. Complicating issues in this patient included B lymphocyte abnormalities, and a positive PPD reaction. The patient continues to be steroid dependent. This case illustrates the difficulties in diagnosis and management of sarcoidosis.

Comparison of mycobacterial lymphadenitis among persons infected with human immunodeficiency virus and seronegative controls.

Lymphadenitis is a common extrapulmonary manifestation of mycobacterial disease in persons with human immunodeficiency virus (HIV) infection. We compared the clinical, mycobacterial, and diagnostic characteristics of mycobacterial adenitis in 11 HIV-seropositive and 29 HIV-seronegative patients. Ninety-three percent of the HIV-seronegative patients and 54% of the HIV-seropositive patients were foreign-born. In contrast to the HIV-seronegative patients, seropositive patients were more likely to be febrile and have negative purified protein derivative skin tests and abnormal chest roentgenograms…

Terminal ileitis, mesenteric lymphadenitis and appendicitis due to Yersinia pseudotuberculosis type VA: case report.

A case of terminal ileitis, mesenteric lymphadenitis and appendicitis is reported. Serological studies indicated infection with Yersinia pseudotuberculosis type VA. The patient's illness ran a chronic course necessitating resection of the terminal ileum. Histological examination of the appendix and a mesenteric lymph node in the acute stage revealed granulomas with central necrosis. This is the first human case in which the subtype VA has been identified.

Mediastinal lymphadenitis caused by Mycobacterium avium complex in children: a rare case of airway compression.

UNLABELLED: Airway compression caused by intrathoracic non-tuberculous mycobacteria (NTM) lymphadenitis is particularly uncommon in otherwise healthy children and remains poorly characterized. We aimed to describe the clinical, radiological, microbiological, and immunological features of a French pediatric cohort diagnosed with mediastinal Mycobacterium avium complex (MAC) lymphadenitis. We conducted a retrospective multicenter study that included children diagnosed with bacteriologically confirmed intrathoracic MAC lymphadenitis in France between 2009 and 2024. Patients with HIV-related immun…

Differences in ultrasonographic features and diagnostic value of biopsy between tuberculous lymphadenitis and reactive lymphadenopathy.

This study aimed to investigate the differences in ultrasonographic features between tuberculous lymphadenitis (TBL) and reactive lymphadenopathy and evaluate the diagnostic value of biopsy techniques. A retrospective analysis was conducted on 225 cases of superficial lymph node lesions confirmed by pathology or microbiology between February 2022 and February 2024, including 128 cases of TBL and 97 cases of reactive lymphadenopathy. Ultrasonographic characteristics were compared between the 2 groups, and diagnostic performance was evaluated using pathological results as the gold standard. A mu…

Beyond Suspected Appendicitis: Ultrasonography in a Child With Multifocal Lymphadenitis and Suspected Cat-Scratch Disease.

Ultrasonography is particularly useful in children because it can be repeated without ionizing radiation and can be adapted to the clinical question during the examination. A 10-year-old boy with right lower quadrant pain, right leg pain, and a limp was referred with suspected acute appendicitis. A comprehensive diagnostic ultrasound examination of the abdomen demonstrated a normal appendix and an enlarged lymph node corresponding to the site of right lower quadrant tenderness. During the examination, conversation with the patient and his mother disclosed household cat ownership, frequent expo
