We report on the successful treatment of a 32-year-old woman with condylar hyperplasia and severe mandibular crowding. In addition, her maxilla was canted to the right, her mandibular midline and chin point deviated to the left, and her maxillary canines were missing. The treatment plan included (1) aligning and leveling the teeth in both arches, (2) correcting overbite and overjet, (3) performing LeFort I osteotomy and bilateral split osteotomies, and (4) correcting the malocclusion postsurgically. The orthodontic treatment was performed with custom lingual braces and clear brackets, and virt

The present article illustrates a treatment option for managing the skeletal Class II malocclusion complicated by a deep bite and short face in an adult and emphasizes the limitations of such an approach and its potential complications. The patient had an inadequate tooth display upon smiling and a deep lower curve of Spee. Treatment consisted of a double jaw surgery with a maxillary three-piece LeFort to expand, downgraft, and advance the maxilla and a bilateral sagittal split osteotomy to advance the mandible. Treatment was staged so that the leveling of the mandibular arch was completed pos

The aim of the study was to present consecutive stages of the extended subtemporal approach (ESA). Seven simulations of ESA were performed on non-fixed human cadavers without any known pathologies in the head and neck. The consecutive stages of the procedure were documented with photographs and schemes. The starting point for ESA is osteotomy of the zygomatic arch and craniectomy including the greater wing of the sphenoid bone. Dislocation or removal of subtemporal fossa contents allows one to penetrate its inside and related structures. Additional widening of inspection allows osteotomy of th

Tumours of the midface and maxillary sinuses have been removed via multiple approaches. The most common approaches are those using incisions in the facial skin, especially in the case of malignant tumours. The Le Fort I procedure via an intraoral incision as described by Sailer in 1986 is a versatile alternative. Combined with a coronal approach and various osteotomies of the upper face it also allows removal of tumours extending into the orbits, the nasoethmoidal complex and the skull base. The versatility of the Le Fort I osteotomy as a surgical approach was analysed in 17 cases. This method

The retrieval of a displaced dental implant from the maxillary sinus-whether through an intraoral lateral window or a transnasal endoscopic route-invariably requires further surgical intervention and a subsequent healing period. For instance, in cases where a lateral window approach was not initially indicated, the accidental displacement of a dental implant into the maxillary sinus often necessitates the creation of such an access, which can lead to patient dissatisfaction. If removal through the lateral window proves unsuccessful, referral to an otolaryngologist may become necessary, and a t

OBJECTIVE: To show our experience in performing endoscopic-assisted maxillectomy (EAM), with the aim of facilitating delineation of tumour resection and improve the achievement of free tumour resection margins.
METHODS: Patients undergoing EAM between 2021 and 2024 were reviewed. During the endoscopic approach, the medial and lateral plates of the pterygoid were drilled, taking as reference the superior margin of the inferior turbinate.
RESULTS: Six patients underwent an EAM surgery. The maxillectomy was completed with an external transfacial approach in four patients and with a transoral ap

It is well known that osteotomy of the premaxilla is an effective surgical procedure for the correction of a displaced premaxilla in patients with bilateral cleft lip and palate. In cases with a posteriorly displaced premaxilla, it is not easy to move the premaxilla forward because of scarring of the palatal mucosal attachment, narrowing of the adjacent maxillary segments, and the stable fixation of this bone segment after its movement. This fixation is also important in cases without secondary bone grafting. We propose a new method that combines osteotomy and a method such as bone distraction

The purpose of this study was to assess skeletal stability and predictors of relapse in patients undergoing an isolated Le Fort I osteotomy. A retrospective cohort study of 92 subjects undergoing Le Fort I osteotomy for Class III malocclusion was implemented. Predictor variables were demographic and perioperative factors. The primary outcome variable was postoperative skeletal position with relapse defined as >2mm sagittal and/or vertical change at A-point on serial lateral cephalograms at immediate postoperative, 1 year, and latest follow-up time points. Mean advancement at A-point was 6.28±2

BACKGROUND: Pterygomaxillary separation is the most critical step of Le Fort I osteotomy. Nonideal separation of pterygomaxillary junction could be associated with many complications. The purpose of this study was to evaluate the anatomic basis of proper pterygoid osteotome in Le Fort I surgery.
MATERIALS AND METHODS: The cone beam computed tomography image of 177 patients were evaluated retrospectively. To design an ideal osteotomy, the angle and the length of the posterior curvature of the maxillary sinus wall were measured. For the classification of the posterior angle as low, medium, and

Many types of surgical osteotomies of the cranio-facial skeleton relay on adequate surgical field preparation. Placement and stabilization of nasotracheal tube during orthognathic surgery is very important. In most cases, long-lasting surgery includes various surgical maneuvers around patient's skull, which depend on accurate nasotracheal tube stabilization. Usage of adhesive plasters for heavy anesthetic tube connector placement and stability might result in tube instability and local disfigurement of nasal projection and visibility in the nasal and infraorbital areas. We present a novel tech

This article reports on a patient with severe maxillary and mandibular crowding treated by both expansion and extraction treatment. Two surgical procedures, surgically assisted palatal expansion and a mandibular midline osteotomy, were performed in sequence for skeletal expansion. A hybrid appliance, consisting of a Hyrax screw and 2 miniplates, was used in the mandible. Since expansion was not enough for relief of the crowding, 3 premolars were later extracted. The patient's final occlusion was satisfactory, but genioplasty may be desired in the future.

Secondary cleft lip-palate deformities frequently involve a combination of skeletal, cartilagenous, dental and muscular deformities. Correction of these deformities should be based on normalization of function by restoring normal anatomy as much as possible. Functional correction of the lip musculature is best done at the same time as surgical correction of the skeletal-dental deformities. In this article, we outline a fundamental technique for secondary lip-nose correction based on muscle reorientation and a concomitant Le Fort I maxillary osteotomy.

OBJECTIVE: Premaxillary distraction osteogenesis was introduced using intraoral devices to correct maxillary hypoplasia and lengthen the alveolar bone horizontally in a patient with unilateral cleft lip and palate.
METHODS: For premaxillary distraction osteogenesis, Le Fort I osteotomy was performed. Vertical osteotomy lines were located distally of the upper right canine and left first premolar to separate the anterior segment of the maxilla. After a 7-day latency period, distraction was allowed to continue for 20 days at a rate of 0.5 mm/d, followed by a 3-month consolidation period. After

Pterygomaxillary separation (PMS) is an important step in Le Fort I osteotomy procedure, without which complete mobilization of the maxilla cannot be achieved. The aim of this study was to evaluate PMS patterns and their relationship with the anatomic measurements in Le Fort I osteotomy. In this prospective observational study cone beam computed tomography (CBCT) was used to measure the anatomic variables of the pterygomaxillary junction (PMJ) region including thickness, width, the distance between the most concave point at the lateral surface of PMJ and the greater palatine foramen (C-GPF), a

The aims of this retrospective clinical study were to present our management protocol for the retrieval of impacted dental implants that have become displaced into the maxillary sinus cavity and to define the role of endoscopic sinus surgery in this setting. All 24 patients (25 implants) who underwent surgical retrieval of dental implants displaced into the maxillary sinus between 2012 and 2019 were included. Data on surgical interventions and complications were collected retrospectively. Eleven patients (46%) had chronic sinusitis associated with the migrated implant. All implants were succes

We present a case report of early Le Fort I osteotomy with maxillary advancement retained postoperatively by Class III elastics anchored on miniplates in a growing patient with complete unilateral cleft lip and palate (UCLP). A 14-year-old boy who underwent orthognathic surgery at the pubertal growth spurt was presented. During surgery, Bollard miniplates were installed in the posterior region of the maxilla and in the anterior region of the mandible. Class III elastics anchored on miniplates were used at night (8-10 h) starting 60 days after surgery. The force of the elastics progressively in

This clinical report describes a combined lateral-crestal approach to elevate the sinus floor when placing implants on a wide maxillary posterior ridge. The buccally enlarged osteotomy broadens the vision of practitioners and facilitates access of instruments in the sinus. Compared with the traditional lateral approach of sinus lift, the proposed technique offers a more conservative treatment modality. A clinical study with sufficient subjects and long-term follow-up is needed to validate the potential and limitations of the proposed technique.

BACKGROUND: Le Fort fractures with maxillary immobility are an uncommon presentation of facial trauma often associated with the disturbance of occlusion. Historically, cases involving high Le Fort fractures require a coronal extensive approach to mobilize the maxilla for occlusion restoration. Here, we review our institutional series of immobile Le Fort fractures and present their treatment approach, outcomes, classification, and then rationalize our treatment with a concept philosophy.
MATERIALS AND METHODS: We treated 12 consecutive patients with unilateral and bilateral immobile Le Fort fr
