Antisaccade eye movement abnormalities in Tourette syndrome: evidence for cortico-striatal network dysfunction?

Saccadic eye movements are rapid eye movements which act to redirect the eyes from one object of interest to another. Accurately and objectively measurable, their underlying neuroanatomical mechanisms have been extensively studied. The antisaccade task allows the study of the frontocortico-striatal network involved in the voluntary control of saccadic eye movements. In this task, the subject is instructed to inhibit a reflex eye movement towards a peripheral target light and, instead, to generate a movement in the equal and opposite direction. An error occurs when the subject fails to suppress

Dopamine receptor availability in Tourette's syndrome.

A large body of evidence suggests that abnormal dopaminergic activity is present in Gilles de la Tourette Syndrome (GTS). To investigate whether dopamine dysregulation involving the D2/D3 receptor occurs in GTS, we performed single slice dynamic single photon emission computed tomography (SPECT) with 123iodo-6-methoxybenzamide (123I-IBZM) in 15 GTS patients (eight unmedicated) and six healthy volunteers. After intravenous administration of 5 mCi (185 MBq) of 123I-IBZM, dynamic SPECT (5 minutes per slice) studies were performed at the level of the basal ganglia for 55 minutes. The mean activity

Microbleeds and Cerebral Amyloid Angiopathy in the Brains of People with Down Syndrome with Alzheimer's Disease.

Cerebrovascular pathology is a significant mediator in Alzheimer's disease (AD) in the general population. In people with Down syndrome (DS), the contribution of vascular pathology to dementia may play a similar role in age of onset and/or the rate of progression of AD. In the current study, we explored the extent of microbleeds (MBs) and the link between cerebral amyloid angiopathy (CAA) and MBs in the frontal cortex (FCTX) and occipital cortex (OCTX) in an autopsy series from individuals with DS (<40 years), DS with AD pathology (DSAD), sporadic AD, and control cases (2-83 years). Crowned dens syndrome, yet another rheumatic disease imposter.

OBJECTIVE: Crowned dens syndrome (CDS) is defined as acute cervical or occipital pain due to a local inflammatory reaction related to calcifications in the ligaments surrounding the odontoid process. Virtually, all previous descriptions of CDS have related to calcium pyrophosphate dehydrate (CPPD) arthropathy. METHODS: We prospectively identified a total of twenty-four consecutive inpatients with Crowned dens syndrome from January 2016 to December 2017 in our institution. RESULTS: All patients (age range 54 to 87 years, 67% females) presented with acute onset pain in the upper neck and/or oc

Medial temporal lobe atrophy in Down syndrome along the Alzheimer's disease continuum.

Medial temporal lobe structures are among the first areas impacted by neurofibrillary tangle pathology, making volumetric changes of these areas promising biomarkers for Alzheimer's disease. To date, little is known about the integrity of these regions in individuals with Down syndrome, a population that almost invariably develops Alzheimer's disease and thus offers a unique opportunity to determine the earliest structural changes related to the disease. We aimed to characterize the sequential involvement of medial temporal lobe structures with Alzheimer's disease progression, explore associat

Motor inhibition in patients with Gilles de la Tourette syndrome: functional activation patterns as revealed by EEG coherence.

There is considerable evidence that Gilles de la Tourette syndrome (TS) is due to frontal-striatal dysfunction. Here we determine whether adaptive cortical changes occur that might ameliorate the effects of this dysfunction. Specifically we test the hypothesis that increased interactions between selected cortical areas may help compensate through strengthened inhibition of inappropriate motor responses. To this end we recorded EEG in nine unmedicated patients with TS and nine age-matched healthy subjects during a variety of behavioural tasks related to motor inhibition. Functional connectivity

Deafness: an unusual onset of genetic Creutzfeldt-Jakob disease.

We describe a case of genetic Creutzfeldt-Jakob disease (CJD) with deafness at the onset. We report clinical features, 14-3-3 protein positivity, electroencephalography and brain stem auditory evoked potential abnormalities, and high signal on magnetic resonance imaging in basal ganglia and temporal cortex. Similarities with CJD Heidenhain variant are discussed.

A permanent pure amnestic syndrome of insidious onset related to Alzheimer's disease.

A 55-year-old patient experienced a pure amnestic syndrome of insidious onset that worsened progressively. Subsequently, her memory disorder stabilized and remained her only cognitive impairment for several years. She ultimately developed more widespread cognitive decline and terminal dementia. Postmortem examination 18 years after the onset revealed numerous senile plaques and neurofibrillary tangles consistent with Alzheimer's disease. A permanent pure amnestic syndrome of insidious onset may represent a further type of focal cerebral degeneration.

Convergence of TMS Sites and Lesion Locations Associated With Nicotine Addiction Improvement on a Common Brain Circuit.

OBJECTIVE: Transcranial magnetic stimulation (TMS) is cleared by the U.S. Food and Drug Administration for smoking cessation, yet the optimal neuroanatomical stimulation target remains unclear. Lesion locations causally linked to nicotine addiction remission have recently been used to identify a candidate brain circuit; however, this circuit has not yet been tested in relation to TMS outcomes. The authors evaluated whether TMS sites more connected to this lesion-derived brain circuit are associated with greater reductions in nicotine addiction. METHODS: Seventy-two participants with tobacco u

Occipital cerebral microbleeds as a predictor of intracranial hemorrhage in posterior reversible encephalopathy syndrome.

BACKGROUND AND OBJECTIVES: Intracranial hemorrhage (ICH), which often occurs in cases of posterior reversible encephalopathy syndrome (PRES), is regarded as a significant predictor of poor disease outcomes. While ICH and cerebral microbleeds (CMBs) can occur due to uncontrolled hypertension, the relationship between these conditions remains unclear. METHODS: This retrospective case-control study categorized PRES patients according to the presence of ICH on the day of symptom onset. The presence of CMBs or intracranial bleeding was assessed using gradient echo (GRE), susceptibility-weighted im

Shared and distinct aberrations in frontal-striatal system functional patterns among patients with irritable bowel syndrome and major depressive disorder.

BACKGROUND: Considering the high comorbidity, shared risk factors, and genetic pathways between irritable bowel syndrome (IBS) and major depressive disorder (MDD), we hypothesized that there would be both shared and disorder-specific alterations in brain function. METHODS: A total of 39 IBS patients, 39 MDD patients, and 40 healthy controls (HCs) were enrolled and matched for sex, age, and educational level. All subjects underwent resting-state functional MRI. The clinical variables of anxiety, depression, gastrointestinal symptoms and alexithymia were recorded. The 12 subregions of the stria

Cerebral perfusion abnormalities in patients with persistent postural-perceptual dizziness (PPPD): a SPECT study.

Persistent postural-perceptual dizziness (PPPD) is a recently defined syndrome with chronic dizziness interrupting daily life. Although the high levels of anxiety and functional changes in postural control strategy and multi-sensory information processing and integration may be underlying the pathophysiology, its neural mechanisms are poorly understood. The aim of this study was to examine the regional cerebral blood flow (rCBF) in patients with PPPD using single photon emission computed tomography (SPECT). A total of 25 patients with PPPD and 25 healthy controls participated in the study. All

Disrupted integrity of white matter in heroin-addicted subjects at different abstinent time.

PURPOSE: To investigate and compare the integrity of white matter in heroin-addicted and healthy control subjects at different abstinent time using diffusion tensor imaging. MATERIALS AND METHODS: We performed voxelwise analysis of fractional anisotropy (FA) and apparent diffusion coefficient (ADC) in 35 currently abstinent heroin abusers who were divided into long-term group (n = 17) and short-term group (n = 18) and 17 healthy volunteers. Measurements of FA and ADC of the identified regions (genu and splenium of corpus callosum, bilateral frontal lobe) were obtained from all subjects. RESU

Post operative supplementary motor area syndrome: clinical features and outcome.

The syndrome of the supplementary motor area (SMA) is not well recognized and its features can easily be confused with pyramidal weakness. The authors describe the SMA syndrome in six patients who underwent surgery for tumours located in the SMA, three in the dominant and three in the non-dominant hemispheres. All of them underwent complete resection of the anatomically described SMA, with partial (n = 4) or total resection (n = 2) of the tumour. In the postoperative period, all these patients exhibited reduction of spontaneous movements and difficulty in performing voluntary motor acts to com

Obsessive compulsive symptoms in Gilles de la Tourette syndrome and obsessive compulsive disorder: differences by diagnosis and family history.

The distribution of obsessive compulsive symptoms was compared in 16 individuals with primary obsessive compulsive disorder (OCD) and 16 individuals with Gilles de la Tourette syndrome (GTS) and associated obsessive compulsive behaviors (OCB). The two groups showed significant differences in the distribution of OC symptomatology. Furthermore, those OCD probands who shared a similar symptom profile with GTS individuals all had a positive family history of OCD. All of the other OCD probands were isolated cases. Implications of this finding on the etiology and pathogenesis of the two disorders ar

[The detection of hepatitis B virus DNA in nerve tissue: the procedural aspects].

A patient with HBsAg-positive chronic aggressive hepatitis and HBeAg and hepatitis B virus DNA (HBV DNA) developed fatal Creutzfeldt-Jakob disease. The investigation of cerebrospinal fluid and a temporal lobe brain tissue post mortem sample with radioactive probes of previously cloned HBV DNA showed the unquestionable presence of viral nucleotide sequences in the nervous tissue (about 9 viral genomes per cell). Although a pathogenetic role in the underlying neurologic disease cannot be attributed to HBV, our observation widens the spectrum of tissues where HBV has been detected, and supports t

Case 266.

History A 1-year-old boy was referred for cochlear implant assessment after he received a diagnosis of bilateral profound sensorineural hearing loss at neonatal hearing screening shortly after birth. The child was born at term via uneventful delivery, and there was no history of familial hearing loss or maternal illness. Tympanic membranes were normal, and hearing loss was confirmed with auditory brainstem testing, which showed no response from either ear. Hearing aids were provided from 3 months of age, but no behavioral responses were noted when these were worn. He was also noted to have som
