TMS-Derived Neurophysiological Measures of Corticospinal Excitability, Intracortical Excitability, and Interhemispheric Interactions Associated With Post-Stroke Motor Recovery: A Systematic Review.

Motor recovery after stroke is associated with changes in corticospinal integrity, intracortical excitability, and interhemispheric interactions. Transcranial magnetic stimulation (TMS) provides neurophysiological measures of these processes, but their relationship with motor recovery across randomized controlled trials remains uncertain. This systematic review synthesized evidence from randomized controlled trials investigating TMS-derived neurophysiological measures associated with motor recovery following TMS-based neuromodulation after stroke. PubMed/MEDLINE, Scopus, and Embase were search

Influence of intraoperative electromyographic changes and surgical extent on post-thyroidectomy esophageal motility: a prospective cohort study.

BACKGROUND: Thyroidectomy may lead to postoperative swallowing difficulties. This study aimed to evaluate the relationship between electromyographic (EMG) changes observed during intraoperative neuromonitoring (IONM) and swallowing problems following lobectomy, total thyroidectomy (TTx), and total thyroidectomy with central lymph node dissection (TTx+CLND), as assessed using high-resolution manometry (HRM). METHODS: This prospective study included 36 patients who underwent thyroid lobectomy (n=9), TTx (n=15), or TTx+CLND (n=12) by using IONM. All patients were questioned about dysphagia and u

Unexpected postoperative incidental recurrent laryngeal nerve palsy post total thyroidectomy after intraoperative nerve monitoring (overstimulation and fatigability): a case report.

BACKGROUND: Postoperative recurrent or permanent recurrent laryngeal nerve injury is one of the most serious complications in the field of thyroid surgery, in either benign or malignant thyroid disease, significantly affecting patients' quality of life. The importance of recurrent laryngeal nerve identification intraoperatively reduces the risk of injury. We report herein a young patient who underwent nerve monitoring in total thyroidectomy that led to recurrent laryngeal nerve injury. CASE PRESENTATION: We report a 30-year-old Arab male patient who presented to our clinic with a longstanding

A Randomized Controlled Trial of Intraoperative Nerve Monitoring in Resident-Performed Thyroidectomy.

OBJECTIVE: Intraoperative nerve monitoring (IONM) is widely used in thyroidectomy to aid recurrent laryngeal nerve (RLN) identification and preservation. This study assessed whether IONM improves RLN identification time and overall operative time in less experienced otorhinolaryngology residents performing thyroidectomies. METHODS: This randomized controlled trial was conducted at Siriraj Hospital, Bangkok, from January 2020 to February 2021. A total of 80 patients were randomly assigned to an IONM or non-IONM group. A total of 10 residents performed eight thyroidectomies each, using IONM in

Learning Curve for Using Intraoperative Neural Monitoring Technology of Thyroid Cancer.

We investigated the learning curve for using intraoperative neural monitoring technology in thyroid cancer, with a view to reducing recurrent laryngeal nerve injury complications. Radical or combined radical surgery for thyroid cancer was performed in 82 patients with thyroid cancer and 147 recurrent laryngeal nerves were dissected. Intraoperative neural monitoring technology was applied and the "four-step method" used to monitor recurrent laryngeal nerve function. When the intraoperative signal was attenuated by more than 50%, recurrent laryngeal nerve injury was diagnosed, and the point and

Exacerbation detection in multiple sclerosis by clinical and evoked potential techniques: a preliminary report.

The EP is well demonstrated as a useful indicator of subclinical demyelinating disease. Our study performed on patients well defined as MS and monitored closely for clinical exacerbations demonstrated an unexpectedly high occurrence of significant worsening in EP parameters. These changes occurred in the presence or absence of clinical worsening. The parameter changes may represent new demyelination or other pathologic or neurophysiologic changes in neuropathologically static lesions.

Noninvasive intracranial pressure monitoring methods: a critical review.

BACKGROUND: Intracranial pressure (ICP) monitoring has been used for decades in management of various neurological conditions. The gold standard for measuring ICP is a ventricular catheter connected to an external strain gauge, which is an invasive system associated with a number of complications. Despite its limitations, no noninvasive ICP monitoring (niICP) method fulfilling the technical requirements for replacing invasive techniques has yet been developed, not even in cases requiring only ICP monitoring without cerebrospinal fluid (CSF) drainage. OBJECTIVES: Here, we review the current me

The Association of Preoperative Cognitive Dysfunction to Common Intraoperative Electroencephalographic Parameters and Cerebral Hypoxia During Cardiac Surgery.

BACKGROUND: Older cardiac surgery patients have a higher prevalence of cognitive dysfunction and elevated risk of perioperative neurocognitive disorders (PND), both independently related to adverse postoperative outcomes. Neuromonitoring using electroencephalogram (EEG) and cerebral oximetry (CO) may predict PND. However, preoperative factors influencing intraoperative neurophysiological characteristics are not well understood. We conducted a study in a cardiac surgery cohort to better understand the relationship of preoperative cognitive dysfunction to intraoperative burst suppression (BS), s

Deformity angular distance ratio independently predicts intraoperative neuromonitoring alerts in spinal deformity correction.

BACKGROUND: Intraoperative neuromonitoring (IONM) alerts are critical concerns for surgeons performing spinal deformity corrective surgeries, as they indicate a heighteded risk of postoperative neurological deficits. Previous studies have demonstrated that patients with large Cobb angle or elevated deformity angular ratio (DAR) are at an increased risk of IONM alerts. However, spinal curves with similar Cobb angles and DARs may exhibit significantly different risks of IONM alerts during surgery. Current methods for evaluating spinal deformity fail to comprehensively and accurately reflect its

Intraoperative neuromonitoring with direct cortical stimulation motor-evoked potentials in supratentorial glioma surgeries with preoperative moderate-to-severe motor weakness.

PURPOSE: Intraoperative neuromonitoring (IONM) is essential for safe brain tumor surgeries. Little is known about how to assess damaged motor function. To preserve damaged motor function in these patients, we evaluated the results of direct cortical stimulation motor-evoked potential (D-MEP) monitoring in glioma surgeries performed in and near the primary motor cortex (PMC) and cortico-spinal tract (CST). METHODS: IONM using D-MEPs in patients with preoperative moderate-to-severe motor weakness was performed in 16 cases (14 patients). Before the tumor resection, a single six-contact subdural

The clinical use of dermatomal somatosensory evoked potentials in lumbosacral spinal stenosis.

DSEPs provide clinicians with a safe, noninvasive technique useful in determining which patients with anatomic spinal stenosis have the added component of neurogenic compromise. Based on physiologic principles, level-by-level prolongation of DSEP latencies, reduction of amplitude, asymmetry, or a complete absence of response is associated with dysfunction in that particular afferent neurologic pathway. This dysfunction does not correspond to the exact level of stenosis noted on MRI because the rootlets in the lumbar and sacral regions pass through multiple spinal segments as they course rostra

Muscular and cortical activation during dynamic and static balance in the elderly: A scoping review.

Falls due to balance impairment are a major cause of injury and disability in the elderly. The study of neurophysiological correlates during static and dynamic balance tasks is an emerging area of research that could lead to novel rehabilitation strategies and reduce fall risk. This review aims to highlight key concepts and identify gaps in the current knowledge of balance control in the elderly that could be addressed by relying on surface electromyographic (EMG) and electroencephalographic (EEG) recordings. The neurophysiological hypotheses underlying balance studies in the elderly as well a

Importance of intraoperative dynamic spinal cord mapping (DSCM) during complex spinal deformity surgery.

STUDY DESIGN: Case report and review of literature. OBJECTIVE: To illustrate the importance of "dynamic spinal cord mapping" (DSCM) in locating the site of neurophysiologic data loss during severe spinal deformity correction. During marked correction of severe spinal deformities, intra-operative neuromonitoring remains of paramount importance. This case illustrates the importance of a unique type of monitoring, particularly useful when obtaining correction at multiple sites. METHODS: A patient with symptomatic, progressive, severe kyphoscoliosis underwent multiple periapical posterior column

Electrodiagnosis of Amyotrophic Lateral Sclerosis: A Review of Existing Guidelines.

Amyotrophic lateral sclerosis (ALS) is characterized by degeneration of upper motor neurons in the motor cortex and lower motor neurons (LMN) in the brainstem and spinal cord, resulting in a progressive functional impairment. Neurophysiology is a diagnostic tool to detect dysfunction of upper motor neurons and LMN, even when the changes are subclinical. Electromyography is the standard neurophysiological investigation to detect LMN changes, which is essential to exclude mimicking disorders and attain early diagnosis. Recently, Awaji criteria was proposed to support ALS diagnosis, in these crit
