Vasovagal syncope: an update on the latest pharmacological therapies.

INTRODUCTION: Syncope is an abrupt loss of consciousness in response to reduced perfusion to the brain. Neurocardiogenic or vasovagal syncope results from a complex neurologic reflex, and treatments to prevent recurrence attempt to modulate aspects of that reflex. AREAS COVERED: Pharmacologic treatments for vasovagal syncope address the syncope reflex in multiple ways. Fludrocortisone and sodium chloride increase systemic fluid volume. Midodrine, β blockers and norepinephrine transport inhibitors modulate the sympathetic nervous system. Other treatments for syncope modulate other neurotransmi

Training using short-time features for OSA discrimination.

Heart rate variability (HRV) is one of the promising directions for a simple and noninvasive way for obstructive sleep apnea syndrome detection (OSA). The interaction between the sympathetic and parasympathetic systems on the HRV recordings, gives rise to several non-stationary components added to the signal. Aiming to improve the classifier accuracy for obstructive sleep apnoea detection, the use of more appropriated techniques for leading with non-stationarity and mixed dynamics, are needed. This work aims at searching a convenient training strategy of combining the feature set to be further

[Vasoactive transmitters in the coronary circulation].

Coronary resistance can be modified by perivascular, endothelial and humoral vasoactive substances. A network of perivascular nerve fibres is present in the adventitia and at the adventitial-medial border of human coronary arteries and veins, and immunocytochemical studies have provided evidence that several neuropeptides are localized in subpopulations of afferent and efferent nerve fibres supplying the human coronary vasculature. Neuropeptide Y seems to be co-localized with noradrenaline in adrenergic perivascular nerve fibres and vasoactive intestinal peptide co-localized with acetylcholine

Effects of systemic hypoxia on R-R interval and blood pressure variabilities in conscious rats.

The effects of systemic hypoxia with different levels of CO2 on R-R interval (RRI) and systolic blood pressure (SBP) variabilities were investigated in conscious rats. Wistar rats chronically instrumented for the measurement of blood pressure, electrocardiogram, and renal sympathetic nerve activity (RSNA) were exposed to hypocapnic (Hypo), isocapnic (Iso), and hypercapnic (Hyper) hypoxia. On another day, the rats were treated with atropine and exposed to the same type of hypoxia. Sinoaortic denervation (SAD)-treated rats were exposed to Iso and Hyper, and RRI and SBP variabilities before and d

Yohimbine increases submaxillary kallikrein release into the saliva in dogs: evidence for alpha 2-adrenoceptor-mediated inhibition of cholinergic pathways.

1. The effects of the alpha 2-adrenoceptor antagonist, yohimbine (0.5 mg kg-1, i.v.) on basal, sympathetic and parasympathetic stimulation-induced submaxillary kallikrein release were investigated in the anaesthetized dog. Kallikrein was measured by its kininogenase activity before and after trypsin activation which also allowed a study of the proportion of active to total enzyme. 2. Yohimbine induced a rapid, three fold increase in basal kallikrein release correlated with an increase in salivary flow rate which lasted for 60 min following injection. 3. Sectioning the chorda tympani did not af

Reversal effect of DM-9384 on scopolamine-induced acetylcholine depletion in certain regions of the mouse brain.

The effect of a new cognition enhancer, DM-9384, N-(2,6-dimethylphenyl)-2-(2-oxo-1-pyrrolidinyl) acetamide, on regional acetylcholine (ACh) levels and against scopolamine-induced ACh depletion was examined in mouse brain. In addition, the effects of DM-9384 were compared with those of oxiracetam, physostigmine and tacrine. Independent administration of DM-9384 (1, 3, 10 or 30 mg/kg, PO) or oxiracetam (10 or 50 mg/kg, PO) to mice had no effect on the ACh level in the hippocampus, frontal cortex, amygdala and striatum. Nevertheless, in all brain regions, pretreatment with DM-9384 significantly r

[Characteristics of the autonomic nervous system tests during group psychotherapy of patients with opioid dependence at the stage of induction of remission].

The time-course of changes in vegetative tests was studied in 47 men suffering from stage II opium dependence. The patients' age ranged from 18 to 28 years, with the dependence standing amounting to 1-10 years. Use was made of cardiointervalography to define the vegetative tone, as well as of the vagal and adrenaline tests to explore vegetative responsiveness. The orthostatic test was employed for studying vegetative maintenance of the activity. A group of patients (n = 22) was distinguished, with a positive attitude towards treatment and experienced in group psychotherapy, as was a group of p

Submandibular responses to stimulation of the sympathetic innervation following parasympathetic denervation in cats.

1. The effects of continuous stimulation of the ascending cervical sympathetic nerve were compared with those of intermittent stimulation in bursts, so arranged as to deliver the same total number of impulses, in cats under chloralose anaesthesia 3 weeks after excision of the chorda tympani. 2. Parasympathetic denervation of the gland in this way enhanced the vasodilator component of the vascular response during sympathetic stimulation in bursts. During continuous stimulation this was manifested as a reduced rise in submandibular vascular resistance (SVR). It also produced a profound increase

Factors affecting the secretion of submandibular salivary kallikrein in cats.

Glandular kallikrein has been assessed in submandibular saliva, homogenates and plasma by the fluorimetric substrate D-Val-Leu-Arg-7-amino-4-trifluoromethylcoumarin (AFC) and histochemically in tissue sections by the 4-methoxy-2-naphthylamide (MNA) analogue. Nerve stimulation was used to produce salivary secretion. Parasympathetic saliva contained low concentrations of kallikrein, independently of any circulating catecholamines from the adrenals. Sympathetic saliva contained very high concentrations of kallikrein; the amounts in individual drops rapidly reached a peak then declined gradually.

Nerve terminals and epithelial cell variety in the human lacrimal gland.

Pieces of tissue taken from three human lacrimal glands were examined electron microscopically. Secretory cells varied according to the electron density and structure of their secretion granules. Secretory cells were arbitrarily categorized as light, medium or dark based on their granule content. Acini were composed of two or all three categories of cells together with myoepithelial cells and lymphocytes. A minority ogic, sympathetic terminals. The appearance of most interstitial and all parenchymal fibres was consistent with that of cholinergic, parasympathetic terminals. Parenchymal terminal

[Horner's syndrome associated with Guillain-Barré syndrome].

Autonomic dysfunctions are classically described during the acute phase of Guillain-Barré syndrome (GBS). This involvement concerns visceral afferent, parasympathetic and/or sympathetic efferent fibers and is closely related to sudden deaths in the acute phase of the disease. We report a case of a patient suffering from a GBS associated with a transient Horner syndrome without extraocular muscle involvement. Pharmacological tests of the pupils pointed to an orthosympathetic pre-ganglionic disturbance. Neither clinical nor electrophysiological parameter suggested a broader involvement of the au

[The short-term effect of measured normobaric hypoxia on autonomic nervous system function in persons with hypo- and hyperacidic syndromes].

The functional activity of the vegetative nervous system was studied before and after the exposure to graded normobaric hypoxia having been realized as breathing with an artificial hypoxic mixture (AHM) during 10 to 40 minutes in subjects presenting with hypo- and hyperacidic syndromes. A relationship was established between activation of the sympathetic or parasympathetic regions and aggravation of the antecedent history (bad habits). It is suggested that a short-term AHM breathing be used as a test for preservation/failure of mechanisms of adaptation.

Depressed respiratory sinus arrhythmia: additional evidence for impairment of vagal activity in human hyperthyroidism.

The efferent activity of the parasympathetic component of the autonomic nervous system was evaluated by measuring the magnitude of the respiratory sinus arrhythmia in 3 female patients with hyperthyroidism (Basedow-Graves' disease) before and after treatment. The heart rate variations induced by the test increased in all patients after treatment of thyrotoxicosis (from 15 +/- 2.9 to 28.3 +/- 6.6 beats/min; mean +/- SEM) with propylthiouracil (600 mg/day). This result confirms our previous observations indicating an important and reversible impairment of the efferent vagal activity in human hyp

New Insights on Extrinsic Innervation of the Enteric Nervous System and Non-neuronal Cell Types That Influence Colon Function.

The enteric nervous system not only innervates the colon to execute various functions in a semi-autonomous manner but also receives neural input from three extrinsic sources, (1) vagal, (2) thoracolumbar (splanchnic), and (3) lumbosacral (pelvic) pathways, that permit bidirectional communication between the colon and central nervous system. Extrinsic pathways signal sensory input via afferent fibers, as well as motor autonomic output via parasympathetic or sympathetic efferent fibers, but the shared and unique roles for each pathway in executing sensory-motor control of colon function have not

Small-Conductance Ca2+-Activated K+ Channels and Their Regulation of Excitability of Autonomic Neurons.

Dyshomeostasis of the autonomic nervous system (ANS) drives chronic illnesses including hypertension, heart failure, diabetes mellitus, metabolic syndrome, Parkinson's disease, and traumatic brain injury. Abnormal neuronal excitability within central nervous system (CNS) nuclei is a major cause of ANS dysfunction. Small-conductance calcium-activated potassium (SK) channels govern excitability in central autonomic nuclei, sustaining neuronal excitation-inhibition balance via medium afterhyperpolarization potentials (mAHP)-mediated negative feedback. This review describes SK channels' molecular

Hypnotic Modulation of Autonomic Nervous System (ANS) Activity.

Hypnosis, a time-honored therapeutic approach, has gained widespread recognition for its effectiveness in addressing a range of psychological and somatic disorders. While its primary effects are mediated by central top-down mechanisms, hypnosis also exerts peripheral influence by modulating the autonomic nervous system (ANS). Psychophysiological measures, such as heart rate (HR) and its variability (HRV), electrodermal activity (EDA), respiratory rate (RR), and the analgesia nociceptive index (ANI), serve as reliable indicators of ANS activity. Studies have consistently demonstrated hypnosis'

Genetic testing for autonomic dysfunction or dysautonomias.

BACKGROUND AND AIM: The autonomic system is made of two divisions called the sympathetic and parasympathetic nervous systems and extends from the central to the peripheral nervous system for controlling homeostasis. Autonomic dysfunction, also known as dysautonomia, occurs when the nerves that control involuntary bodily functions do not work properly. The aim of this mini-review is to summarize all the syndromes characterized by dysautonomia and for which the associated gene is known. METHODS: We searched those syndromes in PubMed and OMIM database. RESULTS: We found 36 genetic syndromes cha…
