How to select healthy sperm for intracytoplasmic sperm injection in samples with high sperm DNA fragmentation?

The body of evidence supports the negative impact of increased sperm DNA fragmentation (SDF) on natural fertility as well as assisted reproduction conditions. High SDF has been correlated with low pregnancy and delivery rates following intrauterine insemination. Also, high SDF is accused of reducing the rates of fertilization, implantation, pregnancy, and live birth following in-vitro fertilization (IVF). Despite no impact of high SDF on fertilization or pregnancy rates following intracytoplasmic sperm injection (ICSI), it has been correlated with poor embryo quality and a higher risk of misca

The association between in vitro fertilization and intracytoplasmic sperm injection treatment and the risk of congenital heart defects.

OBJECTIVE: Assisted reproductive technology (ART), an effective treatment modality for infertility, is associated with a higher prevalence of congenital anomalies such as congenital heart defects (CHDs). The present study aimed to evaluate data linking CHDs in infants to pregnancies resulting from in vitro fertilization (IVF) and/or intracytoplasmic sperm injection (ICSI). METHODS: In this study, we conducted a systematic literature search on CHDs in infants following IVF/ICSI in Google Scholar, Embase, Scopus, MEDLINE, and PubMed databases from inception to February 2020. The search strategy

THE RISK OF HYPERCOAGULABILITY IN OVARIAN HYPERSTIMULATION SYNDROME.

Ovarian hyperstimulation syndrome (OHSS) is a rare and potentially life-threatening complication of infertility treatment occurring during either the luteal phase or early pregnancy. An increasing number of thromboembolic complications associated with the increased use of assisted reproductive techniques have been reported in the literature. Identification of the risk factors is crucial for prevention of thromboembolic events in OHSS patients. Alterations in the hemostatic system cause hypercoagulability in women affected by severe OHSS. Coexistence of inherited hypercoagulable conditions incr

Foundational reproduction programs for U.S. dairy herds.

Before the early 2000's, poor reproductive performance in U.S. dairy herds prevented commercial dairy farms from adopting assisted reproductive technologies. A reproduction revolution occurred in the U.S. dairy industry over the past 25 years during which the long-term decrease in reproductive performance dramatically reversed and began to increase. Although many factors are associated with increased reproductive performance (i.e., genetics, nutrition, management), the adoption of fertility programs by dairy farmers has driven much of this increase. Today, most U.S. dairy farms combine fertili

AI in Reproductive Medicine: Hope or Hype?

Artificial intelligence (AI) is increasingly used in reproductive endocrinology and infertility (REI), influencing nearly all aspects of assisted reproduction. Reported applications include ovarian stimulation, gamete and embryo assessment, endometrial evaluation, and prediction of IVF outcomes. These tools may reduce subjectivity, standardize grading, and provide individualized prognostic information. Predictive models for fertilization, implantation, miscarriage risk, and live birth have been developed, along with decision-support tools for embryo transfer and treatment continuation. However

Wombat reproduction (Marsupialia; Vombatidae): an update and future directions for the development of artificial breeding technology.

This review provides an update on what is currently known about wombat reproductive biology and reports on attempts made to manipulate and/or enhance wombat reproduction as part of the development of artificial reproductive technology (ART) in this taxon. Over the last decade, the logistical difficulties associated with monitoring a nocturnal and semi-fossorial species have largely been overcome, enabling new features of wombat physiology and behaviour to be elucidated. Despite this progress, captive propagation rates are still poor and there are areas of wombat reproductive biology that still

A review of the reproductive biology and breeding management of tapirs.

Tapirs (Tapirus sp.) have been studied extensively in the wild, yet little is known about their fundamental reproductive biology, information that is critical to establishing self-sustaining populations in captivity as a hedge against extinction. This paper reviews information on the reproductive biology of the 4 species of tapirs: Baird's (Tapirus bairdii), lowland (T terrestris), mountain (T pinchaque) and Malayan (T indicus). Both sexes reach puberty between 14 and 48 months of age. Behaviorally, tapirs display few overt signs of estrus, and external signs of pregnancy are not evident until

[Assisted oocyte activation: a new tool for severe male factor infertility treatment].

In severe male infertility, in vitro fertilization (IVF) with intra-cytoplasmic sperm injection (ICSI) represents the sole available therapeutic option. However this technique is not always successful in promoting fertilization, as some couples completely and repeatedly fail to obtain any embryo. In many cases, this failure can be attributed to a defective rise in intracellular calcium, which is required to achieve oocyte activation. Over the last twenty years, several laboratories dedicated to assisted reproduction technologies have been using a calcium ionophore to assist oocyte activation.

An additive opinion to the committee opinion of ASRM and SART on the use of preimplantation genetic testing for aneuploidy (PGT-A).

After over 20 years of progressively increasing clinical utilization of PGT-A (and its precursors), the American Society for Reproductive Medicine (ASRM) and its daughter society, the Society for Assisted Reproduction (SART), for the first time published a committee opinion clearly acknowledging that "the value of PGT-A as a routine screening test for patients undergoing in vitro fertilization (IVF) has not been demonstrated." This statement is timely and welcome but requires some additions and raises some new questions, among those why, if PGT-A in a general population does not improve IVF cy

Electronic witnessing in the medically assisted reproduction laboratory: insights and considerations after 10 years of use.

STUDY QUESTION: What have we learnt after 10 years of electronic witnessing? SUMMARY ANSWER: When applied correctly, an electronic witnessing system can replace manual witnessing in the medically assisted reproduction lab to prevent sample mix-up. WHAT IS KNOWN ALREADY: Electronic witnessing systems have been implemented to improve the correct identification, processing, and traceability of biological materials. When non-matching samples are simultaneously present in a single workstation, a mismatch event is generated to prevent sample mix-up. STUDY DESIGN, SIZE, DURATION: This evaluation i

Preconception vitamin D intake and obstetric outcomes in women using assisted reproductive technology: the Japan Environment and Children's Study.

BACKGROUND: In reproductive medicine, vitamin D (VitD) is of particular interest because its deficiency has been linked to various infertility issues. Thus, preconception care, including appropriate VitD supplementation, is essential, especially in women using assisted reproductive technology (ART). Despite the therapeutic benefits of VitD, adverse events due to a high daily intake may influence obstetric outcomes. However, the effects of either low or high preconception VitD intake on obstetric outcomes, including the outcomes in women who used ART, remain unclear. Therefore, the aim of this

Factors related to infertility in Brazil and their relationship with success rates after assisted reproduction treatment: an integrative review.

This integrative review evaluated the most commonly diagnosed causes of infertility in men and women in Brazil, as well as the medically assisted reproduction technologies regularly employed in these cases. We searched in four electronic databases (PubMed, including Medline; Scopus; Web of Science and LILACS), and two grey literature (Google Scholar and OpenGrey), guided by the focused question: "What are the main factors responsible for male and female infertility in Brazil, and what are its relationships with success rates after assisted reproduction treatment?". We included interventional o

The Risk of Retinopathy of Prematurity in the Infants following Assisted Reproductive Technology: A Meta-Analysis.

Currently, the use of assisted reproductive technology (ART) is increasing. Because of the poor prognosis of retinopathy of prematurity (ROP), the association between ART and the ROP has been explored in several studies, but the result was still inconclusive. Conducting a meta-analysis, we evaluated the risk of ROP in relation to the ART. Subgroup analysis as well as groups with different embryo numbers and different ROP stages was further analyzed. The PubMed, Embase, and Cochrane Library databases were searched for studies recording data about both the use of ART and ROP occurrence simultane

Pushing the boundaries: Uterine transplantation and the limits of reproductive autonomy.

Over the course of recent years, various scientific advances in the realm of reproduction have changed the reproductive landscape, enhancing women's procreative rights and the choices available to them. Uterus transplants (UTx) are the latest of such medical innovations aimed at restoring fertility in women suffering from absolute uterine factor infertility, providing them with the possibility not only of conceiving a genetically related child but also of gestating their own pregnancies. This paper critically examines the primacy of reproductive liberty in the context of uterus transplantation

[Novel aspects of human infertility: the role of the male factor].

In the article new aspects of the 'male factor' and its role in early stages of pregnancy are described. Among others, genetic and immunogenetic (KIR/KAR, HLA) factors are underlined as well as immunological ones (e.g. microchimerism). A significant part of this review is dedicated to infectious agents and semen inflammation as well as to the TORCH syndrome and chlamydiosis, concentrating on the male part, in which there are a lot of unclarified consequences. The problem of somatic diseases and general homeostasis of the male and its influence on pregnancy with particular emphasis on previous

[Identification and selection of completely immotile but viable spermatozoa for assisted reproductive technology].

Intracytoplasmic sperm injection (ICSI) remains the most effective method for severe male infertility patients to obtain their genetic offspring. A viable spermatozoon is the prerequisite for initiating fertilization in ICSI. Motility is the primary sign of sperm viability. However, how to select immotile but viable spermatozoa for ICSI on the day of ovum pick-up is critical for ICSI. This review focuses on the techniques for the identification and selection of immotile but viable spermatozoa for assisted reproductive technology.

Sperm Selection for ICSI: Do We Have a Winner?

In assisted reproductive technology (ART), the aim of sperm cells' preparation is to select competent spermatozoa with the highest fertilization potential and in this context, the intracytoplasmic sperm injection (ICSI) represents the most applied technique for fertilization. This makes the process of identifying the perfect spermatozoa extremely important. A number of methods have now been developed to mimic some of the natural selection processes that exist in the female reproductive tract. Although many studies have been conducted to identify the election technique, many doubts and disagree

A case of avoidable heterotopic pregnancy after single embryo transfer.

A rare case of a heterotopic pregnancy after single embryo and blastocyst transfer is presented. A couple suffered from idiopathic infertility and underwent assisted reproduction techniques in a university hospital. Intercourse on the day of HCG administration was the probable cause for an in vivo fertilization of an oocyte that was not collected during the oocyte retrieval. The patient accessed a regional hospital with a massive haemoperitoneum in the 11th week of pregnancy with the confusing information that only one embryo had been transferred during the assisted reproduction treatment.
