Phasic activities in the human body during REM sleep Part 2: Autonomic activity and general discussion.

Numerous brief events, known as phasic activities, have been observed in terms of rapid eye movements (REMs), muscle activity, and autonomic nervous system activity during REM sleep. This occurs against the backdrop of sustained muscle atonia and vagal tone. The first part of this review was dedicated to REMs and skeletal muscle phasic activities. The second part, here, provides a list of phasic autonomic changes in the human body. Considerably stronger evidence exists for pupillary, respiratory, cardiovascular, and reproductive phasic activity than for gastrointestinal and urinary phasic acti

Eye blinking characteristics as potential biomarkers in Isolated REM sleep behavior disorder during visuospatial attention tasks.

INTRODUCTION: Isolated REM sleep behavior disorder (iRBD) is a prodromal stage of alpha-synucleinopathies, such as Parkinson's disease (PD). Although altered eye blinking patterns are recognized as a clinical feature in PD, no studies have investigated its prodromal stage. This study aimed to investigate eye blinking characteristics in iRBD patients during a visuospatial attention task. METHODS: A total of 131 iRBD (aged 67.1 years) and 50 healthy controls (HCs; aged 66.2 years) participated. Patients who developed PD (PD-CV) or dementia with Lewy bodies (DLB-CV) were grouped as converters, w

Sex differences in the clinical characteristics of possible REM sleep behavior disorder: a community-based study.

BACKGROUND: Individuals with rapid eye movement sleep behavior disorder (RBD) are considered to be at high risk of neurodegenerative diseases. RBD has been reported to be a male-predominant parasomnia and remains to be characterized in different population settings. We investigated sex-related differences in the prevalence, comorbidities, clinical characteristics, and prodromal neurodegenerative symptoms of possible RBD (pRBD) in a community-based cohort. METHODS: The validated RBD Questionnaire-Hong Kong (RBDQ-HK) was used to screen for pRBD among 7314 Chinese older adults (2881 men and 4433

Longitudinal ultrasound assessment of vagus nerve morphology in isolated REM sleep behavior disorder and Parkinson's disease.

INTRODUCTION: Ultrasonography has shown that the vagus nerve cross-sectional area (VN-CSA) is reduced in patients with Parkinson's disease (PD). However, the status of the VN in prodromal PD is unclear. The objectives of this study were to assess: 1) the VN-CSA in healthy control subjects versus patients with isolated rapid eye movement sleep behavior disorder (IRBD) or PD; 2) the laterality of VN size; 3) whether the VN-CSA changes over time; and 4) whether the VN-CSA can serve as an indicator of the progression of neurodegeneration in IRBD or PD. METHODS: We performed ultrasonography to det

Motor features that distinguish isolated REM sleep behavior disorder patients from healthy controls: A systematic review.

Individuals with isolated REM sleep behavior disorder (iRBD) are at high risk of developing α-synucleinopathies, particularly Parkinson's disease (PD) and dementia with Lewy bodies (DLB). With the development of potential neuroprotective treatments for synucleinopathies, including PD, identifying clinical features that can allow for tracking subtle changes in prodromal disease and thereby monitoring risk of phenoconversion in iRBD is paramount. Subtle motor deficits have been suggested to be present in iRBD, making them potentially important clinical markers for predicting future phenoconversi

Exploring the relation between REM sleep behavior disorder onset and striatal dopaminergic dysfunction in Parkinson's Disease.

BACKGROUND: The α-Synuclein Origin and Connectome (SOC) model recently proposed two different Parkinson's Disease (PD) phenotypes clinically based on the relationship between REM sleep behavior disorder (RBD) and motor symptoms' onset: a "body first" phenotype and a "brain first" phenotype in which RBD precedes or may follow the motor onset, respectively. A higher burden of non-motor symptoms as well as a more symmetrical clinical presentation have also been predicted in the body-first phenotype. This point has been poorly assessed through semi-quantitative striatal dopaminergic functional ima

Cardiac autonomic function in REM-related obstructive sleep apnoea: insights from nocturnal heart rate variability profiles.

PURPOSE: In light of the reported association between REM-related obstructive sleep apnoea (OSA) and heightened cardiovascular risk, this study aims to compare cardiac autonomic function in patients with REM-OSA and OSA independent of sleep stage. We hypothesized that REM-OSA patients would exhibit higher sympathetic cardiac modulation based on heart rate variability (HRV) profiles. METHODS: HRV was compared between the OSA group (AHI ≥ 5 events/h, n = 252) and the REM-OSA group (AHI ≥ 5 events/h, AHIREM:AHINREM ≥ 2, n = 137). Time- and frequency-domain measures of HRV were analysed during N2

Causal associations between modifiable risk factors and isolated REM sleep behavior disorder: a mendelian randomization study.

OBJECTIVES: This Mendelian randomization (MR) study identified modifiable risk factors for isolated rapid eye movement sleep behavior disorder (iRBD). METHODS: Genome-wide association study (GWAS) datasets for 29 modifiable risk factors for iRBD in discovery and replication stages were used. GWAS data for iRBD cases were obtained from the International RBD Study Group. The inverse variance weighted (IVW) method was primarily employed to explore causality, with supplementary analyses used to verify the robustness of IVW findings. Co-localization analysis further substantiated causal associatio

Central Apneic Event Prevalence in REM and NREM Sleep in OSA Patients: A Retrospective, Exploratory Study.

Patients with sleep-disordered breathing show a combination of different respiratory events (central, obstructive, mixed), with one type being predominant. We observed a reduced prevalence of central apneic events (CAEs) during REM sleep compared to NREM sleep in patients with predominant obstructive sleep apnea (OSA). The aim of this retrospective, exploratory study was to describe this finding and to suggest pathophysiological explanations. The polysomnography (PSG) data of 141 OSA patients were assessed for the prevalence of CAEs during REM and NREM sleep. On the basis of the apnea-hypopnea

Personality profile and its association with conversion to neurodegenerative disorders in idiopathic REM sleep behavior disorder.

Patients with Parkinson's disease (PD) were described less extraverted and more neurotic. It remained unclear whether similar personality traits could be found in idiopathic rapid eye movement sleep behavior disorder (iRBD), a prodromal stage of PD, and could predict phenoconversion to neurodegenerative disorders. We aimed to investigate the personality profile and its association with future neurodegenerative phenoconversion in iRBD patients. One hundred and eighty-five video-polysomnography confirmed iRBD patients and 91 age- and sex-matched controls underwent personality assessment using th

Genome-wide association study of REM sleep behavior disorder identifies polygenic risk and brain expression effects.

Rapid-eye movement (REM) sleep behavior disorder (RBD), enactment of dreams during REM sleep, is an early clinical symptom of alpha-synucleinopathies and defines a more severe subtype. The genetic background of RBD and its underlying mechanisms are not well understood. Here, we perform a genome-wide association study of RBD, identifying five RBD risk loci near SNCA, GBA, TMEM175, INPP5F, and SCARB2. Expression analyses highlight SNCA-AS1 and potentially SCARB2 differential expression in different brain regions in RBD, with SNCA-AS1 further supported by colocalization analyses. Polygenic risk s

RBDtector: an open-source software to detect REM sleep without atonia according to visual scoring criteria.

REM sleep without atonia (RSWA) is a key feature for the diagnosis of rapid eye movement (REM) sleep behaviour disorder (RBD). We introduce RBDtector, a novel open-source software to score RSWA according to established SINBAR visual scoring criteria. We assessed muscle activity of the mentalis, flexor digitorum superficialis (FDS), and anterior tibialis (AT) muscles. RSWA was scored manually as tonic, phasic, and any activity by human scorers as well as using RBDtector in 20 subjects. Subsequently, 174 subjects (72 without RBD and 102 with RBD) were analysed with RBDtector to show the algorith

APOE ɛ4 and REM Sleep Behavior Disorder as Risk Factors for Sundown Syndrome in Alzheimer's Disease.

BACKGROUND: Sundown syndrome (SS) in patients with Alzheimer's disease (AD) is characterized by aggravation of behavioral problems at sunset. Disturbance of the circadian rhythm, a possible cause of SS, also facilitates amyloidopathy and reduces sleep quality. However, the associations of SS with amyloidopathy and sleep quality remain unclear. OBJECTIVE: To investigate the prevalence of SS in patients with AD, the association between SS and APOEɛ4 carrier, representing an enhanced amyloid pathology, and the relationship between SS and sleep quality in AD. METHODS: We included 104 patients wi

Incorporation of recent waking-life experiences in dreams correlates with frontal theta activity in REM sleep.

Rapid eye movement (REM) sleep and its main oscillatory feature, frontal theta, have been related to the processing of recent emotional memories. As memories constitute much of the source material for our dreams, we explored the link between REM frontal theta and the memory sources of dreaming, so as to elucidate the brain activities behind the formation of dream content. Twenty participants were woken for dream reports in REM and slow wave sleep (SWS) while monitored using electroencephalography. Eighteen participants reported at least one REM dream and 14 at least one SWS dream, and they, an

Microstructural Changes in Patients With Parkinson's Disease Comorbid With REM Sleep Behaviour Disorder and Depressive Symptoms.

The diagnosis of Parkinson's disease (PD) is currently anchored on clinical motor symptoms, which appear more than 20 years after initiation of the neurotoxicity. Extra-nigral involvement in the onset of PD with probable nonmotor manifestations before the development of motor signs, lead us to the preclinical (asymptomatic) or prodromal stages of the disease (various nonmotor or subtle motor signs). REM sleep behavior disorder (RBD) and depression are established prodromal clinical markers of PD and predict worse motor and cognitive outcomes. Nevertheless, taken by themselves, these markers ar

Validation of non-REM sleep stage decoding from resting state fMRI using linear support vector machines.

A growing body of literature suggests that changes in consciousness are reflected in specific connectivity patterns of the brain as obtained from resting state fMRI (rs-fMRI). As simultaneous electroencephalography (EEG) is often unavailable, decoding of potentially confounding sleep patterns from rs-fMRI itself might be useful and improve data interpretation. Linear support vector machine classifiers were trained on combined rs-fMRI/EEG recordings from 25 subjects to separate wakefulness (S0) from non-rapid eye movement (NREM) sleep stages 1 (S1), 2 (S2), slow wave sleep (SW) and all three sl

REM and NREM sleep-state distribution of respiratory events in habitually snoring school-aged community children.

BACKGROUND: Studies ascribe different functions to rapid eye movement (REM) and non-rapid eye movement (NREM) sleep, such that their disruption could result in discrepant clinical outcomes. Although sleep architecture is globally preserved in children with obstructive sleep apnoea (OSA), it is considered to be an REM sleep REMS disorder. Furthermore, body position during sleep affects the occurrence of respiratory events, while the presence of obesity has been claimed to affect sleep-state distribution of respiratory disturbance. METHODS: To explore the distribution of respiratory events duri
