ETI corrector therapy suppresses mitochondrial respiration independently of CFTR mutational status.

The relationship between CFTR mutational status and cellular energy metabolism remains unresolved. Using continuous long-term respirometry via the Resipher platform, we demonstrate that the Class II CFTR misfolding mutations ΔF508, G85E, and P67L all significantly increase cellular respiration in isogenic Fisher rat thyroid (FRT) and human bronchial epithelial (HBE) cells, consistent with the constitutive energy demands of misfolded-protein quality control. In contrast, Class I truncating mutations (G542X, W1282X), which produce no misfolded protein and no functional ion channels, reduced cell

Pregnancy-related effect on elexacaftor, tezacaftor and ivacaftor pharmacokinetics in women with cystic fibrosis.

AIM: The number of pregnancies among women with cystic fibrosis (wwCF) has steadily increased over the past decade. However, the pharmacokinetics (PK) of elexacaftor-tezacaftor-ivacaftor (ETI) during gestation remains uncharacterized, despite its widespread use in this population. To date, no clinical trials have investigated the effectiveness, safety or PK profiles of ETI during pregnancy. Given the significant physiological changes that occur throughout gestation, which can alter drug PK, characterizing how pregnancy impacts ETI exposure represents a key step towards informing appropriate do

From Genomic Insight to Clinical Impact: Dramatic Response in RET Fusion-positive Pancreatic Cancer.

BACKGROUND/AIM: Pancreatic cancer remains one of the deadliest solid malignancies despite advances in systemic treatment. Outcomes with current therapies remain modest, with a 5-year overall survival rate of no more than 20%. Although rare, oncogenic RET fusions represent a clinically actionable alteration and may predict substantial benefit from selective RET inhibition. CASE REPORT: A 48-year-old woman with pancreatic ductal adenocarcinoma developed liver metastases after first-line modified FOLFIRINOX. Comprehensive genomic profiling of the initial biopsy identified an NCOA4-RET fusion inv

Virtual screening of novel alkaloids as potent inhibitors for G2032R-mutant ROS1 kinase in non-small-cell lung cancer.

The Gly2032Arg (G2032R) point mutation in proto-oncogene tyrosine-protein kinase 1 (ROS1) is one of the predominant factors of drug resistance to targeted therapies in patients with ROS1 fusion-positive non-small-cell lung cancer (NSCLC). This study aimed to identify novel inhibitors from a library of alkaloids (447 compounds) using computational approaches. Molecular docking-based virtual screening was performed to identify promising compounds, followed by ADMET property prediction and molecular dynamics simulations to assess their safety and stability. The top compounds identified were yibei

Prospective multicenter study of ctDNA versus tumor tissue guiding FGFR-targeted therapy in metastatic urothelial cancer.

Fibroblast growth factor receptor (FGFR) alterations are targetable in metastatic urothelial carcinoma (mUC). Identifying FGFR alterations currently requires tissue testing, which is limited by sample availability and cancer heterogeneity. Plasma circulating tumor DNA (ctDNA) offers a complementary testing strategy, but the added value of ctDNA relative to conventional FGFR alteration assessment remains unclear. Here, in a prospective, multicentre study, we show that ctDNA testing has high concordance with tissue FGFR testing and identifies additional actionable FGFR alterations. We profile pl

Effect of Once-Daily Macitentan 75 mg on the Pharmacokinetics of Sildenafil, Riociguat, or Rosuvastatin in Healthy Male Participants.

Macitentan is an oral, dual ETA/ETB endothelin receptor antagonist, currently approved in adults for the treatment of pulmonary arterial hypertension (PAH) at a once-daily dose of 10 mg. Pre-clinical and clinical data suggest that greater ETB receptor inhibition may result in greater efficacy. This hypothesis is being tested in the Phase 3 UNISUS study (NCT04273945), comparing once-daily macitentan 75 mg versus macitentan 10 mg. The present Phase 1 study (NCT04211272) evaluated the pharmacokinetics and safety of concomitant administration of once-daily macitentan 75 mg at steady state with sub

Fixed-duration pirtobrutinib plus venetoclax-rituximab versus venetoclax-rituximab for patients with previously treated chronic lymphocytic leukaemia or small lymphocytic lymphoma (BRUIN CLL-322): an open-label, multicentre, randomised, controlled, phase 3 trial.

BACKGROUND: Venetoclax-rituximab (VR) following covalent Bruton tyrosine kinase (BTK) inhibitor therapy is the fixed-duration standard of care for patients with chronic lymphocytic leukaemia (including small lymphocytic lymphoma). Pirtobrutinib, a non-covalent BTK inhibitor, is approved for use after treatment with a covalent BTK inhibitor as a continuous therapy option. We aimed to evaluate the addition of pirtobrutinib to VR as a fixed-duration regimen in patients with relapsed or refractory chronic lymphocytic leukaemia. Refractory macrophage activation syndrome in adult-onset Still's disease: response to interferon-γ blockade and JAK inhibition.

A female patient in her mid-20s presented with fevers, rash and polyarthralgia and was ultimately diagnosed with macrophage activation syndrome (MAS), a widely recognised form of secondary haemophagocytic lymphohistiocytosis in the setting of autoinflammatory disease such as adult-onset Still's disease. Extensive infectious and malignancy workup was negative. Her clinical course was complicated by persistently active disease despite initial immunomodulator therapies, including corticosteroids and anti-interleukin-1 therapy (anakinra). Further escalation of therapy with tocilizumab and etoposid

Straightforward synthesis and in silico evaluation of pyrazolylthiazolidinone derivatives as prospective antiproliferative agents.

Cancer remains a leading cause of mortality, underscoring the need for novel agents to improve therapeutic outcomes and overcome drug resistance. Guided by the known antitumor potential of thiazolidinones, a series of arylidene-thiazolidinone derivatives was synthesized via a one-pot cyclocondensation of pyrazolyl-thiosemicarbazone, with chloroacetic acid and aromatic aldehydes. In vitro antiproliferative screening against HCT-116 (colon) and A549 (lung) cancer cell lines identified compounds 3c (2,4-dihydroxybenzylidene) and 3a (4-dimethylaminobenzylidene) as the most potent, with stronger ac

Discovery of Novel Bifunctional Agents as Potent TRK Inhibitors and Degraders against xDFG Mutation Resistance.

Despite the clinical success of tropomyosin receptor kinases (TRKs) inhibitors in NTRK fusion-positive cancers, prolonged administration has resulted in acquired resistance, particularly xDFG mutations, for which no approved therapies are available. Herein, we first presented a series of bifunctional agents as potent TRK inhibitors and degraders featuring a novel 5-amino-4-carbamoylpyrazole scaffold. The representative compounds 19 and 20 demonstrated potent antiproliferative activities against Ba/F3-LMNA-NTRK1G667C cells with IC50 values of 0.29 and 2.48 nM, respectively, and induced pronounc
