Renin-aldosterone profiling was used to classify patients with hypertension: 243 patients with essential hypertension were classified by renin-urinary sodium indexing; 107 were reclassified by response to administration of furosemide and intravenous saline; 45 were further classified by response to a low-sodium diet. Hypertension was found in 854 (10.4%) of 8228 patients (3657 females, 4571 males) seen in our outpatient clinic during 1975. CONTEXT: Adrenal venous sampling (AVS) is recommended for subtyping primary aldosteronism (PA); however, it is unreliable when apparent bilateral aldosterone suppression (ABAS) occurs, defined as aldosterone/cortisol ratio in the adrenal vein being bilaterally lower than that in the inferior vena cava.
