[Surgical and Percutaneous Dilatational Tracheostomy - Technique and Pitfalls].

A tracheostomy is usually necessary for long-term mechanical ventilation or complicated weaning. Other indications include swallowing disorders with recurrent aspiration in neuromuscular disease and high-grade subglottic stenosis. The tracheostomy can be performed as a percutaneous dilatational tracheostomy or as a surgical tracheostomy. The complication rate is low, and intraoperative complications are differentiated from early and late postoperative complications. This article aims to present the indications, the techniques and complications of percutaneous dilatational and surgical tracheos

Lissencephaly: presentation of a clinical case of LIS 1 with a diagnosis confirmed by MLPA method and indications for rehabilitation treatment in childhood.

Lissencephaly is a very rare clinical condition that affects the formation of the brain and causes severe psychomotor retardation, convulsions and muscular spasticity or hypotonia, often also associated with respiratory problems, facial dysmorphisms, abnormalities of the fingers and toes and difficulty swallowing resulting in reduced life expectancy. The clinical case described in the following article demonstrates the diagnostic process and rehabilitation treatment of a patient through a narrative review of the scientific literature and the presentation of this condition in order to provide i

Localized Amyloidosis of Bilateral Palatine Tonsils Mimicking Tonsillolithiasis.

A rare disease called localized tonsillar amyloidosis can cause serious problems with airway patency in severe cases. This was the case with an elderly man who experienced difficulty breathing and swallowing due to enlarged palatine tonsils. The physical and imaging findings suggested tonsillolithiasis, and the patient underwent bilateral tonsillectomy. The diagnosis of amyloidosis was confirmed with histopathological examination using Congo red staining. Surgical intervention and careful follow-up care can be effective in these cases, which generally have a good prognosis.

Floor of mouth prosthesis with removable depressible tongue: A clinical report.

The rehabilitation of a patient with a total glossectomy with a prosthetic device is challenging and depends on the individual patient. Ablation of a tongue tumor leaves defects that can be either surgically reconstructed or replaced by a prosthesis to help recreate normalcy and balance in the oral cavity. This clinical report describes a prosthetic design to rehabilitate a patient after a total glossectomy. This approach successfully recreated the glossal surface with a soft, hollow, depressible structure that emulates the tongue during speech and a solid structure to facilitate swallowing. T

Cardiovocal Syndrome Secondary to Thoracic Aortic Aneurysm: An Old Sign Revisited.

A 56-year-old male, chronic smoker, presented with persistent dry cough, hoarseness of voice and difficulty in swallowing. Indirect laryngoscopy revealed left vocal cord paralysis. Further evaluation revealed eccentric saccular aneurysms arising from the aortic arch and descending thoracic aorta, compressing the trachea, esophagus, left atrium and left recurrent laryngeal nerve. The patient was diagnosed with Ortner's syndrome which is an uncommon presentation of aortic aneurysm. He awaits an endovascular aorta aneurysm repair.

Case report of extreme gastric distention and perforation with pathologic Sarcina ventriculi colonization and Rett syndrome.

Here we describe a case of a 15-year-old child with Rett syndrome who presented with extreme gastric distension and fatal perforation in the setting of long-standing aerophagia and pathologic colonization with Sarcina ventriculi, a rare bacteria implicated in gastric perforation. This is the first report of gastric perforation associated with colonization by Sarcina in a patient with pathologic aerophagia. Gastric colonization with Sarcina should be considered in intellectually disabled children with pathologic air swallowing who present with severe gastric dilation and/or perforation.

Vascular anomalies of the head and neck.

The head and neck are the most common site of involvement for vascular tumors and malformations, with more than half of all vascular anomalies seen in this region. Lesions in this location can cause significant disfigurement and can be associated with airway obstruction, impairment in vision or hearing, swallowing disorders and hemorrhage. Accurate diagnosis is critical in determining treatment, and interdisciplinary care is essential for optimal management. We review clinical and imaging features that are key to establishing the correct diagnosis, and review treatment modalities, with emphasi

Erythema exsudativum multiforme (EEM) is a sudden inflammation of the skin and/or mucous membranes (minor or major form). It typically affects healthy, young, male adults. The lips are affected first or sometimes exclusively. The buccal mucosa, the buccal gingiva and the tongue can also be affected. Less frequently, intraoral lesions are also found on the hard and soft palate and the floor of the mouth. Patients report severe pain when eating, swallowing and speaking. The clinical picture is usually sufficient diagnostic evidence and no histopathological examination is required.

Epiglottoplasty technique in endoscopic partial laryngectomy.

The main advantage of endoscopic laser surgery for laryngeal cancer is to allow tumour resection, while limiting functional sequelae, thereby improving the postoperative course. In this type of surgery, the epiglottis is often partially resected, leaving a raw zone without any reconstruction. The surgical technique described here involves endoscopic reconstruction of the epiglottis after partial resection. The sectioned edge of the epiglottis is sutured to the base of the tongue to create a neoepiglottis and to reconstruct the vallecula, thus resembling preoperative anatomy, allowing improveme

Gastric Visualization in Dacryoscintigraphy: An Unequivocal Sign of Patent Nasolacrimal Duct.

We reported a 6-year-old girl with a history of left-eye epiphora since 2 years ago who was referred to our nuclear medicine center for dacryoscintigraphy imaging. The scan showed obstruction in the proximal portion of the nasolacrimal duct in the left eye. Imaging also showed patent nasolacrimal duct of the right eye. Tc pertechnetate activity was seen in the esophagus and stomach owing to swallowing of the excreted tracer in the nasal cavity. This was an unequivocal sign of patent lacrimal apparatus.

Feeding Infants with Cleft and the Postoperative Cleft Management.

Cleft lip and palate malformations affect the ability of infants to adequately feed. Choosing the appropriate feeding technique must allow functioning of the suck-swallow-breathing mechanism and development of oral-motor function. Feeding in the postsurgical setting should be returned to normal as soon as possible to maintain adequate nutritional intake and good wound healing, an essential component to cleft repair. This article outlines the complex mechanism of wound healing, gives options to augment the process of aesthetic scar healing, and presents feeding modifications.

Oropharyngeal Dysphagia in children: mechanism, source, and management.

Oropharyngeal dysphagia (OPD) is a challenging and relatively common condition in children. Both developmentally normal and delayed children may be affected. The etiology of OPD is frequently multifactorial with neurologic, inflammatory, and anatomic conditions contributing to discoordination of the pharyngeal phase of swallowing. Depending on the severity and source, OPD may persist for several years with significant burden to a patient's health and family. This article details current understanding of the mechanism and potential sources of OPD in children while providing an algorithm for man

Brittleness of diabetes due to achalasia cardia managed successfully by multiple innovative strategies.

Achalasia cardia is an infrequent disorder of esophageal dysmotility that has failure of the lower end of the esophagus to relax with swallowing as hallmark abnormality. Diabetes mellitus, on the other hand, can afflict the motor activity of gastrointestinal tract by causing autonomic neuropathy. Combination of these diseases can be very distressing to a patient. We present a 45-year-old lady co-affected with both these disorders who presented with severe hypoglycemia and was managed successfully using the multiple strategies to treat achalasia and diabetes.

Giant leiomyoma of the oesophagus with eosinophilic infiltration.

The most common benign tumour of the oesophagus is leiomyoma. Haemopoietic elements rarely infiltrate oesophageal leiomyoma. We report the case of a 24-year-old man with a long history of intermittent dysphagia. Endoscopy revealed external compression with normal oesophageal mucosa. A barium swallow study showed a defect in the oesophageal wall and a narrow oesophageal lumen. A CT scan of the chest confirmed a mass in the mid-oesophagus. A tumour was excised and the histology report revealed leiomyoma infiltrated with eosinophils, which is a rare variant.

[Soft palate reconstruction].

Soft palate reconstruction is complex. It has to restore swallowing and speech replacing the defect by dynamic and sensitive tissues. The means are multiple from direct suture to free flaps. Local flaps have the advantage to bring a mucosa animated by thin muscles, like in sphincteroplasty. Free flaps allow reconstruction of large defects, but they need some local adaptation (association with a local flap, adherence, suspension with tendon…) to improve the functional result. Indications depend on defect's size and local condition especially radiotherapy history.

Laryngopharyngeal reflux and GERD.

In patients with laryngopharygeal reflux (LPR), gastric contents exhibit retrograde flow into the upper aero-digestive tract, causing extraesophageal symptoms including chronic cough, hoarseness, indigestion, difficulty swallowing, globus pharyngis, and asthma. The following on laryngopharyngeal reflux includes commentaries on the use of patient-completed questionaires and anti-human pepsin antibodies and other non-invasive tests in diagnosis; the role of pepsin and acid in the etiologies of laryngeal cancers; and the application of proton pump inhibitor (PPI) therapy for the treatment of LPR.

Cysticercosis of breast.

Human cysticercosis, a parasitic infection caused by cysticercus cellulosae. This results from ova being swallowed or getting asses to human stomach by regurgitation from own adult worm. The larvae are liberated in stomach, penetrate the intestinal mucosa and carried to many parts of the body where they form cysterci, 0.5-1 cm cyst that contain the head of young worm. They do not grow further or migrate. The common sites are skeletal muscle, subcutaneous tissue, brain and eye. Breast is an uncommon site.

Nutrition assessment and management in amyotrophic lateral sclerosis.

In recent years nutrition assessment and management in amyotrophic lateral sclerosis (ALS) have drawn increased attention. Frequent evaluation of nutrition status is warranted in ALS, given the common occurrence of dysphagia and hypermetabolism and varying disease progression rates. Nutrition management includes dietary and swallow strategies, possible gastrostomy tube placement, and recommendations for vitamin and mineral supplementation. Strategies to assess and optimize nutrition status and prolong survival in ALS patients are reviewed with recommendations based on current research.

Stüve-Wiedemann syndrome and related bent bone dysplasias.

Stüve-Wiedemann syndrome (SWS) is a severe congenital skeletal dysplasia associated with life threatening dysautonomic manifestations. Newborns affected with this condition exhibit distinctive shortening and bowing of the long bones with reduced bone volume. The majority of affected newborns die early due to neuromuscular complications namely hyperthermia, apnea, and swallowing difficulties. In this review, we provide an overall picture on the clinical, including long-term management, molecular and cellular aspects of SWS and discuss briefly other related bent bone dysplasias.

Immune trigeminal sensory neuropathy with esophageal achalasia: improvement with long-term immunotherapy.

We report a patient who developed subacute facial-predominant numbness and anhidrosis, oral incoordination, and esophageal achalasia with resultant cachexia. Great auricular nerve biopsy showed extensive epineurial perivascular inflammatory infiltrates. Sensation, sweating, and swallowing improved with pulse intravenous methylprednisolone given over 5 years. We suggest that the patient's deficits, including achalasia, were due to an immune-mediated sensory and autonomic neuropathy and that, in such cases, pathologic studies of the great auricular nerve may be diagnostically informative.
