BACKGROUND: Endomyocardial fibrosis is a grim disease. It is the most common restrictive cardiomyopathy worldwide, but the exact etiology and pathogenesis both remain unknown. Endomyocardial fibrosis is recurrently associated with chronic eosinophilia and probable dietary, environmental, and infectious factors, which contribute not only to the onset of the disease (an inflammatory process) but also to its progression and maintenance (endomyocardial damage and scar formation). The trademark of the disease is the fibrotic obliteration of the affected ventricle. The combination of such processes

The results of balloon valvuloplasty as treatment for pulmonary stenosis in 8 adults (6 male) whose ages ranged from 17 to 58 years are presented. Three patients had been subjected to cardiac surgery 32, 40 and 10 years before. The procedure was performed through the right femoral vein using one or two balloons with a diameter of not less than 20 mm, without complications and excellent tolerance. The pulmonary gradient and the right ventricular pressure were reduced in 7 patients from 81.4 +/- 26.9 to 26.6 +/- 11.3 and from 99.3 +/- 25.5 to 47.7 +/- 13.6 mm Hg respectively (p < 0.0005). Chronic constrictive pericarditis and rheumatic valvular heart disease are common surgical conditions, but the simultaneous occurrence of both is extremely rare and poses problems in diagnosis and treatment. A 47-year-old male with constrictive pericarditis and rheumatic valvular heart disease was successfully treated with operation. The valvular dysfunction had included aortic, mitral and tricuspid regurgitations. He had also complicated severe hepatic dysfunction due to the cardiac constriction and the secondary tricuspid valve regurgitation. With the aid of cardiopulmonary bypass radical pe

To detect tricuspid incompetence (TI) right ventricular angiography was performed in 167 patients suffering from moderate to severe mitral valve disease. Holosystolic reflux of contrast medium to the right atrium through the central part of the tricuspid valve was thought to represent true TI, whereas a jet of contrast medium following the injection catheter and originating from the region of its valve passage was assumed to reflect arteficial regurgitation. True TI was found in 35% of the total group (30% mild to moderate, 5% severe TI). TI was often accompanied by atrial fibrillation (91%),

Pulmonary hypertension is a rare disease in neonates, infants, and children, and is associated with substantial morbidity and mortality. An adequate understanding of the controlling pathophysiologic mechanisms is lacking. Moreover, a minority of research is focused specifically on neonatal and pediatric populations. Although therapeutic options have increased over the past several decades, they remain limited. In advanced pulmonary hypertension, progressive pulmonary vascular functional and structural changes ultimately cause increased pulmonary vascular impedance, right-ventricular failure, a

Left ventricular (LV) deformation with ventricular septal shift is one of the most distinctive echocardiographic observations in patients with chronic right ventricular (RV) pressure overload (PO). However, little is known about the effects of RVPO on LV performance and regional synchrony. Accordingly, our objective was to test the hypothesis that chronic RVPO affects regional wall motion, synchronicity, and global LV function using a novel speckle-tracking approach to quantify and characterize regional LV wall motion dyssynchrony. Displacement and strain imaging echocardiographic studies were

HISTORY AND CLINICAL FINDINGS: A 48-year-old patient with dilated cardiomyopathy complained of dyspnea at rest, severe sleeplessness and a slight pain in the stomach. The clinical examination was normal except for a murmur at the apex of the heart. There was no evidence of edema or congestion of the jugular veins.
INVESTIGATION: The echocardiography demonstrated a dilated left ventricle with severely compromised function. No ventricular thrombi were present at this time. Coronary artery disease was excluded by coronary angiography. Endomyocardial biopsies were obtained from the right ventricu

Left ventricular function and motion in 12 adults with an ostium secundum atrial septal defect were analyzed utilizing biplane cineangiography. Values for left ventricular end-diastolic volume index, stroke volume index, ejection fraction, left ventricular end-diastolic pressure and mean rate of circumferential fiber shortening were compared with values in an age-matched group of 11 normal subjects. Comparisons of ventriculographic and echocardiographic data were also made in 5 patients and 10 control subjects. Cardiac index was smaller in patients than in the normal subjects (3.6 vs. 4.5 lite

BACKGROUND: Patients with congenitally corrected transposition of the great arteries (ccTGA) are prone to the development of advanced atrio-ventricular block requiring chronic ventricular pacing. The morphological right ventricle (RV) often develops systolic dysfunction as it is unable to withstand the chronic pressure overload it is exposed to when supporting the systemic circulation.
CASE SUMMARY: A 56-year-old woman with dextrocardia and complex ccTGA with a history of dual-chamber implantable cardioverter-defibrillator (DDD-ICD, high degree atrio-ventricular-block and syncopal ventricular

Complete atrioventricular block (CAVB) is a life-threatening arrhythmia. A small animal model of chronic CAVB that properly reflects clinical indices of bradycardia would accelerate the understanding of disease progression and pathophysiology, and the development of therapeutic strategies. We sought to develop a surgical model of CAVB in adult rats, which could recapitulate structural remodeling and arrhythmogenicity expected in chronic CAVB. Upon right thoracotomy, we delivered electrosurgical energy subepicardially via a thin needle into the atrioventricular node (AVN) region of adult rats t

Current MR techniques enable both anatomical and functional evaluations of the heart. Although it is rarely used as a primary diagnostic tool in pediatric cardiology, it can be used as a valuable adjunct to echocardiography and angiography. MRI is particularly useful in clarification of morphology of complex congenital heart diseases. It is the most accurate method of determination of visceral and atrial situs. It is easy to evaluate the systemic and pulmonary venous connections that are very important for the Fontan type of operation, especially in patients with visceral heterotaxy. It facili

Pulmonary artery intimal sarcoma (PAIS) is a rare intravascular malignancy that can closely resemble pulmonary embolism (PE). A 65-year-old man receiving therapeutic apixaban for atrial fibrillation presented with 3 weeks of progressive exertional dyspnea. Computed tomography pulmonary angiography showed a large unilateral filling defect in the left main pulmonary artery with occlusion of the left upper- and lower-lobe branches, initially interpreted as PE. However, the D-dimer was low, bilateral lower-extremity ultrasonography showed no deep venous thrombosis, cardiac biomarkers were normal,

BACKGROUND: Pulsed-field ablation (PFA) has emerged as a novel energy source for pulmonary vein isolation in atrial fibrillation. Although generally safe, rare complications may occur during catheter manipulation in challenging pulmonary veins.
CASE SUMMARY: A 71-year-old man underwent PFA with a circular-array catheter for paroxysmal atrial fibrillation. Echocardiography showed preserved left ventricular systolic function and a left atrial diameter of 39 mm. The right inferior pulmonary vein (RIPV) measured 15 mm at the ostium but bifurcated immediately, complicating catheter advancement. Du

Partial agenesis of the pericardium is a rare congenital anomaly in which part of the fibroelastic pericardial membrane is absent. Many patients remain asymptomatic, while others present with nonspecific symptoms such as palpitations, chest pain, or dyspnea. The rarity and variable presentation often delay diagnosis. We report a case of a male in his 30s who presented with intermittent palpitations, more pronounced during moderate exertion. Laboratory tests were largely unremarkable except for elevated triglycerides. Electrocardiogram (ECG) showed sinus rhythm with incomplete right bundle bran

Left ventricular (LV) thrombus in patients with severe coronary artery disease (CAD) poses a difficult decision regarding the safety and timing of revascularization. We report a 61-year-old woman presenting with non-traumatic right ankle pain, a consequence of popliteal artery cardioembolism. Laboratory findings included elevated troponin (549 ng/L), white blood cell count (11.9 k/μL), and hemoglobin A1C (11.5%). Echocardiography revealed distal anteroseptal and apical akinesis with associated LV thrombus, and cardiac catheterization demonstrated underlying multivessel CAD. Given the embolic r

We experienced a case of a patient with pulmonary atresia and intact ventricular septum (PAIVS) accompanying sinusoidal communication. PAIVS can be associated with coronary artery anomalies, including sinu- soidal communications from the right ventricle to coronary. In addition, the coronary circulation depends on the blood supply from the right ventricle. The patient was a 1-year-old boy with PAIVS (9.0 kg in weight and 74 cm in height). He had undergone balloon atrial septostomy, central shunt and Glenn procedure. Preopera- tive percutaneous oxygen saturation was 85% which decreased to 80% w

Impaired heparan sulfate (HS) synthesis in vertebrate development causes complex malformations due to the functional disruption of multiple HS-binding growth factors and morphogens. Here, we report developmental heart defects in mice bearing a targeted disruption of the HS-generating enzyme GlcNAc N-deacetylase/GlcN N-sulfotransferase 1 (NDST1), including ventricular septal defects (VSD), persistent truncus arteriosus (PTA), double outlet right ventricle (DORV), and retroesophageal right subclavian artery (RERSC). These defects closely resemble cardiac anomalies observed in mice made deficient
