Management of intracranial and extracranial chordomas with CyberKnife stereotactic radiosurgery.

Chordomas are rare, malignant bone tumors of the axial skeleton, occurring particularly at the cranial base or in the sacro-coccygeal region. Although slow growing, chordomas are locally aggressive and challenging to treat. We evaluate the outcomes of skull base and spinal chordomas in 20 patients treated with CyberKnife (CK) stereotactic radiosurgery (SRS) (Accuray, Sunnyvale, CA, USA) between 1994 and 2010 at Stanford Hospital. There were 12 males and eight females (10-78 years; median age: 51.5 years). Eleven patients received CK as primary adjuvant therapy and nine patients received CK for…

A late complication of CSF shunting: acquired Chiari I malformation.

PURPOSE: Acquired Chiari I malformation developing after cerebrospinal fluid (CSF) shunting is an intriguing late complication of CSF shunt surgery and not only raises questions as to its pathogenesis but also poses many queries about the possible adverse effects on the subsequent child development as well as on the indications and possibilities of surgical correction. MATERIALS AND METHODS: We report a series of 17 patients with the neuroradiological evidence of an "acquired" Chiari type I malformation. These patients, 3 to 24 years (mean 11.3 years), had been treated previously with an extr

Hydrocephalus in infants: the unique biomechanics and why they matter.

OBJECT: Hydrocephalus diagnosed prenatally or in infancy differs substantially from hydrocephalus that develops later in life. The purpose of this review is to explore hydrocephalus that begins before skull closure and full development of the brain. Understanding the unique biomechanics of hydrocephalus beginning very early in life is essential to explain two poorly understood and controversial issues. The first is why is endoscopic third ventriculostomy (ETV) less likely to be successful in premature babies and in infants? The second relates to shunt failure in a subset of older patients trea

Computed tomographic analysis of calvarial hyperostosis in captive lions.

Osseous malformations in the skull and cervical vertebrae of lions in captivity are believed to be caused by hypovitaminosis A. These often lead to severe neurologic abnormalities and may result in death. We describe the characterization of these abnormalities based on computed tomography (CT). CT images of two affected and three healthy lions were compared with define the normal anatomy of the skull and cervical vertebrae and provide information regarding the aforementioned osseous malformations. Because bone structure is influenced by various factors other than the aforementioned disease, al

Prenatal diagnosis of type 2 Pfeiffer syndrome.

Pfeiffer syndrome is an autosomal dominantly inherited disorder consisting of craniosynostosis, a flattened midface with a beaked nose and ocular proptosis, and broad and medially deviated thumbs and great toes. Recently, based on clinical findings, the disorder has been divided into three subtypes: type 1, characterized by mild expression; type 2, in which clover leaf skull deformity and multiple congenital anomalies are present at birth; and type 3, which is similar to type 2, but lacks the presence of the clover leaf skull at birth. We describe a fetus in whom sonographic findings of clover

Modelling of intracranial behaviour on occiput impact in judo.

Acute subdural hematoma (ASDH) sometimes occurred in judo because of the bridging veins' failure by rotation of the brain in the skull. However, the relationship between intracranial behaviour and the motion of the body on occiput impact has not yet been clarified. In this study, we developed an intracranial mechanical model based on multibody dynamics and compared it with experimental results. The results show the importance of modelling bridging veins and cerebral spinal fluid to the relative displacement between brain and skull. The proposed model will contribute to accident analyses or the

Craniofacial speargun injuries: report of three cases, literature review and proposed management guidelines for maxillo-facial surgeons.

Speargun injuries of the head and neck are extremely rare but potentially devastating because of the high density of vital structures in this region. Most available data derive from isolated case reports, and guidance specific to oral and maxillofacial surgeons remains limited. We report three patients with craniofacial speargun injuries managed in a maxillofacial surgery setting, illustrating three distinct scenarios with increasing complexity: an isolated sinonasal trajectory confined to the midface, an anterior cranial fossa trajectory following a submental/oral entry, and a transoral poste

The history of necrotising otitis externa: from clinical observation to conceptual recognition.

OBJECTIVES: Necrotising otitis externa is a severe, potentially life-threatening infection of the external auditory canal and skull base, first formally described by Chandler in 1968. However, historical literature suggests that its clinical features were recognised long before this, without conceptual unification. METHODS: A narrative historical review was undertaken using primary and secondary sources, including early otological texts, case reports and modern literature. RESULTS: Descriptions consistent with necrotising otitis externa appear as early as the nineteenth century, with recurre…

Endoscopic Management of Sinonasal Ossifying Fibroma: Clinical Experience From a Single Institution.

BACKGROUND: Sinonasal ossifying fibroma (OF) is a rare benign fibro-osseous tumor that can cause locally aggressive disease of the paranasal sinuses and skull base. Data on the outcomes after contemporary endoscopic management remain limited. METHODS: We retrospectively reviewed patients with pathologically confirmed sinonasal OF diagnosed at a tertiary referral center. Seven patients were included in this study. Four patients who underwent definitive endoscopic endonasal surgery with complete resection and postoperative follow-up were included in the outcome analyses. RESULTS: The lesions a…

Malignant External Otitis Complicated by Severe Allergic Reaction to Antibiotic Therapy.

Malignant external otitis (MEO), also known as necrotizing otitis externa, is an aggressive infection of the external auditory canal and skull base, typically affecting elderly diabetic or immunocompromised patients. We present a case of an 88-year-old non-diabetic woman diagnosed with MEO whose management was complicated by a severe allergic reaction to systemic antibiotic therapy. The patient initially presented with severe otalgia and purulent otorrhea of the right ear, unresponsive to conventional treatment. Otoscopic examination revealed granulation tissue at the osseocartilaginous juncti…

Radiologically occult nasopharyngeal carcinoma: the importance of clinical suspicion and repeat biopsy.

A man in his 40s undergoing long-term otolaryngology surveillance following an initial diagnosis of a left-sided nasopharyngeal lesion classified as carcinoma in situ re-presented approximately 3 years later with intermittent aural fullness and tinnitus. Flexible nasendoscopy demonstrated subtle unilateral polypoidal change at the left eustachian tube orifice and fossa of Rosenmüller (FOR), while MRI remained non-diagnostic, with no discrete nasopharyngeal mass or skull base abnormality. Given persistent focal endoscopic abnormalities discordant with reassuring imaging, targeted endoscopic bio…

The 9th Version of the AJCC Staging System for Nasopharyngeal Carcinoma: A Guide for Radiologists.

Version 9 of the AJCC staging system for nasopharyngeal carcinoma (NPC) introduces important refinements, emphasizing improved prognostic accuracy, clarity, and clinical relevance. Building on the 8th edition, it refines TNM classifications to address limitations in survival outcome stratification and incorporates new criteria. Specifically, version 9 clarifies the criteria for T3 disease by requiring unequivocal evidence of bone involvement-namely, involvement of the skull base (including the pterygoid plates), cervical vertebrae, or paranasal sinuses. It additionally introduces advanced radi…

A Case of Extensive Pneumocephalus Following an Electric Scooter Accident.

The use of electric scooters (e-scooters) has rapidly increased in Japan, accompanied by a rise in associated injuries. Severe head trauma can occur even when the Glasgow Coma Scale score suggests a mild injury, particularly when major clinical signs are present. Therefore, careful observation and appropriate management are essential. A 42-year-old woman injured by an e-scooter accident was transferred to our hospital with facial swelling and right vision loss. Head computed tomography revealed extensive pneumocephalus, skull base fractures, and traumatic subarachnoid hemorrhage. She subsequen…

A novel missense pathogenic variants of TMEM53 in an Iranian family with craniotubular dysplasia, Ikegawa type.

Craniotubular dysplasia, Ikegawa type (CTDI) is a rare autosomal recessive skeletal dysplasia characterized by hyperostosis of the calvaria and skull base, metadiaphyseal undermodeling of the long tubular bones, and mild shortening and diaphyseal broadening of the short tubular bones. Its causal gene is TMEM53. Six CTDI families have been reported; however, its clinical course and prognosis still remain to be determined. Here, we report two Iranian siblings carrying a novel homozygous missense variant of TMEM53. The affected individuals were referred for progressive severe visual loss of unkno…
