Persistent medial column sagittal mobility can be encountered despite successful first tarsometatarsal stabilization if fixation has been limited to the first tarsometatarsal joint. The purpose of the present cadaveric research was to quantify the effect of a third point of fixation from the base of the first metatarsal to the middle cuneiform compared with the traditional isolated first tarsometatarsal fixation. Ten matched pairs of below-the-knee specimens, with a known cause of death, sex, ethnicity, and age, height, weight, and body mass index at death, were used for our examination. Porta

The treatment of acetabular fractures in the elderly patients remains challenging. The "Gull Sign," which was recently described, was 100% predictive of failure of reduction and/or fixation. However, we believe that adequate reduction can be achieved and lead to good functional outcomes. Our technique differs from classic methods because it uses an anterior intrapelvic approach (the modified Stoppa) to obtain direct reduction of the impacted fragments. Access to the impacted superomedial dome is achieved by mobilizing the quadrilateral fragment, thus allowing direct visualization of the impact

PURPOSE: First tarsometatarsal joint (TMTJ) arthrodesis is known as a powerful operative procedure to correct moderate to severe hallux valgus deformity. However, there is little evidence about actual complication rates and angular correctional power. The aims of the present study were to evaluate the (1) angular correction power and (2) complication rates of pooled data for this procedure and to perform subgroup analysis of different methods of fixation.
METHODS: A systematic search for the MeSH terms "(hallux OR bunion) AND (lapidus OR TMT OR tarsometatarsal OR metatarsocuneiform) AND (fusi

BACKGROUND: U-shaped sacral fractures or Jumper's fractures are rare injuries in adults and are even rarer in the pediatric population. These fractures share a common pathoanatomy where the pelvis as a unit together with the bilateral alar parts and the lower part of the sacrum, loses its skeletal and soft tissue connections to the remaining axial skeleton and hence the term spinopelvic dissociation. This report describes an unusual pattern of spinopelvic dissociation in a young child where the transverse process of the fifth lumbar vertebra was avulsed on one side (spinal side avulsion), wher

BACKGROUND: Two types of implants used for the surgical fixation of extracapsular hip fractures are cephalocondylic intramedullary nails, which are inserted into the femoral canal proximally to distally across the fracture, and extramedullary implants (e.g. the sliding hip screw).
OBJECTIVES: To compare cephalocondylic intramedullary nails with extramedullary implants for extracapsular hip fractures in adults.
SEARCH STRATEGY: We searched the Cochrane Bone, Joint and Muscle Trauma Group Specialised Register (April 2010), The Cochrane Central Register of Controlled Trials (The Cochrane Librar

BACKGROUND: The standard operative treatment for Lisfranc injuries is to openly reduce them and use screw fixation. Due to the current trend of applying a more physiologic fixation and decreasing the need for screw removal, a recent interest has developed in suture-button fixation. The purpose of this study was to compare the suture button technique for Lisfranc fracture fixation to the traditional interfragmentary screw fixation.
MATERIALS AND METHODS: Eight fresh-frozen matched paired cadavers with an average age of 50 (range, 37 to 55) years were tested. For each specimen, the distance bet

OBJECTIVE: To explore the application of the spiral computerized tomography (CT) image three-dimensional (3D) reconstruction technique associated with the conventional radiography in the diagnosis and treatment of severe talar neck fracture.
METHODS: Using the multi-slice spiral CT image 3D reconstruction technique, we analysed 11 cases of talar neck fracture. The fractures were reduced and fixed through a minimal incision and internal fixation with titanium cannulated lag screws.
RESULTS: In the 11 cases, the results of CT image 3D reconstruction were in concordance with plain radiograph in

The proximal humerus internal locking system is an internal fixation device consisting of a low-profile plate and locking screws, which is used for the fixation of three- and four-part fractures of the proximal humerus. We describe a case in which the screws cut out of the humeral head causing injury to the axillary artery necessitating urgent removal of the implant.

LEARNING OBJECTIVES: After studying this article, the participant should be able to: 1. Review the incidence and etiology of mandible fractures. 2. Discuss indications and techniques for closed and open treatment of mandible fractures. 3. Review complications of mandible fractures.
BACKGROUND: Mandible fractures are among the most common types of facial fractures treated by plastic surgeons. They must be managed carefully to maintain the function of the mandible, reestablish proper occlusion, and minimize secondary complications.
METHODS: Current methods of management include combinations of

UNLABELLED: Fracture of the distal radius is a common injury and treatment varies depending on the fracture, patient factors, and surgeon preference. Internal fixation has grown in popularity because it affords increased stability and early motion of the extremity. The desire to provide stable fracture support while minimizing soft tissue dissection and complications has led to the development of new intramedullary implants. These implants use distal screw divergence for subchondral support, fixed-angle screws locked to the implant, and minimally invasive technique to allow early patient rehab

A Salter-Harris type I proximal femoral physeal fracture was diagnosed in a six-month-old Norfolk Terrier. The fracture was reduced using a ventromedial approach to the hip joint and fixation with two small Kirschner wires applied from the joint surface and countersunk below the cartilage. The ventromedial approach minimizes soft-tissue and vascular damage and affords direct visualization of the fracture, facilitating reduction and fixation. The two-year postoperative outcome was considered satisfactory in this case.

BACKGROUND AND AIMS: Osteosynthesis with the proximal femoral nail (PFN) features the advantages of high rotational stability of the head-neck fragment, an unreamed implantation technique and the possibility of static or dynamic distal locking. However, the use of the nail is technically ambitious and is accompanied by some risks of error, which can lead to failure of the osteosynthesis. In this paper we present the results of a critical analysis of mistakes that were made in our hospital during the introduction period of this implant.
PATIENTS AND METHODS: We carried out a prospective analys

The surgical records of three podiatric physicians were reviewed to identify all chevron-type osteotomies performed during 2000-2001 for the correction of bunion deformity, and complications were reviewed for each. In the 95 cases identified, 15 complications occurred in 12 patients. Of these 15 complications, 4 were soft-tissue infections (4 patients), 4 were cases of painful hardware (3 patients), 4 were cases of second metatarsal head pain (2 patients), 2 were cases of cystic changes initially interpreted as osteomyelitis but later determined to be degenerative changes (2 patients), and 1 w

A bioresorbable syndesmotic screw was used successfully for fixation in 4 patients, 3 with a bimalleolar Weber type-C ankle fracture and 1 with a Maisonneuve-type injury. The 5-mm screw consisted of a polyglycolic acid/polylactic acid copolymer placed in standard fashion at the time of open reduction and internal fixation. The patients healed without difficulty, and follow-up radiographs showed anatomic maintenance of the syndesmotic space. There were no resorption problems. Signs of minimal irritation were noted at the screw-head site in 2 patients at 3 to 4 months after surgery, but the irri

INTRODUCTION: The purpose of this study was to evaluate the results of percutaneous fixation of undisplaced or minimally displaced fractures of the scaphoid using the first generation Herbert screw in terms of union, functional results and scaphoid mobility.
METHODS: 30 of the 50 patients operated on in our department between 1995 and 2000 were available for evaluation by an independent observer. Wrist mobility, grip strength and key pinch were measured. Scaphoid mobility was evaluated by measuring radioscaphoid angles in flexed and extended positions on dynamic X-rays.
RESULTS: The union ra

OBJECTIVE: The wide exposure required for a standard posterior lumbar interbody fusion (PLIF) can cause unnecessary trauma to the lumbar musculoligamentous complex. By combining existing microendoscopic, percutaneous instrumentation and interbody technologies, a novel, minimally invasive, percutaneous PLIF technique was developed to minimize such iatrogenic tissue injury (MIP-PLIF).
METHODS: The MIP-PLIF technique was validated in three cadaveric torsos with six motion segments decompressed and fused. Preoperative variables measured from imaging included interpedicular distance, pedicular hei

Thirty consecutive patients with significant cervical spondylotic myelopathy or radiculopathy were treated with anterior decompression, autologous iliac crest graft, and titanium hollow screw plate (THSP). Eight patients had 1 previous surgery, and 1 patient had 2 previous surgeries. Satisfactory fixation was achieved intraoperatively in all instances. At time of final follow-up, graft incorporation was noted in all instances. There were 4 complications: 2 superficial infections, treated nonoperatively; 1 case of dysphagia, which resolved; and 1 case of spasmodic torticollis, which resolved. T

Between 1980 and 1993 81 patients with a combined mandibular body and bilateral mandibular condyle fractures underwent open reduction and internal fixation of the mandibular body. The following methods were used: 29 x miniplate osteosynthesis, 28 x maxiplate osteosynthesis, 13 x lag screws, 10 x combinations and once titanium mesh. Reoperations because of occlusal disturbances were necessary in two cases of miniplate fixation, in one case of maxiplate fixation and in one case of lag screw fixation. In 1994 28 patients were reviewed. Posterior crossbite as operative sequela was noted in 2 (out
