Advanced maxillary medication-related osteonecrosis of the jaw can cause extensive hard and soft tissue destruction that results in long-term oroantral fistulae. The surgical treatment of medication-related osteonecrosis of the jaw may relieve acute symptoms and eliminate the signs of inflammation, but the primary and sustained plastic closure of these defects can challenge both the clinician and the patients. Although the use of obturator prostheses for maxillary defects after ablative oncologic surgery is well documented, studies about this treatment for similar medication-related osteonecro

Cleft lip with or without cleft palate (CLP) and cleft palate only (CP) are severe disruptions affecting orofacial structures. Patients with orofacial clefts require complex interdisciplinary care, which includes nursing, plastic surgery, maxillofacial surgery, otolaryngology, speech therapy, audiology, psychological and genetic counseling, orthodontics and dental treatment, among others. Overall, treatment of clefts of the lip and palate entails a significant economic burden for families and society. Therefore, prevention is the ultimate objective and this will be facilitated by a complete un

Fulminant gangrene of the penis was described by Fournier more than two hundred years ago, however, to this day there are a number of controversies regarding its management. The diagnosis of Fournier's gangrene is essentially clinical, and although laboratory and various imaging techniques can be used, every acute and severe subcutaneous inflammatory process must be considered as the onset of Fournier's gangrene. Immediate treatment is essential and includes the use of combined broad spectrum antibiotics effective against both aerobic and anaerobic bacteria. Intensive supportive care is requir

Epikeratophakia is a simple and reversible form of refractive surgery for the correction of aphakia and myopia and for the treatment of keratoconus, perforation and pterygium. The tissue lenses are attached to the de-epithelialised cornea; no invasion of the central optical zone is required. The safety of this procedure makes it a potential solution to the problem of optical correction in infants and children with congenital and traumatic cataracts. With the advent of commercially prepared tissue lenses, this surgery should become accessible to many ophthalmic surgeons. In the future, the comb

Retrognathia in a patient who has undergone surgery for correction of a cleft palate constitutes a specific indication for transposition osteotomy of the upper jaw. A satisfactory operative technique is described in which as much as possible of the posterior part of the hard palate is retained. The operation is followed by immobilization of the upper jaw with a halo. The results of follow-up examination of 9 patients who had been operated on between 1975 and mid-1977 are described in detail. They revealed a pronounced tendency to postoperative recurrence of the deformity and showed the importa

Thanks to the rapid development of modern science and medicine, limb reconstruction can and should be taken to the next level. The author proposes the concept of "maximum limb reconstruction", which pursues not merely function but the maximal normalization of structure, morphology, and function, leveraging advances in prosthetics, artificial intelligence, and exoskeleton technology to enhance limb preservation. For the three pathological states-defect, infection, and deformity-sequential strategies are outlined: ① mutilating injuries are managed in three phases (early revascularization, mid-te

Dermatologic and reconstructive surgery involves a range of techniques aimed at maximizing tissue preservation while minimizing postoperative functional impairment. Although classical reconstructive methods remain safe and effective, ongoing innovations enable refinement of established approaches and may contribute to improved tissue preservation as well as enhanced functional and aesthetic outcomes. We present two independent cases involving small-to-medium sized primary defects following surgical excision of a zygomatic cutaneous lesion and a lower lip mucosal/cutaneous lesion. Given the ana

Minimizing tension in skin tissue is one of the key factors in reconstructive surgery, as residual stress after defect closure directly affects tissue regeneration and wound healing. To achieve the required skin mobility and thereby reduce tension, careful flap design is essential. Because skin exhibits intrinsic in-plane anisotropy arising from the preferred orientation of collagen fibers in the dermis and its mechanical properties vary across anatomical regions, incorporating skin mechanics into surgical planning is therefore crucial. To investigate the interaction between flap design and sk

BACKGROUND: Submental artery island flap is a pedicled regional flap which is used as one of the quicker reconstructive options for oral cavity defects. However, concerns about its oncological safety have been raised due to the proximity of the submental lymph nodes, which may harbor metastatic deposits. This study aimed to systematically evaluate the oncological safety of the SAIF oral cavity reconstruction.
METHODS: A systematic review and meta-analysis was conducted according to the PICO model. Studies were included if they assessed the oncological safety of the SAIF in oral cavity reconst

Tongue reconstruction requires a series of decisions tailored to patient needs to restore anatomy and preserve speech and swallowing. The impact of these interdependent choices is difficult to evaluate, as clinical outcomes depend on case-specific factors. However, computational analysis offers a method for analyzing these interdependencies in a controlled fashion. The present study systematically quantifies the impact of key surgical decisions, namely donor site selection (radial forearm vs. anterolateral thigh) and flap volume on the final anatomical and biomechanical outcomes. To achieve th

This article introduces an alternative approach to the endoscopic forehead technique that avoids critical anatomical structures encountered in the standard subtemporoparietal fascial plane and allows unfettered access to the orbital ligament. The technique approaches the orbital ligament deep to the deep temporal fascia lateral to the temporal line of fusion. The temporal line of fusion is broken from a lateral to medial direction to communicate with the previously dissected subpericranial plane in the central forehead area. Fasciotomy of the orbital ligament is performed in a lateral to media

Allograft bone block reconstruction for glenoid bone loss has become increasingly used, with favorable clinical outcomes. Among the available donor grafts, the distal tibia allograft remains the most used. Current evidence evaluating alternative grafts, particularly the distal radius, is largely limited to cadaveric, biomechanical, and radiologic studies. Important characteristics, including bone mineral density, radius of curvature, and graft dimensions, have indicated that the distal radius is a promising alternative donor source for glenoid reconstruction. Prospective comparative clinical s

Digital gangrene following septic shock is a debilitating condition that leads to multiple shortened digits and marked functional loss. In symmetric peripheral gangrene (SPG), both hands and feet are affected, eliminating standard reconstructive options such as toe transfers. We present a patient with SPG secondary to septic shock, exacerbated by prolonged vasopressor use, resulting in gangrene of all 4 extremities. A staged reconstructive approach was performed for both hands, utilizing on-top plasties, ray amputation, and local flap rearrangement to restore basic hand function. The initial m

This article reviews the surgical management of severe chronic forearm osteomyelitis using free vascularized fibular flaps (VFFs), with or without the one-bone forearm (OBF) technique. Based on a 22 patient retrospective series, outcomes show that VFF enables reliable reconstruction of extensive bone and soft tissue defects following radical debridement. In selected cases, OBF offers a salvage solution for both radius and ulna bone loss. The approach led to high infection control rates, bone healing, and functional recovery. Microsurgical reconstruction, particularly using VFF, remains the gol

BACKGROUND: Endoscopic skull base surgery is widely used for various skull base pathologies. The nasoseptal flap (NSF) is considered the gold-standard reconstructive technique to reduce postoperative cerebrospinal fluid (CSF) leaks. However, its impact on olfactory function remains unclear.
OBJECTIVE: To systematically compare olfactory outcomes between NSF and no-NSF reconstruction following skull base surgery.
METHODS: PubMed, Embase, and Cochrane databases were searched for randomized controlled trials and observational studies comparing olfactory function in patients undergoing skull bas

Post-craniotomy infections are serious complications that often require complex surgical decision-making. The optimal management strategy-whether to retain the infected bone flap with antibiotics or remove it and perform mesh cranioplasty-remains unclear. We conducted a systematic review and meta-analysis of studies reporting outcomes for patients with post-craniotomy infections managed with either bone flap retention and antibiotic therapy or bone flap removal followed by immediate mesh cranioplasty. Outcomes of interest included treatment failure, postoperative complications, and mortality.

INTRODUCTION: The management of extremity soft tissue sarcomas (STS) involving major vessels presents unique challenges, historically leading to amputation. Advances in vascular reconstruction have enabled limb-sparing surgery (LSS), but outcomes and perioperative risks remain uncertain. This systematic review and meta-analysis aimed to evaluate oncologic results following LSS with vascular reconstruction in extremity STS.
METHODS: A systematic review and single-arm meta-analysis were performed according to PRISMA guidelines, with registration in PROSPERO. PubMed, Embase, and Cochrane Library
