Antiretroviral drugs for pre-exposure prophylaxis of HIV infection.

Though a large number of antiretrovirals have been developed for the treatment of HIV infection, new HIV infections continue to occur, especially among certain high-risk groups, such as men who have sex with men. Overall, the current estimated incidence of HIV infection is 2.5-fold higher than the number of individuals that begin antiretroviral therapy every year worldwide. Along with the personal drama caused by the diagnosis of HIV infection, other considerations in society, including economics and interpersonal relationships, make the need for HIV prevention strategies a priority. Though va

Evaluating the competing risks of HIV acquisition and maternal mortality in Africa: a decision analysis.

OBJECTIVE: To model the risk of HIV acquisition and maternal mortality for women in four African countries in the light of previous data on risk of HIV acquisition and hormonal contraceptive use. DESIGN: Decision analysis. SETTING: Chad, Kenya, South Africa and Uganda. POPULATION: Women of reproductive age, at risk of HIV, who do not desire pregnancy. METHODS: A decision analysis model was built to compare the consequences of removing progestin injectables from use, assuming an increased risk of HIV acquisition. Three scenarios were considered in four African countries: replacement of prog

HIV-related needs for safety among male-to-female transsexuals (mak nyah) in Malaysia.

This research, commissioned by the Malaysian AIDS Council in 2007, is qualitative and descriptive in nature. In depth face-to-face interviews were carried out with 15 mak nyah respondents from five major towns. The interviews were guided by an interview schedule that had seven main topics: brief background; hormone-taking behaviour; safe sex; health care; substance abuse; harassment from authorities; and HIV prevention. The HIV problem among the mak nyah, mak nyah sex workers and their clients is critical. Many do not have in-depth HIV/AIDS knowledge and do not practise safe sex. The problem g

Targeting HIV-related outcomes with intravenous drug users maintained on methadone: a randomized clinical trial of a harm reduction group therapy.

Methadone maintenance programs (MMP) have the potential to play an important role in reducing HIV risk, given the appropriate type and level of ancillary treatments. In this study, we investigated the efficacy of a 12-session harm reduction group intervention for injection drug users, based upon the Information-Motivation-Behavioral skills model of behavior change, that focused on reducing both drug and sex risk. Two hundred and twenty patients entering an MMP were randomized to receive either standard care (SC)-2 hours of counseling per month and a single-session risk reduction intervention-o

A transitional model of barrier methods compliance with unbalanced loss to follow-up.

Differential loss to follow-up can substantially influence the evaluation of treatment effects on a dichotomous outcome of interest in longitudinal trials. The use of transitional models incorporating loss to follow-up as an additional category of response and the nature of the correlated responses can provide a comprehensive view of a trial with unbalanced loss to follow-up. Under the Markov assumption, transitional models estimate the probability of changing from one outcome to another outcome between follow-up visits. Patterns of the response variable can be described by the estimated trans

The challenge of HIV prevention among high-risk adolescents.

This article reports findings from an exploratory study of HIV knowledge and risk behaviors among 60 teenagers and young men engaged in the street life of Hollywood, California. The sample was composed largely of youths of homosexual or bisexual orientation who were substance abusers, prostitutes, or both. The data suggest that although community-based education efforts may be associated with lower-risk behavior among this population, the overall risk profiles of these socially marginalized youths remained high. Inferences are drawn about the cofactors of risk that must be addressed and the ed

Increasing frequency of heterosexually transmitted AIDS in southern Florida: artifact or reality?

Acquired immunodeficiency syndrome (AIDS) cases reported as the result of heterosexual contact have been increasing in the United States, with Florida reporting a disproportionate number. We investigated 168 such AIDS cases from southern Florida. After follow-up, 50 (30%) patients were reclassified into other transmission categories. The data suggest that the increased rate of heterosexually acquired AIDS cases reported from southern Florida was partially related to misclassification of risk.

Impact of reduced chlamydia testing on STI testing and provider preferences in the Netherlands: an experimental vignette study.

OBJECTIVES: The restrictive chlamydia testing guidelines for asymptomatic individuals at Dutch sexual health centres (SHCs) since January 2025 may influence perceptions and attitudes towards testing for sexually transmitted infections (STIs). This study aimed to assess the potential impact of this guideline change on STI testing preferences among young people and to examine determinants and underlying motivations of these preferences. METHODS: Heterosexuals aged 16-34 in the Netherlands, recruited via social media and SHCs, completed an online survey (April-June 2024). STI testing preferences

Increasing urogenital and oropharyngeal gonorrhoea positivity in women attending a sexual health service in Melbourne, Australia, 2015-2024.

BACKGROUND: Gonorrhoea notifications among women have increased, but the determinants of this trend are unclear. We aimed to examine temporal trends in gonorrhoea positivity and associated clinical characteristics among women attending a major sexual health clinic in Melbourne, Australia between 2015 and 2024. METHOD: We conducted a retrospective analysis of women diagnosed with gonorrhoea between 2015 and 2024. Demographic, behavioural and clinical data were extracted from routine clinical records, including sexual practices, sex work status and condom use. A chart review of a subset of posi

Knowledge and utilization of family planning and safe abortion services among married women of reproductive age in the Raute community of Nepal: A census-based cross-sectional study.

Raute the last nomadic hunter-gatherer marginalized indigenous group face disproportionate and persistent barriers to reproductive health services, including family planning and safe abortion. The lack of the pre-existing population-level quantitative reproductive health data limits the ability of health authority to design cultural-sensitive health services. This census-based cross-sectional study enrolled all 192 eligible married women of reproductive age in the Raute community of Parshuram Municipality, Dadeldhura district, Sudurpaschim Province, Nepal. Data was collected through structured

Phenotypes of successful living among adolescents with HIV in Rwanda: a latent profile analysis.

BACKGROUND: HIV remains a major public health concern globally, and 85% of adolescents with HIV (AWH) live in Sub-Saharan Africa (SSA). Despite advances in treatment and prevention efforts, adolescents continue to be disproportionately affected by HIV and are a group with poor adherence to antiretroviral medication. We identified phenotypes of successful living among adolescents with HIV in Rwanda. METHODS: A multi-stage stratified random sampling was used to collect data from 300 AWH using a validated and piloted questionnaire from four provinces and Kigali City in Rwanda. Data were collecte

Brief Report: Sexual Partnership Heterogeneity and HIV Seropositivity Among Adolescent Girls and Young Women in Cameroon and Côte d'Ivoire: A Partner-Level Latent Class Analysis.

BACKGROUND: Adolescent girls' and young women's (AGYW) heightened HIV vulnerability has been understudied in West and Central Africa, where AGYW account for 1 in 5 new HIV diagnoses. Identifying contextually specific drivers of AGYW's HIV risk can help tailor HIV prevention programming to AGYW in the region. METHODS: We pooled data from nationally representative HIV-seroprevalence surveys for sexually active AGYW in Cameroon and Côte d'Ivoire. We used latent class analysis to partition past-year sexual partnerships into discrete typologies based on 6 relationship characteristics: cohabitation

Willingness of female sex workers to use HIV self-testing in Burkina Faso: analysis of cross-sectional respondent-driven sampling data.

BACKGROUND: HIV self-testing (HIVST) is an approach that allows individuals to test for HIV and obtain results in their homes or other private settings. It is recommended by the World Health Organization (WHO), particularly for key populations, to help achieve the first 95. The objective of this study was to evaluate the willingness to use HIVST among female sex workers (FSWs) in Burkina Faso. METHODS: We performed a biobehavioral cross-sectional study conducted in five regions in Burkina Faso. FSWs were recruited using the respondent-driven sampling (RDS) method. To evaluate FSW's willingnes

Machine learning model for predicting hepatitis C seroconversion in methadone maintenance patients in China.

INTRODUCTION: Hepatitis C virus (HCV) infection is a substantial public health concern, particularly among individuals with opioid addiction. The methadone maintenance treatment (MMT) programmes serve as a harm reduction strategy to mitigate HIV disease spread, yet the risk of HCV infection remains high within these settings. Accurate risk prediction for HCV seroconversion is therefore crucial for improving patient outcomes. METHODS: We collected data from 1547 individuals with opioid use disorder who entered the MMT programme from May 2005 to October 2023 in Shanghai, China, and 283 individu

Contextual barriers to PrEP uptake and continuation among young Black gay and bisexual men who have sex with men living in the South.

Young, Black Gay, and Bisexual men who have sex with men (YBGBM) are disproportionately impacted by HIV, especially in Southern United States. We conducted a cross-sectional survey (Feb19-Mar20). Eligibility criteria were self-reported age 16-29 years, HIV-negative, Black race, and cis-gender male. We assessed associations between demographics, religiosity, intersectional stigma, and pre-exposure prophylaxis (PrEP) use (never, previous or current) among YBGBM in Alabama. Univariate and multivariable multinomial logistic regression models were fit with factors selected a priori, guided by a con
