Dexmedetomidine nasal administration improves perioperative sleep quality and neurocognitive deficits in elderly patients undergoing general anesthesia.

OBJECTIVE: To investigate the improvement of perioperative sleep quality and neurocognitive impairment in elderly patients under general anesthesia by nasal administration of dexmedetomidine. METHODS: One hundred and twenty patients admitted to our hospital for various laparoscopic elective gynecological surgeries lasting more than 1 h under general anesthesia from July 2021 to March 2023 were selected. All subjects were divided into 3 groups according to the random number table method. From 21:00 to 21:30 every night from one day before to 5 days after surgery, group A was given alprazolam 0

Lung ultrasonography to assess pulmonary complications after head and neck cancer surgery.

OBJECTIVES: Postoperative pulmonary complications (PPCs) are common after major head and neck cancer surgery. This trial evaluated whether adding daily lung ultrasonography (LUS) to the current diagnostic strategy (clinical evaluation with on-demand chest X-ray [CXR]) improves PPC detection. METHODS: In this randomized controlled trial, 196 adults at intermediate/high risk for PPCs undergoing head and neck cancer surgery with free-flap reconstruction were assigned to the current strategy alone or a routine strategy (daily LUS plus current strategy). The primary outcome was the detection rate

The Anesthetic Management of a Patient With Advanced Heart Failure Undergoing Non-cardiac Surgery.

Severe heart failure (HF) is associated with high perioperative morbidity and mortality during non-cardiac surgery. Anesthetic management for patients with critically reduced left ventricular ejection fraction (LVEF) requires strategies that minimize myocardial depression and autonomic instability. We present the case of a 73-year-old male with severe HF (LVEF 18%), a permanent pacemaker, severe anemia, and ongoing clopidogrel therapy who required urgent fixation of an open distal humerus fracture. General anesthesia was considered prohibitively high risk. An ultrasound-guided supraclavicular

Keratometry, biometry, and intraocular lens power calculation in Bornean sun bears (Helarctos malayanus euryspilus).

BACKGROUND: Cataracts are a common ocular condition that affects captive sun bears, yet there is a lack of detailed case reports and ocular studies of this species. AIM: The examinations aim to calculate the intraocular lens (IOL) power required to achieve emmetropia following cataract surgery. METHODS: Five Bornean sun bears diagnosed with cataracts at the Bornean Sun Bear Conservation Centre in Malaysia underwent ocular examination under general anesthesia. Keratometry and biometry data, including anterior chamber depth, crystalline lens thickness, and axial globe length, were collected us

Headache resolution following the middle fossa repair of superior semicircular canal dehiscence.

OBJECTIVE: To investigate predictors of headache resolution following surgical intervention for superior semicircular canal dehiscence (SSCD) syndrome. METHOD: We conducted a retrospective chart review on 344 surgical repairs of SSCD via the middle cranial fossa (MCF) approach conducted at an institution. The primary outcome measure was the resolution of headaches after SSCD repair. Analyses explored the predictive potential of several comorbidities and lifestyle factors pre-operatively. RESULTS: Among a total of 344 repairs, 193 (56 %) reported headaches preoperatively. Among which, 114 (59

Perioperative Risk in Adults with Congenital Heart Disease Undergoing Non-Cardiac Surgery: Challenges and Tailored Strategies.

Advances in surgical and medical management of congenital heart disease have improved survival rates, leading to a growing population of adult congenital heart disease (ACHD) patients requiring specialized perioperative care. Studies indicate that ACHD patients undergoing non-cardiac surgery (NC surgery) have increased mortality and morbidity risks compared to the general population, with complication rates particularly high in those with complex defects, such as Fontan circulation, Eisenmenger syndrome, or cyanotic congenital heart disease. Key perioperative concerns include hemodynamic insta

Perioperative anesthesia management of remote 5G robot surgery and precautions for operation team.

Remote 5G robot surgery not only improves the accuracy, accessibility and efficiency of surgery, but also promotes the remote sharing and integration of medical resources, effectively avoids the risk of transshipment, reduces the cost of remote medical treatment, and promotes the development of smart medical care. To successfully carry out remote 5G surgery, it is necessary to establish a medical team, and anesthesiologists should not only escort the operation but also find problems in time when accidents occur and coordinate the team for joint treatment. General anesthesia is selected for thi

Mortality 30, 60, and 90 Days After Discharge Is Greater in Patients Who Experienced Postoperative Respiratory Depression and Pulmonary Complication.

Upon the induction of general anesthesia, a predictable sequence of physiological changes occurs within the respiratory and neuromuscular systems. The sequelae of these changes include an assortment of postoperative pulmonary complications (PPCs), including postoperative respiratory depression (PORD), that are observed during the immediate postoperative period and in the post-anesthesia care unit (PACU). These adverse events are anticipated, because several of the drugs that are used during surgery (e.g., opioids, which are traditionally used to manage pain during and after surgery), albeit th

Effects of body mass index on mortality in elderly patients with hip fractures.

Hip fractures remain a substantial health concern, particularly among elderly individuals with osteoporosis, leading to high global mortality rates. This study aimed to analyze the association between body mass index (BMI) and postoperative mortality in patients who underwent surgery for hip fractures. A total of 680 patients treated at a single institution between January 2018 and December 2022 were included. Factors such as age, BMI, sex, Charlson Comorbidity Index (CCI), preoperative hemoglobin levels, American Society of Anesthesiologists score, anesthesia method, duration of surgery, and

Respiratory distress associated with acute hydrothorax during transurethral electrocoagulation: a case report.

BACKGROUND: In patients undergoing abdominal radiotherapy or transurethral surgery, bladder perforations are a possible complication. Likewise, pleural effusions due to a pleuroperitoneal leak caused by either a congenital or acquired diaphragmatic defect can also occur. We report a case in which a saline solution, which migrated into the abdominal cavity from a bladder perforation during transurethral electrocoagulation, further formed bilateral pleural effusions and caused rapid ventilation failure. CASE PRESENTATION: A patient undergoing radiation therapy and hormone therapy for prostate c

[A Case of Recurrent Colon Cancer with Long-Term Complete Response Treated with FOLFOX Therapy].

A 69-year-old female was admitted to our hospital due to general malaise and appetite loss. On physical examination, a tumor was palpable in the upper left abdomen. A complete blood count revealed anemia. An abdominal computed tomography suggested ileus secondary to transverse colon cancer. Transverse colectomy with D3 lymph node dissection was performed. The final diagnosis was moderately differentiated tubular adenocarcinoma, StageⅡ, T4aN0M0. The tumor marker level was re-elevated at 9 months after surgery; computed tomography and lower gastrointestinal endoscopy indicated local recurrence.

Occipital Hairline: A forgotten area.

Hair restoration surgery is one of the fascinating fields of modern day cosmetic surgery. A good hair transplant is characterized by a naturalness in terms of strategic distribution of hair. A very important point which is very rarely taken care is the "posterior or occipital" hairline. It is formed by the posterior 1 cm of hair which are placed on the vertex or crown and constitute the part which is normally hidden by the long hair. Many surgeons while creating the slits in the recipient area forget this equally important point. And make the slits in the lines. As the care is taken while crea

Repair of a Tibiotarsal Fracture in a Pomeranian Goose ( Anser anser) With a Locking Plate.

A 2-year-old Pomeranian goose ( Anser anser) weighing 8.1 kg was examined because of non-weight-bearing lameness of the right limb. A closed, transverse, diaphyseal fracture of the distal third of the right tibiotarsus with a craniolateral displacement of the distal fragment was diagnosed radiographically. Surgery under general anesthesia was performed to repair the fracture with a 14-hole, 2.7-mm locking plate fixed with 6 screws in a bicortical manner. Two days later, the bird was fully weight-bearing on the leg. Radiographs performed 4 and 8 weeks after surgery showed good healing of the af

Current treatment options for meningioma.

With an annual incidence of 5/100,000, meningioma is the most frequent primary tumor of the central nervous system. Risk factors are radiotherapy and hormone intake. Most meningiomas are grade I benign tumors, but up to 15% are atypical and 2% anaplastic according to the WHO 2016 histological criteria. Areas covered: This review details the current standard therapy based on international guidelines and recent literature, and describes new approaches developed to treat refractory cases. First-line treatments are observation and surgery, but adjuvant radiotherapy/radiosurgery is discussed for at

Malignant Hyperthermia: Report of Two Cases with a Neglected Complication in Cardiac Surgery.

Malignant hyperthermia (MH) can develop after contact with volatile anesthetics (halothane, enflurane, isoflurane, sevoflurane, and desflurane) as well as succinylcholine and cause hypermetabolism during anesthesia, which is associated with high mortality when untreated. Early diagnosis and treatment could be life-saving. During cardiac surgery, hypothermia and cardiopulmonary bypass make the diagnosis of MH extremely challenging compared with other settings such as general surgery. We herein report 2 cases of MH, graded as "very likely" or "almost certain" based on the MH clinical grading sca…
