Reconstruction of the hip stump.

The formation of a functional limb stump after combat injuries with extensive soft tissue damage is challenging. After amputations or revision surgeries, long femoral stumps may remain. Although functionally inferior to tibial stumps, they can still provide satisfactory movement and performance. However, prolonged prosthesis use or poor-quality muscle grafting can lead to complications, such as skin breakdown, trophic issues, and inability to continue prosthetic use. These problems are partly due to the mismatch between the area of the bone stump end and the applied body weight. Ilizarov compr

Current Management of Craniosynostosis: A Nordic Pediatric Neurosurgery Network Study.

Historically, management of craniosynostosis (CS) has been marked by substantial variation. Although recent advances in diagnostics, minimally invasive techniques, and computer-assisted planning might be expected to promote greater uniformity, significant differences persist, as shown in a large US survey of current practice patterns. To clarify the extent of heterogeneity in the Nordic countries (28 million inhabitants), the authors conducted a comprehensive survey of all centers treating CS in the region. All 11 centers responded (100%). Marked heterogeneity was evident across organizational

Associations of Hospital-Level and Regional Factors With Receipt of Endoscopic Repair for Nonsyndromic Craniosynostosis.

BACKGROUND: Endoscopic strip craniectomy represents an alternative to open cranial vault remodeling for repair of craniosynostosis. In addition to individual sociodemographic factors, hospital-level variables may affect the reception of such endoscopic procedures. This study incorporates a large national database to identify hospital-level and regional determinants of endoscopic repair for nonsyndromic craniosynostosis. METHODS: Open cranial vault remodeling and endoscopic strip craniectomy surgeries were identified in the 2016-2022 National Inpatient Sample. Admissions were characterized by

Ischial Pressure Ulcer Reconstruction: A Critical Review of the Paradigm Shift From Wound Coverage to Functional Restoration.

Ischial pressure ulcers (IPUs), severe complications in patients with prolonged immobility, are challenging due to high recurrence. Moving beyond anatomical closure, the field now prioritises functional-integrated reconstruction. This paradigm emphasises radical debridement and the transfer of well-vascularized perforator flaps to obliterate dead space, cover wounds, and restore protective sensation, aiming to break the recurrence cycle and restore patient autonomy. This review critically synthesises contemporary advances driving this shift. It details the development of precision and minimall

Robotics in Pediatric Surgery: Expanding Indications and Techniques.

Pediatric lower extremity reconstruction, especially following trauma or oncologic resection, presents unique challenges requiring a multidisciplinary approach. Management emphasizes early, thorough debridement, appropriate fixation-often with flexible intramedullary nails-and timely, well-vascularized soft tissue coverage. Nerve injuries are addressed with autografts or allografts, with children demonstrating better regenerative capacity than adults. Postoperative protocols focus on immobilization, pain management, psychosocial support, and structured rehabilitation, with family involvement c

Regenerative Potential of Human Umbilical Cord Vein for Urethral Reconstruction in Male Rabbit Model.

Tissue engineering offers an alternative for augmentation urethroplasty; however, no ideal material has yet been developed. Recently, materials derived from amniotic tissues appear to exhibit promising properties. Herein, the aim of this study was to provide a proof of concept for the integration of the human umbilical cord vein for urethral reconstructions in rabbits. Rabbits were included in two groups; the control group underwent urethral reconstruction using autograft urethral tissue, and the test group received xenograft tissue (umbilical cord vein) after creating a 1 × 1 cm defect in the

Rate of fistula repair following palatoplasty in patients with cleft lip and palate: A systematic review of the literature.

BACKGROUND: Oronasal fistulas are among the most common and burdensome complications of cleft palate repair. Despite exhaustive literature detailing the nature and incidence of palatal fistulas, the reported rates of surgical fistula repair remain contested. The aim of this study was to identify the rate of palatal fistula repair following primary palatoplasty for cleft lip and palate. METHODS: The PubMed, Scopus, and Cochrane databases were queried for studies that focused on the incidence of surgical fistula repair following primary palatoplasty. Posterior auricular seborrheic keratosis treated with bilobed flap reconstruction: a case report.

BACKGROUND: Seborrheic keratosis is a common benign epidermal neoplasm, typically arising on the trunk, face, and extremities. Auricular involvement is rare, and posterior auricular presentation is particularly uncommon, with few cases reported. CASE PRESENTATION: We describe a 68-year-old Iranian woman presenting with a 3 cm × 2 cm posterior auricular mass that had slowly enlarged over 5 years. The lesion was firm, nontender, and fixed to underlying tissue, raising concern for malignancy. A preoperative biopsy was performed, and histopathology confirmed seborrheic keratosis. Given the lesion

Acellular Dermal Substitutes for Lining the Exenterated Orbit-A Systematic Review and Meta-Analysis of Clinical Outcomes.

Orbital exenteration reconstruction remains a major reconstructive challenge because of the orbit's complex anatomy and its close relationship with the paranasal sinuses, which predispose patients to delayed healing. Traditional approaches, such as secondary intention healing or split-thickness skin grafting, may result in prolonged epithelialization and can compromise timely prosthetic rehabilitation. Acellular dermal matrices (ADM) have emerged as biological scaffolds that provide immediate coverage of the defect while preserving an open orbital cavity suitable for prosthetic fitting. A syst

Soft tissue augmentation options for volume replacement following parotidectomy: A review of the literature.

BACKGROUND: Salivary gland tumors primarily arise in the parotid gland with surgery representing the cornerstone of treatment. Parotidectomies often lead to a soft tissue deficit that may benefit from reconstruction to reduce incidence of Frey's syndrome, improve aesthetic outcomes, and allow for appropriate postoperative therapies. Several options exist including avascular tissue transfer, locoregional tissue flaps, and microvascular free flaps. METHODS: This paper is a literature review exploring the efficacy of the various reconstructive options, and includes randomized control trials, pro

Microsurgical Reconstruction of the Columella With a First Web Space Free Flap: A Case Report and Review of Techniques.

Reconstruction of the columella, one of the smallest facial subunits, presents significant aesthetic and functional challenges due to its complex anatomy, limited local tissue for repair, and restricted vascularisation. Although many methods are described, literature on microsurgical solutions for complex secondary reconstructions, especially after conventional methods have failed, remains limited. This report introduces a tailored, single-stage approach that combines a first web space free flap from the foot with an iliac bone graft to provide both soft tissue coverage and structural support,

Pedicled lateral nasal wall flap for endoscopic skull base reconstruction: how I do it.

BACKGROUND: The pedicled nasoseptal flap (Hadad-Bassagasteguy flap, HBF) has been considered as the workhorse for reconstruction of high-flow cerebrospinal fluid (CSF) leak following endoscopic endonasal skull base surgery. However, if the septal mucosa is affected by a tumor or the vascular pedicle is injured due to prior surgery, the use of HBF is precluded. METHOD: We illustrate the harvest techniques of the lateral nasal wall flap (LNWF) based on the posterior lateral nasal artery (PLNA). CONCLUSION: The LNWF is a robust intranasal regional flap with a consistent blood supply and compara

Turn-in and epispadias repair as the first step of 2-stage bladder exstrophy repair - A preliminary report.

BACKGROUND: We report Bladder Turn-in & Epispadias Repair (TIER) as a first step of two-staged Bladder Exstrophy repair. TECHNIQUE: Circumferential incision was made around the bladder and urethral plate. Penis was degloved and corporal bodies were mobilised. The epispadiac urethra and spongiosa were dissected off the corporal bodies leaving the frenulum and prepuce attached to hemi-glans. Perineal muscle was identified by muscle stimulator and wrapped around the bladder neck. Bladder was closed leaving reimplantation/bladder neck repair to second stage. The neourethra was tubularised and mea

Clitoral reconstructive surgery for patients with vulvar carcinoma.

BACKGROUND: Vulvar carcinoma involving the clitoral region often necessitates partial or complete excision of the clitoris, with profound psychosexual and functional impact. While vulvar reconstruction using flaps is standard practice, clitoral reconstruction remains uncommon. We describe a technique to reconstruct both anatomically and functionally the clitoris, incorporating the Foldès method, originally developed for reconstruction after female genital mutilation/cutting (FGM/C). OBJECTIVE: To give an anatomical overview of the clitoris and describe the anatomical foundations and surgical
