A retrospective review of 209 children with spina bifida was performed to determine predictive factors for the development of hydronephrotic changes and to assess the outcome of management. Renal sonography demonstrated hydronephrosis in 100 cases (48%). Two risk factors were identified: level of spinal defect and age of the patient. The widespread use of obstetric and neonatal sonography has resulted an increasing number of neonatal hydronephrosis, and some of them are asymptomatic. From November 1987 to May 1990 49 neonates with hydronephrosis or multicystic dysplastic kidney were seen. The most common conditions found were obstruction of the distal ureter (10), multicystic dysplatic kidney (12). The incidence of crossed "fused" renal ectopia has been reported at 1 in 7,500 autopsies, an incidence approximately ten times more frequent than crossed renal ectopia without fusion. Most cases remain asymptomatic and are incidentally encountered on autopsy or during routine workup for unrelated disorders. We herein report a case of ureteropelvic junction obstruction secondary to a high ureteral insertion in the pelvis of a crossed unfused ectopic kidney.

Partial obstruction of the left ureter was created in rats less than 36 h of age and its effects were studied at the age of 2-6 days. A considerable hydronephrosis with distension of the kidney developed within 1 day after obstruction, but there was no parenchymal weight reduction. We report a typical case of traumatic ruptured hydronephrosis (moderate trauma, unknown hydronephrosis). Conservative treatment with pyeloplasty appears to be the best solution in these cases when as is frequently the case the major lesions affect the renal pelvis.

The clinical presentation and management of a young child with renal failure caused by bilharzial obstructive uropathy is discussed. Children from areas in which bilharzia is endemic have a high incidence of urological disease. Ureteric obstruction from inflammatory reaction will respond to medical treatment, whereas true mechanical obstruction from fibrosis will require surgical intervention.

Ultrasound monitoring of the urinary tract was performed three times in 158 pregnant women (before the 20th week, between the 20th and 30th weeks, and after the 30th week). Dilatation of the kidney was found in 82 women (51.9 percent). All patients with pyelitis gravidarum were found to have dilatation of the kidney. Patients with hydronephrosis are a high-risk group for infection of the urinary tract. Recurrences are common among these women.

Thirteen cases of fetal hydronephrosis associated initially with normal volumes of amniotic fluid were diagnosed by maternal sonography. No invasive procedures were done in utero. Postnatal evaluation demonstrated 8 neonates with urinary tract obstruction and 5 normal neonates. Long-term followup of these 13 patients revealed that 12 are healthy and have normal renal function. The use of ultrasonography in the diagnosis of upper abdominal masses with nonfunctioning kidneys is described. Two cases of congential hydronephrosis due to obstruction of the pelviuretic junction are presented, both of which were diagnosed by B-scan ultrasound examination. Diagnostic ultrasound is a simple, noninvasive and reliable method of differentiating cystic from solid lesions and of assessing whether or not a mass is renal or extrarenal. It is recommended that all patients seen with an upper abdominal mass and/or nonfunctioning kidney be examined by ultrasound.
