PURPOSE: To report the clinical appearance and course of intrastromal clefts occurring with acute hydrops in keratoconus.
METHODS: In eight patients with bilateral keratoconus, nine eyes developed acute corneal hydrops complicated by intrastromal cleft formation. One patient developed this complication in both eyes. Psychological testing of 63 normal subjects and 84 patients with keratocone was carried out separately for men and women aged 18-64 years using 11 scores of a brief multifactorial questionnaire for personality examination (BMQP). Comparison of averaged profiles of examined men and women with keratocone showed features of sensitive and psychasthenic personalities. According to BMQP scores, the population of men with keratocone is characterized by increased levels of psychasthenia and schizophrenia, whereas women develop depression, psychasthenia, and schizophrenia.

Keratoconus is a corneal dystrophy that degrades the optical function of the cornea. The onset of the process manifests optical signs: evolving astigmatism, failure of optical correction by spectacles, and distorted images. We studied two sets of identical twins with keratoconus. The first 21-year-old pair had bilateral fully developed keratoconus and the second 35-year-old pair presented with early bilateral keratoconus detected by videokeratography. Epikeratoplasty (EKP) in keratoconus generally has few intraoperative complications. Perforations during EKP are rare but may force the surgeon to abandon the procedure. We developed a special suture technique to manage this problem. Keratoconus is a common corneal dystrophy. In order to characterize an Australian experience with this condition, a retrospective study of 295 consecutive cases of keratoconus presenting to one Brisbane ophthalmologist (GR) was performed. The majority (82%) of patients presented before the age of 40 years. Keratoconus was bilateral in 240 (81%) patients and unilateral in 55 (19%) patients. PURPOSE: Present videokeratographs provide corneal radius of curvature measurements in terms of axial (sagittal) radius and, for some instruments, instantaneous (tangential) radius. Ninety-nine eyes with keratoconus of 64 patients were examined using the sagittal radii method to determine the corneal flattening and compared with 100 eyes of 50 healthy controls. To express the degree of this flattening, four numerical eccentricity (E) values for each eye were calculated. The highest E value of the four was chosen as a parameter for diagnosis of the disease. Between 1982 and 1990, penetrating keratoplasties for keratoconus were performed in 9 eyes of 7 patients (age 17-41 years) with Down's syndrome. All eyes showed dense scarring of the central cornea. Follow-up was 10-49 months (mean 28 months). Preoperative corrected visual acuity was hand movements to 20/400.
